Evidências qualitativas sobre o acompanhamento por doulas no trabalho de parto e no parto

The objective of this study was to conduct a metasynthesis of evidence of the work of doulas assisting women in labor and during childbirth. Articles between 2000 and 2009 were located in the Medline, PubMed, SciELO, and Lilacs databases using the key search words: doulas, gestation, labor, and alternative therapy. Seven articles were selected for the study and four categories were created: the support provided by doulas; the birth mothers experiences; professional relationship: and opinions and experiences of professionals. The doulas offered physical, emotional, spiritual and social support. Experiments showed that the professionals stimulated the mother/child relationship, oriented towards successful breastfeeding, and contributed to the prevention of post-partum depression. Controversy was observed among professionals regarding acceptance of the role of the doula as a member of the obstetrics team. The doulas care was considered innovative, calming, encouraging, and attended all the needs of the pregnant woman. The conclusion is that qualitative studies on the work of doulas are recent, incipient, but revealing as to the important possibility of humanizing labor and childbirth.

Uso de práticas integrativas e complementares por doulas em maternidades de Fortaleza (CE) e Campinas (SP)

Abstract This study aimed to analyze the Integrative and Complementary Practices (ICP) applied by doulas in the cities of Fortaleza and Campinas-SP. This is a qualitative study encompassing fifteen doulas: nine from Fortaleza (Brazil, CE) and six from Campinas (Brazil, SP). The data were collected in the second half of 2010 by conducting semi-structured and pre-organized interviews following the procedures of thematic content analysis. The interpretations of the results were based on the idea that institu-tionalization of knowledge and practices happen through the conformation of nuclei and fields. The nucleus demarcates the identity of an area of knowl-edge and the professionals’ practices, and the field demarcates the blurred limits among disciplines that can be submitted to conflicts. We observed that the support offered by doulas permeates a variety of practices framed in traditional medicine as well as in complementary and alternative medicine. ICP was associated with decreases in length of labor, superior pain management, ability of making deci-sion and empowering of women. It is understood that the range of activities offered by doulas and the use of ICP converge to the uniqueness, respect and autonomy of women. Furthermore, it proposes a new model of awareness and practices centered on the importance of the natural process of childbirth.

The ways in which blind mothers cope with taking care of their children under 12 years old

Resumen pt: Objetivou-se analisar o enfrentamento de maes cegas no cuidado dos filhos menores de 12 anos, com enfase na promocao da saude, prevencao de riscos e educ...This study was aimed at analyzing the ways in which blind mothers cope with taking care of their children under 12 years old and is focused on health promotion, risk prevention and education. This is a qualitative study held in Fortaleza, in the state of Ceara, at the homes of nine blind mothers. A semi-structured interview was performed, from which the core meanings emerged: suitability of the home environment in order to prevent domestic accidents; prevention and treatment of diseases and monitoring of education. Data analysis was based on the concepts of coping and symbolic interactionism. The fact that the mothers are aware of the suitability of the home environment was evident. The pursuit for health promotion was done through the demand for healthcare system. Concerning education, the mothers are aware of the importance of the study and of monitoring its development. Despite the limitations and difficulties, it was observed that they care for and educate their children with the intention of raising good natured and responsible people for the society.

Realities and perspectives of adolescent mothers in their first pregnancy

OBJECTIVE to understand the trajectory of adolescents regarding the first pregnancy, contemplating realities and perspectives. METHOD qualitative study, based on the Schütz theoretical framework, with 30 adolescents assisted in an outpatient clinic for adolescents in Fortaleza, Ceará, Brazil. From the narrative interviews carried out in the first semester of 2013, in the return for the first consultation of the puerperium, the communicative processes were identified, the central ideas and units of meanings expressed in the themes were abstracted: I was dating and became pregnant; realities and perspectives. RESULTS adolescents justified pregnancy through sexual drive and insufficient prevention; they narrated the fear faced, difficulties in motherhood and continuity of studies. The realities coexisted with the prospects of family members and partners helping to educate the child to achieve a different future from what they themselves experienced. FINAL CONSIDERATIONS adolescents experienced motherhood with ambivalent conflicts because they were young mothers, but they wanted to raise and educate their children, even those with minimal living conditions.

Meanings Assigned by Puerperal Women to Episiotomy

Objective: identify the knowledge and perception of puerperal mothers after completion of routine episiotomy. Method: this is a descriptive and qualitative study conducted with 13 puerperal mothers in a public hospital in Fortaleza/CE in the month of November 2011, using a previously elaborated interview script. Data were analyzed according to thematic analysis, giving rise to the following category: “Knowledge and perception of puerperal women regarding the episiotomy”. Result: most women were young, at reproductive phase, outside the formal labor market and ended up going through obstetric interventions without questioning their realization, and only half of the women in the study claimed to have received information about the procedure. The information provided by healthcare professionals on episiotomy conferred greater confidence to mothers. However, only few patients received knowledge in this way. Conclusion: knowledge about episiotomy is important for women, enabling them to express their freedom of opinion to comply or reject its realization.

Assessment of Nurses’ Knowledge about Food Reintroduction in Tracheostomized Patients

Objective: assess the knowledge of nurses working in a hospital regarding food reintroduction to tracheostomized patients. Method: this is a quantitative, descriptive and cross-sectional study which used an instrument that consists of 20 closed questions, divided into two parts, applied to a sample of 50 clinical nurses in April 2011. Results: the sample was composed of 90% of female subjects with mean age of 35.4 years, 37% of respondents reported having more than ten years of professional experience and 83% stated that were never approached about the subject and that were unaware of the medical conditions to which the patient should submit to resume feeding by mouth. Conclusion: it was found professional unpreparedness and lack of knowledge in the subject and that approaching the theme since graduation and continuing education measures to be implemented in institutions that meet tracheostomized patients could provide the reintroduction of oral diet safely.

Pregnant Women’s Perception before the Difficulties of Access to High-Risk Prenatal Care

Objective: To identify the pregnant women’s perception before the finding of high-risk pregnancies and the difficulties of access to highrisk prenatal care.Methodology: This is a descriptive and exploratory study with a qualitative approach, conducted with 15 pregnant women at high-risk prenatal care service of a public hospital in Fortaleza/Ceara.Results: Pregnant women reported as difficulties the negligence on availability and assistance of high-risk prenatal service, lack of information about their disease, and the absence of a concrete link to reference and counter reference.Conclusion: The need for a close look at all professionals working in health care services is emphasized, especially those working in primary care for the realization not only of early diagnosis but to charge those responsible for solving referred cases, fulfilling the principles of universality, comprehensiveness and equity.
