Co-infection Tuberculose/VIH et Maladie de kaposi sous traitement de substitution par la méthadone: à propos d’un cas

Late diagnosis of HIV infection can be fatal because it favors the appearance of opportunistic infections whose management requires the use of several molecules which can cause drug interactions. We report the case of a 45-year old female patient under heroin substitution treatment, using methadone and with HIV1 under antiretroviral treatment. This patient had nonspecific pulmonary appearance associated with dry nagging cough and progressive dyspnea evolving in a feverish context. Moreover, clinical examination showed left lower limb lymphedema with painless angiomatous nodules evolving over three years associated with plaques, angiomatous nodules occurred more recently at the level of the anterior face of the thorax. Sputum GeneXpert test allowed isolation of Mycobacterium tuberculosis. The diagnosis of pulmonary tuberculosis associated with Kaposis sarcoma and immunosuppression caused by HIV was retained.Le diagnostic tardif de l’infection à VIH peut être fatal car favorisant l’apparition d’infections opportunistes dont la prise en charge nécessite l’utilisation de plusieurs molécules pouvant provoquer des interactions médicamenteuses. Nous rapportons le cas d’une patiente de 45 ans, sous traitement de substitution à l’héroïne par la méthadone, VIH1 sous traitement antirétroviral. Cette patiente présentait un tableau pulmonaire non spécifique associant une toux quinteuse sèche avec une dyspnée d’apparition progressive évoluant dans un contexte fébrile. Par ailleurs l’examen notait un lymphœdème du membre inferieur gauche surmontée de nodules angiomateux indolores évoluant depuis trois ans associé à des plaques, des nodules angiomateux d’apparition plus récente au niveau de la face antérieure du thorax. Le GeneXpert sur les crachats avait permis d’isoler Mycobacterium tuberculosis. Le diagnostic retenu était celui d’une tuberculose pulmonaire associée à une maladie de kaposi cutané sur un terrain d’immunodépression au VIH.

Therapeutic itinerary of severe malaria in adults admitted to a teaching hospital in Dakar, Senegal.

INTRODUCTION Despite prevention efforts, malaria remains a public health problem. METHODOLOGY This was a prospective study conducted between October and December 2010 that aimed to describe the therapeutic route of adults presenting with severe malaria prior to being admitted to Fann Teaching Hospital in Dakar, Senegal. RESULTS A total of 90 patients were included. The majority of them had consulted a public or private health care facility (92%) prior to admission. First consultation occurred on average two days after the onset of the disease. Self-medication (67.4%) and traditional medicine (26.1%) were the main causes of delaying care. CONCLUSIONS Early care and adequate management are needed to reduce malaria mortality.

Évaluation de la tolérance de l’association zidovudine-lamivudine-névirapine chez les patients VIH-1 au Centre hospitalier de Fann à Dakar

We conducted a study to evaluate the tolerance of the zidovudine (AZT), lamivudine (3TC) and nevirapine (NVP) combination regimen in HIV-1 patients by a descriptive analytical retrospective study of all HIV-1 patients receiving AZT-3TC-NVP combination between 2008 and 2011. Seventy patients were included. Two thirds of the patients presented at least one side effect (44 cases). The digestive disorders (15 cases) and neuropsychiatric (14 cases) were the most frequent. Epigastralgia (20%), headaches (20%) and arthralgias (13%) were main side effects. A maculo-papular exanthema was noted in three cases. During the follow-up, five patients presented with anemia. No patient presented hepatic cytolysis due to NVP. All the patients followed for more than six months presented a side effect against 29.7% when the duration of treatment was equal to or less than 6 months (p=10(-5)). Most of the side effects due to the association AZT/3TC/NVP are minor. The evaluation of the clinical and biological tolerance must be maintained during all the follow-up.
