An Assessment of the Quality of Life of Patients With Class III Deformities Treated With Orthognathic Surgery

PURPOSE To evaluate quality of life (QoL) from a broad perspective by a comparison of Turkish patients undergoing orthognathic surgery to correct Angle Class III skeletal deformity with a control group composed of participants without dentofacial deformity using general health, generic oral health, and condition-specific QoL approaches. MATERIALS AND METHODS Thirty patients who underwent orthognathic surgery to correct Class III relations (monomaxillary and bimaxillary groups) and 30 participants with Class I skeletal structure and good dentofacial harmony (control group) were evaluated. Condition-specific QoL through a 22-item Orthognathic Quality of Life Questionnaire (OQLQ), generic oral health-related QoL through a 14-item Short-Form Oral Health Impact Profile (OHIP-14), and generic health-related QoL through a 36-item Short-Form Health Survey (SF-36) were assessed. RESULTS When the groups were compared, OQLQ scores showed a significant difference in the oral function domain only in the bimaxillary group (P < .05), and OHIP-14 scores showed significant differences in half the OHIP-14 subscale scores in the monomaxillary and bimaxillary groups (P < .05). The SF-36 scores showed significant differences only for the vitality domain in the monomaxillary group and the vitality and mental health domains in the bimaxillary group (P < .05). CONCLUSIONS For condition-specific and health-related QoL, the QoL of orthognathic surgical patients appeared to be similar to that of participants without dentofacial deformities. Oral health-related QoL of orthognathic surgical patients seemed moderately similar to that of participants without dentofacial deformities. The combined use of these methods is helpful for evaluating QoL from a larger perspective.

Effects of a Single-Dose of Pre-Emptive Pregabalin on Postoperative Pain and Opioid Consumption After Double-Jaw Surgery: A Randomized Controlled Trial.

PURPOSE The effect of a single-dose of pre-emptive pregabalin is still unknown, although it is used as an adjuvant in controlling acute postoperative pain. The purpose of this study was to evaluate the effects of pre-emptive single-dose pregabalin on postoperative acute pain and 24-hour opioid consumption in patients who underwent double-jaw surgery. PATIENTS AND METHODS Forty patients (18 to 45 yr old; American Society of Anesthesiologists status I to II) for whom elective double-jaw surgery was planned under general anesthesia were included in this study, which had been planned as a prospective, randomized, and double-blinded study. Patients were randomly divided into 2 groups: the pregabalin group (n = 20) was given pregabalin 150 mg orally 1 hour before general anesthesia and the placebo group (n = 20) was given an oral placebo capsule. The groups were administered the routine general anesthesia protocol. Postoperative analgesia was performed intravenously in the 2 groups twice a day with dexketoprofen trometamol 50 mg and patient-controlled analgesia with fentanyl. Postoperative analgesia was evaluated using the visual analog scale (VAS). Fentanyl consumption, additional analgesia requirement, and side-effects were recorded during the first 24 hours after surgery. Descriptive and bivariate statistics were computed, and significance was set at a P value less than .05. RESULTS Compared with placebo, the VAS score was statistically lower in the pregabalin group during the early postoperative period (P < .05). The 24-hour opioid consumption was significantly higher in the placebo group compared with the pregabalin group (509.40 ± 261.56 vs. 260.10 ± 246.53 μq, respectively; P = .004). In addition, the analgesia requirement was statistically lower in the pregabalin group (P < .05). Nausea or vomiting was observed more often in the placebo group, whereas other side-effects were similar for the 2 groups. CONCLUSION A single 150-mg dose of pre-emptive pregabalin decreased postoperative opioid consumption in the first 24 hours after double-jaw surgery. Multimodal analgesia techniques that contain pre-emptive analgesia can be used successfully in preventing postoperative pain caused by orthognathic surgery.

An Analysis of Mandibular Symphyseal Graft Sufficiency for Alveolar Cleft Bone Grafting

Abstract The purpose of this study was to evaluate the sufficiency of the mandibular symphysis as a donor site for unilateral and bilateral alveolar grafting, measuring both the alveolar cleft volume and maximum bone graft volume that can be harvested from the mandibular symphysis using 3-dimensional computed tomography (CT) and software in children and adults. Computed tomography data obtained from 20 unilateral and bilateral cleft lip palates patients in the preoperative period were used in this study. The patients were divided into 2 groups: children (female, n = 5; male, n = 5) and adults (female, n = 5; male, n = 5). The required bone graft volume for grafting and the maximum bone graft volume that can be harvested from the mandibular symphysis were measured based on cone beam CT data and software. The average required bone graft volume (cleft volume) for unilateral alveolar grafting was 963.51 ± 172.31 mm3 in the children and 1001.21 ± 268.16 mm3 in the adults. The average required bone graft volume for bilateral alveolar grafting was 1457.82 ± 148.18 mm3 in the children and 2189.59 ± 600.97 mm3 in the adults. The average the mandibular symphysis bone graft volume was 819.29 ± 330.85 mm3 in the children and 2164.9 ± 1095.86 mm3 in the adults. The results demonstrated that the mandibular symphysis region provided an adequate bone volume for alveolar grafting in adults with unilateral alveolar clefts. However, it is difficult to standardize these results, due to cleft volume and graft volume that could be harvested from the mandibular symphysis are highly variable among individuals.

Odontogenic and nonodontogenic cysts: An analysis of 526 cases in Turkey

Purpose: The aim of this study was to determine the age, gender, and sites of different types of odontogenic cysts (OCs) and non-OCs (nOCs) (seen in eastern Anatolian population in Turkey. Materials and Methods: The following data were collected from the clinical records and histopathology reports of the Department of Oral and Maxillofacial Surgery and the Department of Oral Pathology, Ataturk University, Turkey, during a period of 10 years. They were analyzed descriptively in terms of incidence, age, sex, cyst type, and site. Results: A total of 526 cysts were diagnosed during a 10-year period. Of these, 330 (62.7%) were in males, and 196 (37.3%) were in females, the male:female ratio was 1.7:1. There were 509 (96.8%) OCs and 17 (3.2%) nOCs. There were 406 (77.2%) inflammatory OCs and 103 (19.6%) developmental OCs. Radicular cysts were the most frequent (66.4%), followed by dentigerous (19.2%) and residual (10.8%) cysts. Only nasopalatine duct cyst was found as nOC in this study (3.2%). Conclusion: The distribution of jaw cysts in the Turkish Eastern Anatolian population is relatively similar to that of other populations reported in previous studies in the literature, in which most OCs were of inflammatory origin. However, the relative rates of cysts were higher than those reported in other studies.

Maksilla Posterior Bölgede Vertikal Kemik Miktarının Yetersizliği Durumunda Uygulanan Kısa İmplantların Üzerindeki Ve Etrafındaki Kuvvet Dağılımının Sonlu Elemanlar Analizi ile Değerlendirilmesi

Amac: Bu calismanin amaci; 3 boyutlu sonlu elemanlar analiz yontemi ile posterior maksiller dissiz bolgeye uygulanan kisa dental implantlarin uzerinde ve cevresinde olusan kuvvet dagilimlarinin incelenmesidir. Yontem: Maksiller sinus pnomatizasyonu bulunan dissiz posterior alana sahip saglikli bir bireyin bilgisayarli tomografi goruntuleri temel alinarak bir model olusturuldu. Ardindan 6,5 mm uzunlugunda ve 4 mm capinda vida tipi silindirik implant modeli olusturuldu. Vertikal ve oblik kuvvetler simule edilerek uc boyutlu sonlu elemanlar stres analizi yontemi ile implant uzerinde ve cevre kemik uzerinde olusan stresler degerlendirildi. Bulgular: Vertikal ve oblik yukleme esnasinda implant uzerinde olusan streslerin degerlendirilimesi icin von Mises stresi sonuclari hem sayisal olarak hem de renklendirilmis goruntuler olarak kaydedilmistir. Kortikal kemik ve kanselloz kemik uzerinde belirlenen 4 bolge uzerinde olusan von Mises stres, cekme stresi ve baski stresi sonuclari hem sayisal olarak hem de renklendirilmis goruntuler olarak degerlendirilmistir. Buna gore; streslerin kanselloz kemige oranla kortikal kemikte yogun oldugu gorulmustur. Ayrica, en yuksek kuvvetler implant icerisinde olustugu ve dayanak-implant birlesimi yakininda, implantin kortikal kemik ile komsuluk yaptigi bolge sinirinda yogunlastigi gorulmustur. Sonuc: Calisilan tum modellerde elde edilen stres degerlerinin yikici sinirlara yaklasmamasi sebebiyle, maksiller posterior dissiz bolgede kisa implantlarin kullaniminin basarili olabilecegi ongorulmustur. Implantlara gelen oblik kuvvetlerin olusturdugu stresin daha yuksek degerlerde oldugu dusunulerek, ozellikle kisa implant yerlestirilecegi zaman, implantlarin mumkun oldugunca okluzal kuvvetlere paralel sekilde yerlestirilmesi biyomekanik acidan onemlidir.

Benign tumors and tumor-like lesions of the oral cavity and jaws: An analysis of 709 cases

Purpose: The purpose was to examine the prevalence, gender, age and site(s) of odontogenic and nonodontogenic benign tumors, and tumor-like lesions occurring in the oral cavity and jaws in a Turkish population, particularly, in the Eastern Turkey, and to compare findings of this study with other reports. Materials and Methods: The data were collected from the files of the Department of Oral and Maxillofacial Surgery and the Department of Oral Pathology, Ataturk University, Turkey, during a 10-year period from January 2005 to January 2015. They were analyzed descriptively regarding prevalence, age, sex, lesion type, and site. Results: A total of 709 benign tumor and tumor-like lesions of the oral cavity, and jaws were selected during a period of 10 years. One hundred and twenty-one of these lesions (17%) were odontogenic benign tumors while 588 (83%) were nonodontogenic benign tumor and tumor-like lesions. Conclusions: This study revealed that the distribution and characteristics of benign tumors and tumor-like lesions of oral cavity and jaws in the Turkish population, particularly including the Eastern region of Turkey have some differences as well as similarities with the findings of studies in different populations.

Comprehensive analysis of the volume of bone for grafting that can be harvested from iliac crest donor sites

Our aim was to calculate the volumes of cancellous, cortical, and corticocancellous bone that can be harvested as a graft from the anterior and posterior iliac crests using 3-dimensional computed tomography (CT) and software in a living adult population. We selected random CT scans of the pelvis from 31 men and 29 women from the Department of Radiology imaging database. CT data in DICOM file format were imported into Mimics software. The anterior iliac crest and posterior iliac crest bone graft-harvested boundaries were measured. The volume of the 3-dimensional cortical and cancellous bone grafts was measured using the Mimics software. There were significant differences in all comparisons between the anterior and posterior iliac crest, except for volumes of cortical bone. More cancellous and total corticocancellous bone can be harvested from the posterior than the anterior iliac crest, together with similar or smaller volumes of cortical bone. Sex, but not age, is an important factor in terms of the amount of bone that can be harvested, with a wide range of volumes individually from both iliac crests.

With the long term success of the dental implants which provide satisfactory patient comfort, in some situations autotransplantation may be the choice of treatment for missing and impacted tooth. Again impacted teeth, especially the maxillary canines if they can be treated with orthodontics but the patient does not want it because of economic insufficiency or lack of time etc. autotransplantation can be considered. Because it improves aesthetics; allows normal alveolar and facial growth; maintains arch integrity, phonetics, occlusion and the sense of proprioception which dental implants do not have, it has many advantages. Besides, it is economically viable and fastest way to replacing missing tooth. Autotransplantation has high success rate if the prognostic factors taken into consideration. In this literature review, autotransplantation with aspects of success rates, prognostic factors, complications and different treatment methods were presented
