Úlceras venosas: caracterização clínica e tratamento em usuários atendidos em rede ambulatorial

Ulcera venosa representa un desafio para los profesionales de salud, siendo una lesion cronica, recurrente y con impacto biopsicosocial. El objetivo de este estudio fue caracterizar las ulceras venosas de los usuarios atendidos en salas de curativos de la red municipal de salud. Esta pesquisa descriptiva, realizada entre octubre 2009 y julio 2010 incluyo a 58 personas. Los datos fueron recolectados mediante protocolo que incluyo entrevista, examen fisico, aplicacion de escala, registro fotografico de la lesion y planigraphia. Los resultados mostraron lesiones con mala cicatrizacion y tratamiento con violacion de las principales recomendaciones de la literatura en el area. Se concluye que es necesario repensar la organizacion de los servicios para servir mejor a esta poblacion.Venous ulcer represents a challenge for healths professionals, because its a chronic injury, recurrent and with biologic and psychosocial impact. The aim of this study was to characterize the venous ulcers of outpatients attended in curative rooms of the municipal health network and describe the treatment received. This descriptive study, carried out from October 2009 to July 2010 included 58 people. Data were collected using protocol that involved an interview, physical examination, application of scale, photographic record of the lesion and planigraphy. The results showed lesions with poor healing and treatment in violation of the main recommendations of the literature in the area. We conclude that it is necessary to rethink the organization of services to better serve this population.

Pessoas com úlceras vasculogênicas em atendimento ambulatorial de enfermagem: estudo das variáveis clínicas e sociodemográficas

The aim of this study was to analyze the sociodemographic and clinical characteristics of people with vascular ulcers and to investigate the association between these variables. This cross-sectional, observational clinical study was conducted in outpatient clinics from February to August 2009. Interview, clinical exam, Pressure Ulcer Scale Healing and photographic registry of the wounds were performed. Forty-two individuals participated who were, on average, 60 (± 12) years old, 73.8% males, with single wounds (66.7%) resulting from alterations in venous circulation (90.5%). Their wounds had lasted for up to one year (55.5%). There was an association between the PUSH score (p=0.019) and depth of wound (p=0.027) with currently performing occupational activity, as well as between history of tobacco use and gender (p=0.049). The sociodemographic characteristics that were observed were similar to the others, except for being male, which indicates the need for more studies in the population in Goiânia, Brazil.Objetivou-se analisar caracteristicas sociodemograficas e clinicas de pessoas com ulceras vasculogenicas e investigar a associacao entre estas variaveis. Realizou-se um estudo clinico observacional transversal, em salas de curativo de servicos municipais de saude, de fevereiro a agosto de 2009. Realizou-se entrevista, exame clinico, aplicacao da escala Pressure Ulcer Scale Healing (PUSH) e registro fotografico das lesoes. Participaram 42 individuos, com media de 60 anos (± 12), 73,8% do sexo masculino, com lesoes unicas (66,7%), decorrentes de alteracao da circulacao venosa (90,5%), duracao de ate um ano (55,5%). Houve associacao entre o escore da PUSH (p=0,019) e profundidade da lesao (p=0,027) com exercicio de atividade ocupacional na atualidade, assim como entre o historico de tabagismo e o genero (p=0,049). As caracteristicas sociodemograficas evidenciadas sao semelhantes as de outros, com excecao do genero masculino, o que indica a necessidade de maiores investigacoes na populacao goianiense.

Calf circumference: clinical validation for evaluation of muscle mass in the elderly

OBJECTIVE To validate calf circumference as a technology for assessing muscle mass in the elderly. METHOD Cross-sectional study with 132 elderly people from Goiânia, Goiás, Brazil. Decreased muscle mass was determined by the skeletal muscle mass index (IME) using Dual Energy X-Ray Absortometry (DEXA). The cutoff circumferences (CC) cutoff points to indicate muscle mass decrease were estimated by ROC curve, sensitivity, specificity and accuracy. RESULTS The most accurate cut-off points for detecting decreased muscle mass in the elderly were 34 cm for men (sensitivity: 71.5%, specificity: 77.4%) and 33 cm for women (sensitivity: 80.0%; specificity: 84.6%). CONCLUSION CC can be used as a measure for early identification of muscle mass decrease in routine evaluations of the elderly in primary care.

Impaired tissue integrity, related factors and defining characteristics in persons with vascular ulcers

This transversal study aimed to analyze factors related to, and defining characteristics of, Impaired Tissue Integrity in persons with vascular ulcers, and compare the mean of these factors between the groups presenting different degrees in the severity of tissue compromise. The study was undertaken in February-August 2009, in the municipal health network of Goiânia in the state of Goias, using interviews, clinical examinations and photographic records of the wounds. For analysis, simple and percentage frequencies and the Student t-test(p<0.05) were used. Five related factors were identified in more than half of the 42 participants: impaired circulation, knowledge deficit, chemical irritants, fluidexcessand nutritional factors. Three defining characteristics were identified in all of the participants. Caracterização de profissionais de enfermagem que atendem pessoas com úlceras vasculares na rede ambulatorial

Objetivou-se descrever caracteristicas demograficas e profissiograficas dos trabalhadores de enfermagem que atendem pessoas com ulceras vasculares, em salas de curativos da rede municipal de saude. Trata-se de pesquisa transversal, descritiva, de abordagem quantitativa realizada numa grande cidade da regiao Centro-Oeste do Brasil, de setembro de 2010 a fevereiro de 2011. Utilizou-se na coleta de dados um questionario para obtencao das caracteristicas de interesse. Realizou-se analise descritiva e teste qui quadrado (p<0,05). Participaram 218 profissionais, dos quais 58,3% nunca realizaram atividades de atualizacao em avaliacao e tratamento de feridas, 40,7% relataram pouca experiencia na area, 61% eram tecnicos e auxiliares de enfermagem. Aproximadamente 10% dos enfermeiros atuavam de modo fixo nas salas de curativos; os demais participavam do atendimento quando chamados. Os dados obtidos indicam fragilidades no processo de atendimento de enfermagem a pessoas com ulceras vasculares.
