Contrary to previously expressed concern, endoscopic papillotomy (EPT) is feasible in patients whose calculous biliary tract disease occurs in the presence of a juxtapapillary diverticula. EPT was accomplished in 33 such cases. The series was marked by but a single complication; in this instance, a retroperitoneal perforation healed following antibiotic therapy.

The authors have found the Olympus GIF-P2 to be an effective instrument, not only for conventional peroral endoscopy but also for polypectomy and, in selected cases, for ERCP and choledochoscopy.

Parapapillar choledochoduodenal fistulas usually tend to occur on the longitudinal fold of the duodenal papilla or proximal to it. Two illustrative cases are described wherein the fistulas were corrected by endoscopic papillotomy. Residual biliary tract stones, that commonly occur with choledochoduodenal fistulas, can be removed by this technique. Endoscopic papillotomy is a useful alternative to surgical treatment of choledochoduodenal fistulas, especially in high risk patients.

The authors experienced 233 cases of erosive gastritis from April, 1964 to August, 1971. Among these cases, the courses of 79 cases could be observed endoseopically at the interval of 3 to 6 months. In this present communication, the results of the observation of 15 cases, the courses of which were further investigated by gastric biopsy, were reported. In 8 cases which showed erosive gastritis continuously, the hyperplasia of glandular epithelium and pyrolic gland was markedly observed, while 7 cases, in which erosive gastritis disappeared in the course of observation, showed remarkable inflammatory cell infiltration and hyperplasia of pyrolic gland was hardly observed. On several cases, enzymic histoehemical staining (fl-glucuronidase, LDH, G-6-PDH, NADH) were performed together with hematoxilin-eosin staining. On the sample of atypical cell which was considered to stem from erosive gastritis, changes of glucuronidase activity in gland were observed. Among the cases of erosive gastritis which were subjected to surgery, 2 cases of atypical cell and one case of cancer, which were confirmed by histological examination, were demonstrated and some consideration was made.
