Plant species used in dental diseases: Ethnopharmacology aspects and antimicrobial activity evaluation

ETHNOPHARMACOLOGICAL RELEVANCE Ethnopharmacological surveys show that several plant species are used empirically by the population, in oral diseases. However, it is necessary to check the properties of these plant species. AIM OF THE STUDY To evaluate in vitro antimicrobial activity against Streptococcus mutans from plant species selected in a previous ethnopharmacology study. MATERIALS AND METHODS An ethnopharmacological survey was conducted with users of a dental clinic school services, located in Sao Luis, Maranhão, Brazil, aiming to identify plant species used in oral diseases treatment. From the ethnopharmacological survey, species were selected for in vitro antimicrobial activity evaluation against Streptococcus mutans, by agar diffusion method and determination of Minimum Inhibitory Concentration (MIC). RESULTS Two hundred and seventy one people participated in the research: 55.7% reported the use of plants for medicinal purposes, 29.5% of which have knowledge and/or use plants for some type of oral disease. Thirty four species belonging to 24 (twenty four) botanical families were reported, being Aloe vera L., Anacardium occidentale L., Schinus terebinthifolius Raddi, Chenopodium ambrosioides L. and Punica granatum L. the most cited. The most commonly reported indications were healing after tooth extraction, followed by toothache, inflammation and bleeding gums., The determination of Minimum Inhibitory Concentration (MIC) demonstrated that Punica granatum L., Psidium guajava L. and Schinus terebinthifolius Raddi showed similar activity to 0.12% chlorhexidine, used as positive control. CONCLUSION That result is important to follow up the study of these species in the search for new anticariogenic agents originated by plants.

Association between chronic apical periodontitis and low-birth-weight preterm births.

INTRODUCTION The objective of this study was to investigate the association between chronic apical periodontitis (CAP) and low-birth-weight preterm births (LBWPB). METHODS Sixty-three women in postpartum period were included in this case-control study. The case group consisted of mothers of LBWPB infants (n = 33), and the control group was represented by mothers of newborns at term (n = 30). The CAP diagnosis was performed by using periapical radiographs through the periapical index in postpartum period. The χ(2) test, Fisher exact test, and linear and logistic regression were used for statistical analysis. RESULTS It was observed that CAP was present in 54.5% of mothers in the case group and 20.0% in the control group (P = .004); postpartum women with CAP had about 3.5 times greater odds of LBWPB newborns than women without CAP (adjusted odds ratio, 3.52; 95% confidence interval, 1.01-12.32). Postpartum women who reported 6 or more prenatal consultations reduced odds of LBWPB newborns in 80% (adjusted odds ratio, 0.20; 95% CI, 0.06-0.69). It can be estimated that the increase of 1 unit of periapical index had a significant association with the reduction of 1½ gestational weeks in the crude analysis (β = -1.5, P = .010) and the reduction of 211 g in birth weight after the adjusted analysis (β = -211, P = .058). CONCLUSIONS Prematurity and low birth weight were associated with radiographically detected CAP. Women with CAP in postpartum period had greater odds of LBWPB.

Association between periodontal changes and osteoporosis in postmenopausal women

Abstract Objective To investigate the possible association between periodontal changes and osteoporosis in postmenopausal women through a longitudinal study. Methods This longitudinal study included 33 patients. The participants were divided into three groups according to the bone mineral density assessed in the lumbar region: normal bone (G1, n = 15), osteopenia (G2, n = 12) and osteoporosis (G3, n = 6). Periodontal evaluation included clinical attachment level, probing depth, gingival bleeding index and visible plaque index, evaluated by two examiners blinded to systemic bone condition. The statistical process included the t-test for paired samples, with a significance level of 5% to check for changes in periodontal parameters considered at initial and final systemic bone density. Results The results showed that, after follow-up, there was a significant increase in gingival bleeding index in the group of women who had normal initial bone condition and progressed to osteopenia (after 3 years, 59.89%, p = 0.010) and osteoporosis (after 3 years, 74.37%, p = 0.035). In addition, the group diagnosed with osteopenia at baseline who progressed to osteoporosis after 3 years also showed a significant increase in gingival bleeding index (p < 0.001). Conclusions The findings suggest that periodontal changes can be associated with osteoporosis in postmenopausal women.

Analog microwave photonic link based on a high power directly modulated laser, a high power photodiode and passive impedance matching

We report on the design and characterization of a high-power and high dynamic range directly modulated analog fiber optic link in the S (2-4 GHz) band. The link is amplifier free, and uses passive impedance matching networks associated to high power laser diode and high power photodiode. Thanks to the high components efficiencies, together with the benefit of reactive impedance matchings, we achieved a high gain (≥ 0 dB) over 500 MHz bandwidths. Moreover, the use of high power and high linearity components allowed us to achieve simultaneously an output -1dB compression point higher than 20 dBm, and an output third order intercept point (OIP3) of more than 31 dBm. The measured spurious free dynamic range is as high as 114 dB.Hz2/3.

Salivary levels of calcium, phosphorus, potassium, albumin and correlation with serum biomarkers in hemodialysis patients

OBJECTIVES Evidences suggest that hemodialysis patients have reduced salivary flow and changes in the composition of salivary secretion. These changes may reflect local and systemic disorders. The objectives of this study were to compare the salivary levels of calcium (Ca), phosphorus (P), potassium (K) and albumin in hemodialysis patients and healthy subjects, and to investigate a possible correlation between their serum and salivary levels. DESIGN A case-control study was conducted with 60 hemodialysis patients (HD group) and 37 systemically healthy individuals (control group). Stimulated saliva samples were collected for biochemical analysis (Ca, P, K and albumin). Serum data were collected in the HD group. Statistical analysis included t-test, Pearson correlation and simple linear regression. RESULTS The HD group exhibited higher salivary levels of Ca, P, and albumin (p<0.05). There was a significant positive correlation between serum PTH and salivary phosphorus (r=0.342, p=0.009), and between serum PTH and salivary potassium (r=0.306, p=0.020). An increase of 100 pg/dL in serum PTH was associated with an elevation of salivary P levels (0.34 mg/dL, p=0.009), and salivary K levels (0.20 mmol/dL, p=0.02), in the HD group. CONCLUSIONS The findings suggest that HD patients present increased levels of salivary components (Ca, P, and albumin), and changes commonly observed in HD patients, such as hyperparathyroidism, appear to have an influence on salivary composition.

The influence of hormone replacement therapy on the salivary flow of post-menopausal women.

Abstract The aim of this study is to investigate the influence of hormone therapy on salivary flow in menopausal women. It is a case–control study involving 86 post-menopausal women. The case group consisted of 47 women undergoing estroprogestative or estrogen hormone replacement therapy (HRT), and the control group consisted of 39 women who did not receive any HRT. All patients were submitted to a standard questionnaire, followed by total stimulated sialometry and determination of body mass index (BMI). The salivary flow was classified as follows: normal (1.0–3.0 mL/min), low (0.7–1.0 mL/min), and hyposalivation (<0.7 mL/min). The results were analyzed statistically by the chi-square test, logistic regression model, and linear regression (p < 0.05). The HRT group presented an association of protection, even after adjusting the analysis, for low salivary flow (Adjusted OR = 0.22; 95% CI = 0.05–0.88; p = 0.034), and hyposalivation (Adjusted OR = 0.30; 95% CI = 0.10–0.92; p = 0.036). The results suggest that estroprogestative therapy (β = + 0.53; p = 0.022) has greater influence on the increase of salivary flow than estrogen therapy (β = +0.35; p = 0.137). The study concludes stating salivary flow was influenced by HRT on the post-menopausal women studied.

Association between early childhood caries and maternal caries status: A cross-section study in São Luís, Maranhão, Brazil.

Objective: This study aims to evaluate the association between early childhood caries (ECC) and maternal caries status, and the maternal perception of ECC risk factors. Materials and Methods: A cross-sectional study was carried out with 77 mother-child pairs, the children ranging from 12 to 36 months of age and their mothers, who were seeking dental care at a health center in São Luís, Maranhão, Brazil. Data collection was conducted using a specific questionnaire for mothers. Oral clinical examination of the mother-child binomial to assess caries incidence, gingival bleeding (GB) and visible plaque was done. Home visits were performed in 10% of the sample in order to observe the environmental conditions, dietary habits and dental hygiene practices. Results: The findings showed that the caries prevalence in children was 22.5 times higher in the mother who had decayed tooth (prevalence ratio [PR] = 22.5, confidence interval [CI] 95% = 3.2-156.6, P < 0.001). GB also was observed in 14 mothers and children, the PR in pair was 12.2 (CI95% = 1.6-88.9, P < 0.001). The variables are related for the mother-child binomial in regression linear analysis. Conclusion: The maternal caries status was associated with ECC.

Relationship between periodontal status and degree of visual impairment in institutionalized individuals.

Objective: Periodontal disease is a set of inflammatory infections that affect the supporting structures of the dentition. Patients with visual impairment (VI) may have more difficulty in cleaning and maintaining oral health. The purpose of this study was to investigate the possible relationship between periodontal status and degree of VI in institutionalized individuals. Materials and Methods: Fifty-two visually impaired individuals were included in this cross-sectional study. The periodontal parameters assessed were clinical attachment level (CAL), probing depth (PD), and visible plaque index. The degree of VI was established as: Group 1 (mild or moderate VI), Group 2 (severe or profound VI), and Group 3 (completely blind); and the types of VI were considered as congenital and acquired. Fisher′s exact, Kruskal-Wallis, Mann-Whitney, and Spearman correlation coefficient test were used. The level of significance was set at 5%. Result: Only plaque index was higher on proximal surfaces of subjects with mild/moderate VI when compared to the other degrees of VI (P = 0.01). Furthermore, we observed higher values for interproximal CAL (P = 0.01), total PD (P = 0.04), and interproximal PD in subjects with acquired VI when compared to subjects with congenital VI (P = 0.01). Conclusions: These findings suggest that periodontal status may be more related to the type of disability than with the degree of VI. Acquired VI people presented a worse periodontal health than the group with congenital VI.

Oral, systemic and socioeconomic factors associated with preterm birth

BACKGROUND The rates of preterm births have been increasing worldwide. Complications related to preterm births are associated with increased costs of care, and have a direct impact on the health system of the countries. Therefore, it is important to address factors associated with preterm birth in order to provide prevention strategies. OBJECTIVE This case-control study investigated oral, systemic, and socioeconomic factors associated with preterm birth in postpartum women. Participants were 279 postpartum women that gave birth to a singleton live-born infant. Cases were women giving birth before 37 completed weeks of gestation (preterm birth). Controls were women giving birth at term (≥37weeks). Data were collected through questionnaires, medical records and intra-oral clinical examinations, which included dental caries registration according to World Health Organization criteria and oral biofilm evaluation through visible plaque index. RESULTS Ninety-one women had preterm birth (cases) and 188 women had birth at term (controls), ratio 1:2. Caries lesions were present in 62.3% of the cases and in 62.5% of the controls. The univariate analysis showed no association between dental caries and preterm birth (Odds Ratio=1.08, p=0.90). The multivariate analysis showed that maternal educational level (Odds Ratio=2.56, p=0.01) and arterial hypertension (Odds Ratio=2.32, p=0.01) were associated with prematurity. CONCLUSION This study demonstrated that dental caries is frequent in postpartum women, but it does not appear to be associated with preterm birth. Meanwhile, maternal education level and arterial hypertension were associated with prematurity in this population.
