Differential impact of lower urinary tract symptoms on generic and disease-specific quality of life in men and women.

Introduction: We investigated how generic and disease-specific measures differ in estimating the quality of life (QoL) impact of lower urinary tract symptoms (LUTS). Materials and Methods: A total of 172 men and 67 women ≧40 years old who attended the public lectures completed the questionnaire comprising the International Prostate Symptom Score, benign prostatic hyperplasia impact index and SF-36 Health Survey. The multiple regression analysis was performed to evaluate the QoL impact of LUTS. Results: Both storage and voiding symptoms showed significant correlations with disease-specific QoL. Among men, voiding symptom correlated more closely with disease-specific QoL than storage symptom did, and this finding was reversed among women. Storage but not voiding symptoms showed a significant correlation with generic QoL. Among men, storage symptom correlated more closely with mental health than physical health, and the reversed finding was obtained among women. Generic QoL, especially physical health, was significantly influenced by some comorbid diseases, but disease-specific QoL was not. Conclusions: The QoL impact of LUTS was differentially estimated by disease-specific and generic measures with sex differences.

Congenital prepubic sinus accompanied by prevesical abscess

Congenital prepubic sinus is an extremely rare anomaly. The etiology is uncertain and the anatomical features often differ from each other. We report a 22-year-old woman with a congenital prepubic sinus accompanied by a prevesical abscess. She was admitted to our hospital with high-grade fever and low abdominal pain. Computed tomography revealed a prevesical abscess. After treatment of the prevesical abscess, we completely excised the congenital prepubic sinus. To our knowledge, this is the first reported case that accompanied by prevesical abscess on a congenital prepubic sinus. Moreover, this case represents the oldest reported age of a patient with a congenital prepubic sinus.

whether they did or did not report retrograde ejaculation (REj) as an adverse event (AE) during the study. RESULTS: (Table) Of the silodosin (SIL)-treated patients, 335 had no REj (-REj), while 131 had REj (+REj). Baseline IPSS and Qmax were comparable between groups. At 12 weeks, improvements in IPSS (LOCF) and Qmax (LOCF) were significantly greater for SIL(-REj) vs placebo and for SIL(+REj) vs placebo. Improvements in symptoms and peak flow rate were greater in the SIL(+REj) vs the SIL(-REj) group, with a trend toward significance for Qmax. CONCLUSIONS: The 28.1% experiencing retrograde ejaculation with silodosin treatment experienced greater improvement in symptoms and clinically meaningful greater improvement in peak flow rate compared with those not having this effect. This observation suggests that silodosin relaxes the smooth musculature of the lower urinary and genital tracts sufficiently to induce retrograde ejaculation, and that such superior relaxation results in greater objective efficacy in terms of unobstructing patients with LUTS secondary to BPH.
