Effect of resistance exercise on free radical production.

The purposes of this investigation were to see whether free radical production changed with high intensity resistance exercise and, secondly, to see whether vitamin E supplementation would have any effect on free radical formation or variables associated with muscle membrane disruption. Twelve recreationally weight-trained males were divided into two groups. The supplement group (S) received 1200 IUs of vitamin E once a day (3 x 400 IU x d[-1]) for a period of 2 wk. The placebo group (P) received cellulose-based placebo pills once a day for the same period of time. Creatine kinase activity was significantly elevated between preexercise and immediately postexercise, 6 h postexercise, and 24 h postexercise for both groups. The placebo group also had a significant increase in creatine kinase activity at 48 h postexercise. There was a significant difference in creatine kinase activity between the groups at 24 h after exercise. Plasma malondialdehyde significantly increased from preexercise levels for the P group at 6 and 24 h postexercise. Plasma malondialdehyde concentrations significantly increased in the S group between preexercise and immediately postexercise levels. This study indicates that high intensity resistance exercise increases free radical production and that vitamin E supplementation may decrease muscle membrane disruption.

Influence of exercise training on physiological and performance changes with weight loss in men.

PURPOSE The purpose of this study was to examine the physiological effects of a weight-loss dietary regimen with or without exercise. METHODS Thirty-five overweight men were matched and randomly placed into either a control group (C; N = 6) or one of three dietary groups; a diet-only group (D; N = 8), a diet group that performed aerobic exercise three times per week (DE; N = 11); and a diet group that performed both aerobic and strength training three times per week (DES; N = 10). RESULTS After 12 wk, D, DE, and DES demonstrated a similar and significant (P < or = 0.05) reduction in body mass (-9.64, -8.99, and -9.90 kg, respectively) with fat mass comprising 69, 78, and 97% of the total loss in body mass, respectively. The diet-only group also demonstrated a significant reduction in fat-free mass. Maximum strength, as determined by 1-RM testing in the bench press and squat exercise was significantly increased for DES in both the bench press (+19.6%) and squat exercise (+32.6%). Absolute peak O2 consumption was significantly elevated in DE (+24.8%) and DES (+15.4%). There were no differences in performance during a 30-s Wingate test for the DE and DES, whereas D demonstrated a significant decline in peak and mean power output. Resting metabolic rate (RMR) (kcal x d(-1)) was not significantly different for any of the groups except for the DE group. There were no significant changes in basal concentrations of serum glucose, BUN, cortisol, testosterone, and high density lipoprotein (HDL) cholesterol for any of the groups. Serum total cholesterol and low density lipoprotein (LDL) cholesterol were significantly decreased for all dietary groups. Serum triglycerides were significantly reduced for D and DES at week 6 and remained lower at week 12 for D, while triglycerides returned to baseline values for DES. CONCLUSIONS These data indicate that a weight-loss dietary regimen in conjunction with aerobic and resistance exercise prevents the normal decline in fat-free mass and muscular power and augments body composition, maximal strength, and maximum oxygen consumption compared with weight-loss induced by diet alone.

Alteration of brain default network in subacute phase of injury in concussed individuals: Resting-state fMRI study

There are a number of symptoms, both neurological and behavioral, associated with a single episode of r mild traumatic brain injury (mTBI). Neuropsychological testing and conventional neuroimaging techniques are not sufficiently sensitive to detect these changes, which adds to the complexity and difficulty in relating symptoms from mTBI to their underlying structural or functional deficits. With the inability of traditional brain imaging techniques to properly assess the severity of brain damage induced by mTBI, there is hope that more advanced neuroimaging applications will be more sensitive, as well as specific, in accurately assessing mTBI. In this study, we used resting state functional magnetic resonance imaging to evaluate the default mode network (DMN) in the subacute phase of mTBI. Fourteen concussed student-athletes who were asymptomatic based upon clinical symptoms resolution and clearance for aerobic exercise by medical professionals were scanned using resting state functional magnetic resonance imaging. Nine additional asymptomatic yet not medically cleared athletes were recruited to investigate the effect of a single episode of mTBI versus multiple mTBIs on the resting state DMN. In concussed individuals the resting state DMN showed a reduced number of connections and strength of connections in the posterior cingulate and lateral parietal cortices. An increased number of connections and strength of connections was seen in the medial prefrontal cortex. Connections between the left dorso-lateral prefrontal cortex and left lateral parietal cortex showed a significant reduction in magnitude as the number of concussions increased. Regression analysis also indicated an overall loss of connectivity as the number of mTBI episodes increased. Our findings indicate that alterations in the brain resting state default mode network in the subacute phase of injury may be of use clinically in assessing the severity of mTBI and offering some insight into the pathophysiology of the disorder.

Differential rate of recovery in athletes after first and second concussion episodes.

OBJECTIVE Clinical observations suggest that a history of previous concussions may cause a slower recovery of neurological function after recurrent concussion episodes. However, direct examination of this notion has not been provided. This report investigates the differential rate of restoring the visual-kinesthetic integration in collegiate athletes experiencing single versus recurrent concussion episodes. METHODS One hundred sixty collegiate athletes were tested preseason using multimodal research methodology. Of these, 38 experienced mild traumatic brain injury (MTBI) and were tested on Days 10, 15, and 30 after injury. Nine of these MTBI patients experienced a second MTBI within 1 year after the first brain injury and were retested. The postconcussion symptoms checklist, neuropsychological evaluations, and postural responses to visual field motion were recorded using a virtual reality environment. RESULTS All patients were asymptomatic at Day 10 of testing and were cleared for sport participation based on clinical symptoms resolution. Balance deficits, as evident by incoherence with visual field motion postural responses, were present at least 30 days after injury (P < 0.001). Most importantly, the rate of balance symptoms restoration was significantly reduced after a recurrent, second concussion (P < 0.001) compared with those after the first concussion. CONCLUSION The findings of this study confirm our previous research indicating the presence of long-term residual visual-motor disintegration in concussed individuals with normal neuropsychological measures. Most importantly, athletes with a history of previous concussion demonstrate significantly slower rates of recovery of neurological functions after the second episode of MTBI.

Prevention of Arthrofibrosis After Anterior Cruciate Ligament Reconstruction Using the Central Third Patellar Tendon Autograft

A retrospective analysis was performed to explain the decreasing incidence of postoperative arthrofibrosis of the knee in 191 consecutive patients who had anterior cruciate ligament reconstruction using the central third patellar tendon from 1987 through 1991. Follow-up data were available on 188 patients (98%). Age, sex, time interval from injury, preoperative motion, and concomi tant meniscal repair or partial meniscectomy were evaluated for their significance as risk factors. Twenty- two of 188 patients (12%) developed arthrofibrosis; the incidence was lower when the acute anterior cruciate ligament reconstruction was delayed at least 3 weeks from the injury, and when preoperative extension was 10° or better. Age, sex, preoperative flexion, and need for concomitant meniscal surgery were not risk factors. The postoperative motion protocol evolved during the study period. Group 1 patients were braced in 45° of flexion for 1 week before passive extension was al lowed. In Group 2, motion was started after 48 hours. Group 3 patients were braced in full extension, with mo tion starting within 24 hours. With these changes, the incidence dropped from 23% to 3%. Decreases in the incidence of arthrofibrosis with modifications in opera tive technique and postoperative analgesia were not statistically significant.

Alteration of brain functional network at rest and in response to YMCA physical stress test in concussed athletes: RsFMRI study.

There is still controversy in the literature whether a single episode of mild traumatic brain injury (mTBI) results in short- and/or long-term functional and structural deficits in the concussed brain. With the inability of traditional brain imaging techniques to properly assess the severity of brain damage induced by a concussive blow, there is hope that more advanced applications such as resting state functional magnetic resonance imaging (rsFMRI) will be more specific in accurately diagnosing mTBI. In this rsFMRI study, we examined 17 subjects 10±2 days post-sports-related mTBI and 17 age-matched normal volunteers (NVs) to investigate the possibility that the integrity of the resting state brain network is disrupted following a single concussive blow. We hypothesized that advanced brain imaging techniques may reveal subtle alterations of functional brain connections in asymptomatic mTBI subjects. There are several findings of interest. All mTBI subjects were asymptomatic based upon clinical evaluation and neuropsychological (NP) assessments prior to the MRI session. The mTBI subjects revealed a disrupted functional network both at rest and in response to the YMCA physical stress test. Specifically, interhemispheric connectivity was significantly reduced in the primary visual cortex, hippocampal and dorsolateral prefrontal cortex networks (p<0.05). The YMCA physical stress induced nonspecific and similar changes in brain network connectivity patterns in both the mTBI and NV groups. These major findings are discussed in relation to underlying mechanisms, clinical assessment of mTBI, and current debate regarding functional brain connectivity in a clinical population. Overall, our major findings clearly indicate that functional brain alterations in the acute phase of injury are overlooked when conventional clinical and neuropsychological examinations are used.

Bilateral Deficits in Postural Control following Lateral Ankle Sprain

Background: Although postural control deficits have been identified after lateral ankle sprains, objective and subjective comparisons of data before and after injury are limited. The purpose of this project was to prospectively assess and compare the changes in postural control and self-reported functional status in athletes who suffer acute lateral ankle sprains. We evaluated postural control and self-reported functional status before injury and at 1, 7, 14, 21, and 28 days after acute lateral ankle sprain. Methods: Postural control in single-limb stance and self-reported functional status were evaluated in 460 collegiate athletes during preseason examinations. Twenty-eight athletes suffered a lateral ankle sprain during the competitive season and participated in testing at 1, 7, 14, 21, and 28 days after injury. Results: Significant deficits in postural control were noted in both the injured and the uninjured ankles at 1 day after injury compared to the baseline measurements taken during the preseason examinations. Significant differences (p < .05) also were noted between the uninjured and injured ankles at 1, 7, and 21 days after injury. Conclusions: After lateral ankle sprain, postural control deficits occur in the injured and uninjured ankles, suggesting a central impairment in neuromuscular control. Changes in self-reported functional status followed a trend of deficit and improvement that was similar to that shown by the postural control data taken after injury, but the two measures did not strongly correlate.

Influence of a compression garment on repetitive power output production before and after different types of muscle fatigue

Compression shorts have become a very popular item of sports apparel. Few data exist about whether they influence athletic performance. The purpose of this study was to determine whether compression shorts affected vertical jump performance after different fatigue tasks (i.e., endurance, strength, and power). In addition, experiments on the influence of a compression garment on joint position sense at the hip and muscle movement velocity upon landing impact was also studied. Healthy college age men and women participated in the various studies. Subjects were thoroughly familiarized with the jump tests and all other experimental techniques. Jumps were performed on an AMTI force plate which was interfaced to a computer with customized software used to determine jump power. Ten consecutive maximal counter movement jumps with arms held at waist level were performed. The compressive garment had no effect on the maximal power of the highest jump in either men or women. The Effect of a Pneumatic Leg Brace on Return to Play in Athletes with Tibial Stress Fractures

A total of 18 competitive and recreational athletes were enrolled in a randomized, prospective study looking at the effect of pneumatic leg braces on the time to return to full activity after a tibial stress fracture. All patients had positive bone scans and 15 had positive radio graphic findings by Week 12. There were two treat ment groups. The traditional treatment group was treated with rest and, after 3 pain-free days, a gradual return to activity. The pneumatic leg brace (Aircast) group had the brace applied to the affected leg and then followed the same return to activity guidelines. The guidelines consisted of a detailed functional pro gression that allowed pain-free return to play. The brace group was able to resume light activity in 7 days (median) and the traditional group began light activity in 21 days (median). The brace group returned to full, unrestricted activity in 21 ± 2 days, and the traditional group required 77 ± 7 days to resume full activity. The Aircast pneumatic brace is effective in allowing ath letes with tibial stress fractures to return to full, unre stricted, pain-free activity significantly sooner than tra ditional treatment.

Repair of Peripheral Meniscal Tears: Open Versus Arthroscopic Technique

Tears in the peripheral vascular zone of 71 menisci in 68 knees were repaired by us from 1978 to 1986. The meniscus repair was done by open arthrotomy in 26 cases and by arthroscopic techniques in 45 cases. We have assessed the relative efficacies of open and arthroscopic repair techniques. The results were compared in knees with and without anterior cruciate laxity. The indications for meniscal repair included unstable peripheral detachments and longitudinal tears of the outer third of the meniscus. Open repair was performed by a posteromedial arthrotomy incision. Arthroscopic repair was done using the double-lumen guide system with a limited posterior incision for retrieval of needles. We have found that the arthroscopic technique is easier to perform than the open repair because some tears are too far inside the rim to lend themselves to open suture. The average follow-up is 4 years, 2 months, with a range of 2-10 years. There have been no neurologic or vascular injuries from either technique. Twenty-five patients have had a repeat arthroscopy. The overall failure rate was 9.8%. The difference between the failure rate of 11% in the open-repair group and 8.8% in the arthroscopic repair group was not statistically significant. The failure rate in anterior cruciate-stable knees was 8% versus a 13% failure in cruciate-deficient knees. We conclude that both open and arthroscopic meniscus repair techniques are safe and effective with few complications in both stable and unstable knees. Anterior cruciate ligament stability is ideal, but it is not mandatory for a successful result.
