The effect of small dose of topiroxostat on serum uric acid in patients receiving hemodialysis

Introduction: Topiroxostat, a recently developed xanthine oxidase inhibitor, is expected to have fewer adverse effects than allopurinol because it has different mechanism of action from alloprinol. However, its dosage, usage and safety have not been established in patients with impaired renal function or those undergoing dialysis at the development since no studies was conducted in these patients.

Successful pregnancy and delivery in two patients with chronic renal failure

今回, 当施設において慢性腎不全患者2例の妊娠, 分娩を経験したので報告する.症例1は29歳の 妊娠15週, Cr 4.4mg/dlにて当院産婦人科へ入院. 逆流性腎症による慢性腎不全と診断し, 妊娠中絶を勧めたが, 妊娠継続の希望が強いため, 入院にて経過観察を行なった. 妊娠21週にBUN 77.9mg/dl, Cr 5.2mg/dlと腎不全が進展したため, 血液透析を導入した. 妊娠23週には性器出血, 胎胞の膨隆を認めたため, 頸管縫縮術を行ない, β2stimulantを投与するとともに, 以後床上安静とした. BUN 50mg/dlを目標に週3-4回の透析を行ない, Ht25%以上を保つため, 計32単位の輸血を施行した, 妊娠32週6日経膣分娩にて1790gの男児を出産, Apgar scoreは8点であった. 母児ともに順調な経過をたどり, 母親は分娩後血液透析を離脱し, 保存的に経過観察を行なっている.症例2は30歳の27歳で慢性腎不全のため週3回の血液透析開始. 29歳で結婚し, 妊娠. 妊娠継続希望が強いため, 妊娠19週2日にて当院産婦人科へ紹介され入院した. 週4回の血液透析にてBUN 50mg/dl以下に, また計34単位の輸血にてHt25%を保持した. 妊娠33週2日, に1690gの男児を経膣分娩し, Apgar scoreは9点であった.腎不全患者の妊娠, 分娩に関しては, 未だ結論は得られていないが, urea generationを考慮した透析回数, 貧血および予測される合併症への対策, 胎児発育を加味した基準体重の適切な設定など十分な産科的及び内科的管理を行なえば, 分娩も可能と考える.
