Obesidad como Factor de Riesgo para Trastornos Metabólicos en Adolescentes Mexicanos, 2005

OBJECTIVE Determining the prevalence and estimating the risk of obesity for dyslipidemia and hyperinsulinemia in adolescents. The existence of a linear association betweenanthropometric measures, lipids and insulin was also evaluated. MATERIAL AND METHODS A comparative study was carried out amongst obese (body mass index=BMI >95th percentile for age and gender; n=120) and non-obese adolescents (BMI <85th percentile for age and sex; n=120) aged 10-19. A structured questionnaire was used for collecting anthropometric and demographic data. Glucose, insulin and lipid profiles were obtained for each adolescent. RESULTS Prevalence of at least one dyslipidemia was 56,6 % among obese adolescents and 20,8 % amongst non-obese ones (p<.001). The former registered 50 % prevalence of hyperinsulinemia, the latter 4 % (p<.001). Obesity increased hyperinsulinemia risk having a 23 odds ratio (8.3-68.9 95 % CI) and for at least one dyslipidemia (OR=5,0; 2,7-9,2 95 % CI). Insulin level significantly correlated with BMI (r=0,57), triglycerides (r=0,57), VLDL (r=0,57), HDL (-0,37), waist-hip circumference index (r=0,29), cholesterol (r=0,22), and LDL (r=0,13). CONCLUSIONS Obesity can be considered to be a risk factor for developing metabolic disorders in adolescents. In fact, there was a linear relationship between anthropometric measurement, lipids and insulin. Prevention should focus on improving predisposing environments for obesity amongst families having children and teenagers. Emphasising life-styles and healthy behaviour is essential, as well as training and treatment options for complete care of individuals in this age-group.

Diferencias en la utilización de métodos de planificación familiar por mujeres adolescentes en Monterrey, México, según el modelo de educación utilizado

Fundamento: El objetivo fue comparar la utilizacion de metodos de planificacion familiar durante el puerperio inmediato y dos anos despues del parto en las mujeres adolescentes que habian seguido dos programas diferentes de intervencion durante su embarazo. Metodos: Se diseno un estudio cuasiexperimental. Se selecciono a 62 adolescentes embarazadas que se integraron en dos grupos de intervencion, el modelo PRECEDE y el de Creencias en Salud (MCS). Se aplicaron pruebas estadisticas no parametricas y se estimaron intervalos de confianza de 95%. Resultados: El promedio de conocimiento inicial en el grupo MCS fue de 69,12 puntos (IC95% 63,27-74,97) y al final 89,71 puntos (IC95% 86,24- 93,17), mientras que el inicial del grupo PRECEDE fue de 49,39 puntos (IC 95% 42,24- 56,54) y al final de 75,25 puntos (IC 95% 71,12- 79,38). En el puerperio inmediato el 93% (IC 95% 83,5-100) de las adolescentes del grupo PRECEDE aceptaron el uso de un metodo de planificacion familiar, similar al empleado por el grupo MCS, de 94,2 (IC 95% 86,3-100). En cuanto a la continuidad en el uso del metodo, es decir dos anos despues, el efecto de la estrategia PRECEDE fue superior al del MCS, 92% (IC 95% 82- 100) y 72% (IC 95% 56,9-87,1) respectivamente. Conclusiones: Se encontro diferencia entre modelos en lo concerniente a la utilizacion de metodos de planificacion familiar a los 2 anos. Se propone el programa PRECEDE como estrategia educativa para prevenir un segundo embarazo en mujeres adolescentes.
