Fatores associados à notificação de maus-tratos em crianças e adolescentes na atenção básica

This study aimed to analyze factors associated with the reporting of child and adolescent abuse in primary healthcare. It is a cross-sectional study conducted with 381 professionals (physicians, nurses and dental surgeons) from three healthcare regions of the state of Ceara, Brazil. It used a self-administered questionnaire asking for socio-demographic data, vocational education and training, instrumentation and knowledge on the issue, identification and reporting of abuse cases. Bivariate and multivariate analyses using logistic regression were performed. There was a frequency of 17% of abuse reporting by basic healthcare professionals. The multivariate analysis showed that participating in the training on the issue (OR = 2.18; CI95% = 1.05-4.53), being familiar with the reporting form (OR = 3.03; CI95% = 1.47-6.24) and knowing how to refer the case properly (OR = 2.55; CI95% = 1.01-6.43) increased the chance of professionals reporting child and adolescent abuse. The variables identified in the final model showed a direct association between the act of reporting and matters related to knowledge and training on the issue. Investments in the training of healthcare professionals are required in order to favor the qualification of the practice for this public health problem.

Realities and perspectives of adolescent mothers in their first pregnancy

OBJECTIVE to understand the trajectory of adolescents regarding the first pregnancy, contemplating realities and perspectives. METHOD qualitative study, based on the Schütz theoretical framework, with 30 adolescents assisted in an outpatient clinic for adolescents in Fortaleza, Ceará, Brazil. From the narrative interviews carried out in the first semester of 2013, in the return for the first consultation of the puerperium, the communicative processes were identified, the central ideas and units of meanings expressed in the themes were abstracted: I was dating and became pregnant; realities and perspectives. RESULTS adolescents justified pregnancy through sexual drive and insufficient prevention; they narrated the fear faced, difficulties in motherhood and continuity of studies. The realities coexisted with the prospects of family members and partners helping to educate the child to achieve a different future from what they themselves experienced. FINAL CONSIDERATIONS adolescents experienced motherhood with ambivalent conflicts because they were young mothers, but they wanted to raise and educate their children, even those with minimal living conditions.

Construcción del estereotipo del del “macho nordestino “ brasileño en las letras de forró

Objetiva-se compreender a construcao e as modificacoes da identidade de genero masculina na Regiao Nordeste, com base no estudo das letras de forro. A partir das analises das cancoes como um territorio semiotico, o corpus da pesquisa conglomera letras de representantes iconicos, de cada periodo do forro, incluindo desde o compositor brasileiro Luiz Gonzaga ate a contemporaneidade. Este corpus organiza-se nas categorias ‘Nordestinidade’, ‘Imagetica Masculina’, ‘Imagetica feminina’ e ‘Violencia de Genero’. Pode-se apreender que a identidade cultural do nordestino foi historicamente consolidada nos principios patriarcais. Os discursos do forro comprovaram a adaptacao desses preceitos as leis de mercado do mundo globalizado, levando, cada vez mais, a mulher a se perceber como mercadoria e a legitimar sua subordinacao. Neste contexto, ressalta-se a relevância da compreensao dos simbolos culturais e de seu papel na construcao das subjetividades masculinas e femininas no Nordeste brasileiro.

Entrelaçamento voz e emoção na percepção docente sob a ótica da fenomenologia de Merleau-Ponty

A voz e uma ferramenta de trabalho do professor, refletindo suas condicoes de saude e emocoes. Este estudo objetiva compreender a percepcao de professoras sobre o entrelacamento de voz e emocoes. Trata-se de um estudo qualitativo realizado a partir de grupos focais com 26 professoras do ensino fundamental. Utilizou-se a fenomenologia da percepcao de Merleau-Ponty para analise dos dados, conduzindo- se pela nocao de corpo e de entrelacamento homem e mundo. As categorias identificadas foram: “desdobramentos do trabalho na voz, no corpo e nas emocoes” e “emocoes no corpo expressos pela voz”. As discussoes mostram a nao separabilidade corpo e mente e a percepcao da voz como um modo de vivenciar o corpo e as emocoes. A nocao de corpo proprio, complexo e sistemico extrapola o entendimento de corpo orgânico-biologico e contribui com a ampliacao dos debates sobre a saude vocal do professor.

Near Miss materno como indicador de atenção à saúde: uma revisão integrativa

Objective: The objective was to make a synthetic and descriptive compilation of the results of scientific studies addressing maternal near miss. Methods: A search was conducted in the databases of the Electronic Scientific Library Online (SciELO), Latin American and Caribbean Literature (LILACS) and International Literature in Health and Biomedical Sciences (PubMed),in the period from 2011 to June 2016, with the descriptors “maternal near miss” and “severe maternal morbidity” and their representations in Portuguese and English. After the exclusion of duplicate texts, the reading of the abstracts and the analysis by validated instrument, this review comprised 28 publications. Results: The studies indicate clinical determinants directly related to the occurrence of maternal near miss, among which, these stand out: high-risk gestation; hospitalizations during pregnancy; clinical comorbidities, mainly severe hypertension, infections and hemorrhage; cesarean delivery; use of forceps and need for ICU admission. Conclusion: In addition to the clinical determinants, the studies point out that socioeconomic and healthcare factors are directly related to the occurrence of maternal near miss, which makes it a viable tool for evaluation of the quality of the healthcare service and the impact of public policies aimed at improving maternal health and reducing social inequities. Early detection and provision of emergency obstetric care are essential to reduce maternal near miss and mortality rates.

Assistance Network and Morbi-Mortality Pattern Of Hypertension in a Metropolitan area Of Northeast Brazil

The aim of this study is to know the assistance network and the morbimortality pattern of hypertension in a metropolitan area. A search was performed in the government platforms to determine the assistance network for patients with hypertension in the city of Fortaleza, Northeast Brazil. We have reviewed the database in the period from 2010 to 2014 to investigate the hospital admissions and deaths due to hypertension and its related diseases. 23,709 patients are registered with diagnosis of hypertension. Primary care is given in 92 health centers, and the specialized care is provided in 112 units. Tertiary attention is given in 4 reference hospitals. Hypertension was the main cause of death in 67 patients admitted in that period, and cardiovascular diseases were responsible for 7,955 deaths. The assistance network for hypertension is well structured in this city, but due to the increasing population there is need to expand this network
