Short chain fatty acid in intestinal obstruction

Short chain fatty acid (SCFA) and enteric bacteria in enteric fluid of 26 patients with non-strangulated intestinal obstruction were investigated. Sixteen to 905 mg/L of SCFA was detected in all the samples and acetic, propionic and lactic acid were prominent in the fraction. Concentration of SCFA reached a peak around the time of operation and then gradually decreased, in most cases. It also gradually decreased with clinical improvement in conservatively treated cases. Cultures of these enteric samples revealed 104–11/ml of SCFA producing bacteria. SCFA may be one factor related to inhibition of water and electrolytes absorption and lead to distention of the bowel.

A Case of Ulcerative Colitis Complicated with Rapidly Progressed Sigmoid Colonic Cancer.

30歳の男性.19歳で全大腸炎型の潰瘍性大腸炎と診断されたが症状は乏しかった.1990年8月に行った注腸検査では隆起性病変は認められなかったが, 1991年2月より便柱の細小化と裏急後重が生じ, 下血も認められ, 4月8日に潰瘍性大腸炎の再燃の疑いで緊急入院した.ステロイドの強力静注療法を行ったところ下血は改善したが, S状結腸に全周性の狭窄を認め, S状結腸癌の合併した潰瘍性大腸炎と診断して開腹術を5月11日に施行した.しかし高度の腹膜播種と周囲への直接浸潤により切除不能であった.化学療法と免疫療法を行ったが奏効せず, 術後47日目に呼吸不全で死亡した.発症後10年以上経過した全大腸炎型潰瘍性大腸炎患者に対して1年に1回のcancer surveillanceの施行が推奨されている.しかし本症例はこの方法で早期に癌が発見できたかは疑問であり, 予防的治療も含め症例に応じた対応が必要と思われた.

BILE ACID METABOLISM IN NON-STRANGULATED INTESTINAL OBSTRUCTION

非絞扼性腸閉塞症42例の腸液中の胆汁酸分析を行った.総胆汁酸濃度は減少し, 抱合胆汁酸G/T比は低下した.腸内細菌の増殖が認められ, 遊離胆汁酸の検出率は増加した, 胆汁酸分画ではdeoxychol酸が減少した.治療後にはG/T比は上昇し, 腸内細菌は減少した.しかし, 総胆汁酸濃度, 胆汁酸分画組成比, 遊離胆汁酸検出率は, 手術的治療後には改善がみられたが, 非手術的治療では改善が認められなかった.胆汁酸には腸管における水分, 電解質の吸収を阻害する作用のあることが報告されており, 腸閉塞時には腸管内の液体貯留の一因となっていると考えられた.
