We report the case of a 26-year-old man who presented to us with dysphagia secondary to blunt trauma to the neck. The patient was found to have a hyoid bone fracture with pharyngeal perforation and resultant neck abscess. The patient responded to prompt surgical and medical management. We believe this to be the first report of such a case.

Acute Suppurative Otitis Media: An Unusual Cause of Orbital Apex Syndrome

Ashwani Sethi; Deepika Sareen; Sumit Mrig; A. K. Agarwal; Pushpinder S. Khera

We report the case of a 40-year-old female who presented to us with earache and a rapidly progressive painful ophthalmoplegia involving the right eye with right temporal swelling. A clinical and radiological assessment revealed it to be temporal cellulitis with orbital apex syndrome secondary to acute otitis media. In spite of prompt initiation of treatment, the patient died within 30 hr of presentation. The case is discussed in detail with a brief review of medical literature.

Coexistent acute pyogenic and tubercular petrous apicitis : a diagnostic dilemma

Ashwani Sethi; Deepika Sethi; Sumit Mrig; J. C. Passey; N Srivastav

We report the case of a 16-year-old boy who presented to us with acute otitis media, facial weakness and retro-orbital pain. Computed tomography and magnetic resonance imaging (MRI) scans of the head and temporal bone revealed otitis media with petrous apicitis. The patient responded to broad-spectrum, parenteral antibiotics, with disappearance of facial weakness and reduction in pain. One month following the completion of treatment, the patient continued to have dull retro-orbital pain and developed ear discharge. A repeat MRI of the temporal bone revealed a persistent inflammatory lesion in the petrous apex, with a nodular, ring-enhancing lesion in the cerebellum, strongly suggestive of tuberculosis. The ear discharge stained positive for acid-fast bacilli and the patients serum enzyme-linked immunosorbent assay for tuberculosis was reactive. The patient responded well to anti-tubercular treatment and was disease free eight months following the completion of treatment.

Giant cell tumour (osteoclastoma) of the zygoma: An extremely unusual neoplasm

Ashwani Sethi; J. C Passey; Sumit Mrig; Deepika Sareen; Prashant Sharma

We report an extremely rare case of a giant cell tumour of the zygomatic bone in a 36-year-old female. The tumour was excised with healthy margins. The patient is asymptomatic without any evidence of recurrence 14 months postoperatively. To our knowledge, this is only the third reported case of this tumour occurring in the zygomatic bone in the medical literature.

Haemangioendothelioma of the Nose and Paranasal Sinuses

Deepika Sareen; Ashwani Sethi; Sumit Mrig; Sonu Nigam; Arun Kumar Agarwa

Abstract This report is of a 20-year-old man with haemangioendothelioma who presented with recurrent episodes of epistaxis. Nasal endoscopy revealed a reddish polypoidal mass arising from the middle meatus. Computed tomography scan revealed a homogenous mass involving the maxillary antrum and the nasal cavity without bony destruction. A medial maxillectomy with wide excision of the mass was performed and the histopathology was shown to be haemangioendothelioma. The patient remains free of disease 15 months after treatment.
