Is the Sciatic Functional Index always reliable and reproducible

The Sciatic Functional Index (SFI) is a quite useful tool for the evaluation of functional recovery of the sciatic nerve of rats in a number of experimental injuries and treatments. Although it is an objective method, it depends on the examiners ability to adequately recognize and mark the previously established footprint key points, which is an entirely subjective step, thus potentially interfering with the calculations according to the mathematical formulae proposed by different authors. Thus, an interpersonal evaluation of the reproducibility of an SFI computer-aided method was carried out here to study data variability. A severe crush injury was produced on a 5 mm-long segment of the right sciatic nerve of 20 Wistar rats (a 5000 g load directly applied for 10 min) and the SFI was measured by four different examiners (an experienced one and three newcomers) preoperatively and at weekly intervals from the 1st to the 8th postoperative week. Three measurements were made for each print and the average was calculated and used for statistical analysis. The results showed that interpersonal correlation was high (0.82) in the 3rd, 4th, 5th, 7th and 8th weeks, with an unexpected but significant (p<0.01) drop in the 6th week. There was virtually no interpersonal correlation (correlation index close to 0) on the 1st and 2nd weeks, a period during which the variability between animals and examiners (p=0.24 and 0.32, respectively) was similar, certainly due to a poor definition of the footprints. The authors conclude that the SFI method studied here is only reliable from the 3rd week on after a severe lesion of the sciatic nerve of rats.

Efeitos da aplicação do laser de baixa potência na regeneração do nervo isquiático de ratos

Peripheral nerves are commonly subject to traumatic injuries, leading to functional loss. Low-power laser therapy has been used in order to minimize harmful effects of inflammation and to accelerate healing of injured tissues. The purpose of this study was to assess the effect of 830 nm-laser irradiation on rat sciatic nerves submitted to crush. Twenty male Wistar rats had their sciatic nerve crushed and were divided into 4 groups (n=5): Sham7 and Sham14, placebo-treated for 7 and 14 days; L7 and L14, laser-treated (at 4 J/cm²) for 7 and 14 days. Sham group animals were submitted to the same procedures, but with the laser turned off. Assessed parameters were inflammatory infiltrates, fibroblasts, myelin sheath destruction, and axonal degeneration. The statistical analysis showed significant differences in three parameters: L14 animals showed more fibroblasts (p=0.0001), lesser myelin sheath degeneration (p=0.007), and lesser inflammatory infiltrates (p=0.001). Low-power laser therapy hence contributed to reduce the inflammatory process due to rat sciatic nerve injury.

Uma nova pinça regulável para a produção de lesões por esmagamento do nervo ciático do rato

Objective: A new adjustable pinch has been developed for producing a crush injury, with a previously known load of 5 kg, on a 5 mm-long segment of the nerve. Methods: Stainless steel was the material selected for building the pinch due its durability and possibility of sterilization with anti-septic substances, which are often corrosive. The crushing load of the pinch is adjustable by increasing or decreasing the tension of the spring by means of a screw used for calibration, which is performed by a load cell. Result: This pinch has been used in a few experimental investigations and was shown to be as efficient as both the universal testing machine and the dead weight machine, previously used. Conclusion: The developed pinch has the advantages of being portable and user-friendly. In addition, the pinch is cheap and allows for the standardization of the applied load.

Remobilization through stretching improves gait recovery in the rat.

Remobilization of a previously immobilized rat right hindlimb in the ankle plantar-flexion-shortened position by free movement alone or associated with intermittent passive stretching was assessed by analysis of gait variables and dorsiflexion range of motion. The variables were related with the expression of extracellular matrix proteins and the addition of serial sarcomeres. Sixty-four female Wistar rats were divided into 8 groups: immobilized, free for 10 days, immobilized/stretched/free for 1, 3 or 10 days, immobilized/free for 1, 3 or 10 days. Gait variables, range of motion, serial sarcomeres number, localization and staining intensity of fibronectin, and expressions of types I and III collagen were analyzed. The hypokinesia changed the functional variables of gait, reduced the dorsiflexion range of motion (ROM), increased the number of fibers with intracellular fibronectin/total number of fibers (FIF/TNF), and decreased the expression of the type I collagen. After three days, morphological changes were exacerbated and the number of serial sarcomeres was increased in both groups, immobilized/stretched/free and immobilized/free. Functional impairment, ROM restriction and increased FIF/TNF were also observed. Despite the above described alterations, 10 days of stretching program increased the effectiveness of remobilization leading to recovery of the abnormalities observed in the muscle.

Esteira com velocidade controlada para captação da marcha

Histological and electrophysiological methods for evaluation of peripheral nerve regeneration do not faithfully reproduce the functional index of limbs, even in controlled experimental conditions. Some methods of functional evaluation have been proposed, but their correlation to histological and electrophysiological data is not completely established, requiring more investigations with improvement of collection, management and processing of obtained data. In this study we developed a treadmill with controlled speed for recording footprint of rats submitted to different kinds of sciatic, fibular and tibial nerve lesions. The footprints were obtained on a walking track by means of a webcam connected to the treadmill and a computer with capacity to record the gait that will be submitted to functional evaluation. The developed treadmill allows the collection and filming of gaits and it has been tested in current experimental studies in rats with sciatic nerve lesion submitted to different kinds of treatment. The treadmill also enables researchers to record gait with constant, controlled and pre-established speed, with the possibility of direct visualization through the treadmill wall. In this manner, some variables that could damage the results of the research are resolved.

A new treadmill-type motorized walking belt machine for video recording of the rat's gait and sciatic functional index measurement. A comparative study with other methods

The sciatic functional index (SFI) is a remarkable tool to assess dysfunction and functional recovery of the sciatic nerve of rats. Usually measured on hind foot imprints on paper, a new method is now being proposed, by direct analysis of video recorded foot sole images obtained with a treadmill-type walking belt machine functioning with gait speed control (G1). Results were compared with the SFI measured on imprints on paper (G2) and on video recorded foot sole images obtained with a static see-through runway (G3). The right sciatic nerve of 19 adult female Wistar rats was crushed by the application of a controlled load. Impressions/images obtained both preoperatively and at weekly intervals for eight consecutive postoperative weeks were digitized, stored and analyzed in a computer loaded with specific software, the SFI being automatically calculated after measuring the appropriate parameters. SFI differed significantly between G1 and G2 and G1 and G3 (p<0.05), but not between G2 and G3 (p>0.05) during the first and second postoperative weeks, nonsignificant differences (p>0.05) being observed for any comparison between groups during the third through eighth postoperative weeks. We conclude that the three methods yielded equivalent results from the third week onward, but both video recording methods (G2 and G3) permitted a more adequate early evaluation (first and second weeks), since the SFI parameters were more easily identifiable. Images obtained with the walking belt machine are more uniform and sharper, thus contributing to reduce the influence of biases observed with imprints on paper.

Análise biomecânica e histológica de músculos gastrocnêmios de ratas submetidas à lesão muscular e tratados com laserterapia de baixa intensidade

OBJETIVE: To evaluate, by means of biomechanical and histological analysis, the effect of low-level laser therapy on the gastrocnemius of rats subjected to muscle lesion by impact. METHODS: 45 female Wistar rats, weighing 250g, were divided into 3 groups (n=15/group): C (control), ML (animals submitted to muscle lesion) and ML-L (animals submitted to muscle lesion and laser therapy). The animals from groups ML and ML-L were subjected to an experimental muscle lesion by means of an impact of a 250g load, released from a height of 30 cm, directly on the gastrocnemius. The animals from group ML-L were submitted to a treatment with a 960 Nm laser, 2 J/cm2, for 3 days, twice a day. RESULTS: The mechanical tests were performed on the Universal Testing Machine. The average values of maximum force were: 35.70 (± 2.69) N in group C, 31.77 (± 2.59) N in group ML and 34.36 (± 3.63) N in group ML-L, with a statistically significant difference between groups C and ML (p 0.05). Histological analysis showed a presence of blood vessels in group ML-L and a hematoma in the healing stage. CONCLUSION: Treatment with low-level laser had a positive effect in the healing process of muscle injury.

Correlações entre as estruturas dos membros inferiores

A literatura sugere que mudancas no ângulo Q podem alterar o arco plantar longitudinal e que um mau alinhamento do retrope poderia tambem modificar a postura do antepe. Contudo, nao foram encontrados estudos que correlacionem quantitativamente, na postura ortostatica bipodal, todos esses segmentos. Assim, o proposito deste estudo foi avaliar quantitativamente e verificar eventuais correlacoes entre o ângulo Q, arco plantar longitudinal, alinhamento frontal do tendao do calcâneo e antepe, na postura ortostatica bipodal, por meio de fotogrametria. Foram avaliados 30 individuos jovens (60 membros inferiores) de ambos os sexos, com idade entre 18 e 24 anos. Foi feita captura fotografica digital no plano frontal anterior de joelho e antepe; no plano frontal posterior, do tendao do calcâneo; e da impressao plantar exposta no podoscopio. Os dados foram analisados estatisticamente e o nivel de signficância fixado em 5%. Foram obtidas as seguintes correlacoes entre: ângulo Q X arco plantar, r=0,29 (p=0,021); ângulo Q X antepe, r=0,23 (p=0,092); ângulo Q X tendao do calcâneo, r=0,06 (p=0,627); arco plantar X antepe, r=0,09 (p=0,464); arco plantar X tendao do calcâneo, r=-0,05 (p=0,680); e antepe X tendao do calcâneo, r=-0,02 (p=0,857). A unica correlacao significativa encontrada, embora fraca, foi entre o ângulo Q e o arco plantar longitudinal medial, o que sugere cautela ao se inferirem correlacoes entre estruturas dos membros inferiores.

Estimulação elétrica de alta voltagem em nervo ciático de ratos: estudo pelo IFC

OBJECTIVE: To analyze the efficiency of high voltage pulsed current (HVPC) with early application in three different sites, in the regeneration of the sciatic nerve in rats submitted to crush injury, the sciatic functional index (SFI) was used to assess the functional recovery. METHODS: After crushing of the nerve, 57 animals were submitted to cathodal HVPC at frequency of 50Hz and voltage of 100V, 20 minutes per day, 5 days per week. The rats were divided into five groups: control group; ganglion group; ganglion + muscle group; muscle group; and sham group. The SFI was determined weekly for seven weeks, from the preoperative period to the 6th postoperative week. RESULTS: Compared with the control group, the results showed a significantly better performance of group 2 for the first 3 weeks; group 3 showed significantly better performance in the third week; and group 4 showed a significantly negative performance during the 4th and 6th weeks. CONCLUSION: Early application of HVPC had a positive effect in the treatment of the spinal cord region and the sciatic nerve root ganglion with a dispersive electrode on the contralateral lumbar region or on the gastrocnemius. However, HVPC had a negative effect in the treatment with an active electrode on the gastrocnemius and a dispersive electrode on the contralateral thigh. O ultrassom terapêutico na medula espinhal acelera a regeneração do nervo ciático de ratos

OBJECTIVE: To study the effects of low intensity ultrasound irradiation applied on the spinal cord on the regeneration of the rats sciatic nerve after a controlled crush injury, evaluating the functional results of the sciatic functional index as measured on the video recorded images of the foot sole. METHODS: Eighteen rats were submitted to a controlled crush injury of the right sciatic nerve and divided into two groups according to the treatment: Group 1 (n=9), simulated irradiation; Group 2 (n=9), effective irradiation. Low-intensity ultrasound irradiation was started on the 7th postoperative day and applied daily for 6 weeks. Images of the animals´ foot sole were video recorded on a see-through treadmill type walking belt machine at weekly intervals until the 6th week of irradiation and the corresponding sciatic functional index (SFI) was measured with specific software. RESULTS: The SFI during the first and last week of treatment was -59.12 and -12.55 in Group 1, -53.31 and -1.32 in Group 2, indicating a 79% and 97% improvement, respectively, but differences between groups were only significant (p<0.05) during the third week. CONCLUSION: The authors conclude that low intensity therapeutic ultrasound enhances nerve regeneration, with significance during the 3rd week of treatment. Level of Evidence: Level II, prospective comparative study.
