Research priorities in modeling the transmission risks of H7N9 bird flu

Viroj Wiwanitkit; Benyun Shi; Shang Xia; Guo-Jing Yang; Xiao-Nong Zhou; Jiming Liu

The epidemic of H7N9 bird flu in eastern China in early 2013 has caused much attention from researchers as well as public health workers. The issue on modeling the transmission risks is very interesting topic. In this article, this issue is debated and discussed in order to promote further researches on prediction and prevention of avian influenza viruses supported by better interdisciplinary datasets from the surveillance and response system.

Zika virus infection, do medical personnel know about the situation?

Somsri Wiwanitkit; Viroj Wiwanitkit

Dear Sir, The emerging infection is usually problematic in public health. Zika virus infection is the present problem worldwide. This arboviral infection can cause acute hemorrhagic fever and induce congenital malformation.[1‐3] To combat with this disease, the primary medical personnel have to understand the situation. Here, the authors tried simple survey asking a sample of fifty medical personnel whether they know that there are already cases of Zika virus infection in their country or not. Despite, there are already reports on cases, all personnel replies that they were not sure. Some also answered that there was no case in their city. This can imply that the medical personnel has no up‐to‐date knowledge on the situation of the present crisis. This observation is similar to the previous survey during emerging of Ebola.[4] How to promote and give the knowledge to primary medical personnel during the new emerging disease should be the focused public health policies.

Zika Virus Meningoencephalitis

Beuy Joob; Viroj Wiwanitkit

coronary tone is much more complicated than just looking at the worse narrowing, or percent stenosis, in an epicardial coronary vessel. Thus, it is almost naive to define such a luminal narrowing as exemplifying the nature of coronary blood flow and coronary artery flow reserve. I believe that we need to deal with the physiology of ischemia (whether it is fractional flow reserve determined using computed tomography or traditional fractional flow reserve determined during coronary angiography or just true angina experienced by a patient) and not the anatomy perceived by “stenosis” during visual inspection of an angiogram. However, the main focus of my editorial was that atherosclerotic plaque disease is a disorder of the arterial wall, and it escapes detection by coronary angiography (or stress testing or perfusion testing) until it has advanced to be a disorder of the lumen. As I had stated, coronary atherosclerotic disease is a continuum and not a threshold. Again, I thank Dr Rosenthal for his correspondence.

Wilms' tumour and chemotherapeutic access

Sora Yasri; Viroj Wiwanitkit

208 Sir, We enjoy reading the recent report on ‘Wilms’ tumour and chemotherapeutic access’.[1] Anyanwa et al. reported that ‘an institutionalised approach can help to improve access to anti-cancer drugs, reduce the rate of loss to follow-up and thus improve outcome’.[1] In fact, the problem of anticancer drug access can be seen in many developing countries. In a previous report by Uba and Chirdan, ‘poor availability of cytotoxic drugs’ is an important factor contributing to the poor outcome of disease in Nigeria.[2] It is suggested that pre-operative combined chemotherapy is required for the better operative curative outcome of paediatric cases of Wilms’ tumours.[3] To manage the high cost of chemotherapy, some new studies show that metronomic chemotherapy can be the hope for the management of the problem.[4]

Zika virus infection and psychosis

Beuy Joob; Viroj Wiwanitkit

We read the publication on ‘‘Zika virus infection followed by a first episode of psychosis’’ with great interest. Corrêa-Oliveira et al. noted that ‘‘to the best of our knowledge, this is the first report in which psychiatric symptoms were the only complication of acute ZIKV infection’’ and concluded that ‘‘Neuroimmune mechanisms leading to psychosis during acute CNS stress is an open and prolific field for research.’’ In fact, a wide clinical spectrum of Zika virus infection has been reported, and psychological problems are possible. Neuropsychiatric problems as the only clinical presentation in acute Zika virus disease is not a new finding. At least, psychotic features in acute Zika virus infection have already been observed in cases in which meningoencephalitis was later proved. In the Zika virus infection with meningoencephalitis, spatial delusions with visual and kinesthetic hallucinations are observable. Nanotechnology for health: A new useful technology in medicine

Beuy Joob; Viroj Wiwanitkit

Nanotechnology is accepted as a very useful new technology at present. The impact of new nanotechnology on health is very interesting. Indeed, several usefulness of nanotechnology on animal and human health can be expected. On the other hand, the negative effect of nanotechnology can also be seen. The applied nanotechnologies in public health and medicine already exist. In this short article, the summarization and discussion on nanotechnology and health are presented. It can be seen that the nanotechnology can be applicable in several fields of medicine and can help practitioner in diagnostic, therapeutic, and preventive activities.

Behavior of red blood cells indices in iron deficiency anemia and β-thalassemia trait

Beuy Joob; Viroj Wiwanitkit

Dear Editor, I read the article of “Behavior of red blood cells indices in iron deficiency anemia and β-thalassemia trait.”[1] Jassim noted that “red blood cell count, hemoglobin, hematocrit, mean corpuscular hemoglobin, and mean corpuscular hemoglobin concentration were significantly lower in iron deficiency anemia (IDA) participants than in β-thalassemia trait (TT) participants, whereas red blood cell distribution width was significantly higher in IDA participants than in β-TT participants. There was no significant difference regarding mean corpuscular volume between IDA participants and β-TT participants.”[1] We would like to share ideas and experience with this observation. In our area, Southeast Asia, the prevalence of both IDA and β-TT is extremely high. The point that should not be forgotten is the complex case with concurrence of both IDA and β-TT. In those cases, the clinical hematology parameters will be difficult to interpret. In addition, if there is also other additional abnormalities such as anemia due to chronic disease or other nutritional deficiency anemia (such as folate deficiency), the interpretation of the hematological parameters will be more complex. In the report by Jassim, the interesting point is the lack of ruling out of those mentioned possible concurrent disorders.
