ABANDONO DO TRATAMENTO DA TUBERCULOSE E INTEGRALIDADE DA ATENÇÃO NA ESTRATÉGIA SAÚDE DA FAMÍLIA

Qualitative study that aimed to analyze the reasons why the tuberculosis patients abandon treatment in the metropolitan region of Joao Pessoa-PB. Was used as a theoretical to comprehensive care. Data were collected from October to November 2008, by means of semi-structured interview. Participated nine users who abandoned the treatment of tuberculosis. In the identification of the units of meaning, the technique of analyzis of content was used. The results show inconsistencies related to the holistic care of the sick, when it comes to doctrinal guidelines of the Unified Health, the consideration of socio-cultural context of the user with tuberculosis and the weaknesses in continuity of care, and the inadequate monitoring by the network of health care. These aspects, complicate the construction of a network of care and support for tuberculosis patients and their families, contributing to the abandonment of tuberculosis treatment. Maus-tratos a idosos: revisão integrativa da literatura

The study aimed to identify, through an integrative literature review, scientific knowledge, produced between 2005 and 2009 about the mistreatment of the elderly. It was selected sixteen studies in the data banks LILACS, SciELO and BDENF. Of the sixteen studies, 56.25% comprised original articles. The journal that published the most was the Public Health Journal (25%), being the majority of the studies from the Southeast (56.25%). The survey revealed that the physical violence was prevalent among the elderly; the home was the environment where aggression occurs more frequently; and family members were the main aggressors. Among the elderly attacked, it was observed that women are the victims most affected. O cuidado à saúde da mulher com tuberculose na perspectiva do enfoque familiar

To analyze the relationship between the practice of family health team workers and women with tuberculosis, according to the family focus dimension. The participants were eight family health team workers from a city in the Joao Pessoa metropolitan area (Paraiba -PB). Data were collected through semi-structured interviews, and then subjected to thematic content analysis. The workers recognize the critical social condition that women with tuberculosis live in, as well as the prejudice they face. They suggest that these women require care based on the concept of comprehensiveness in health. They also highlight the importance of Directly Observed Therapy - Short Course, the existence of incentives, and the womens poor adherence to educational activities and groups. Few workers recognize the inclusion of relatives in the care of women with tuberculosis, which implies the need for discussion regarding this fact with the purpose of improving tuberculosis treatment effectiveness. Early treatment and cast services in the care of tuberculosis patient

ABSTRACT Objective : to evaluate factors related to noncompliance with treatment for tuberculosis, according to the size of the range of services. Method : this evaluative qualitative, which used the thematic oral history as a methodological reference. The study included nine users who had abandoned as a criterion for termination of the treatment of tuberculosis in two districts of the metropolitan area of Joao Pessoa, Paraiba after approved by the Ethics Committee of the Federal University of Paraiba with protocol number 067/08 . The empirical material was produced from July to September 2008 and analyzed by the technique of discourse analysis. Results : we observed as related to cessation of treatment: alcoholism, lack of information about the disease, the illusion of healing before the end of treatment and the large number of tablets taken. How weaknesses in the provision of services available, it was found that the User is difficult to access testing, inputs and social incentives (food stamps, food stamps). Conclusion : considering the size of the services available, it is believed to prevent the abandonment of TB treatment, is necessary to rescue the notion of subjectivity of care, and to strengthen the principle of integrated and intersectoral approach in guiding the actions and health services. Descriptors : tuberculosis; family health; patient dropouts . RESUMO Objetivo : avaliar os fatores relacionados ao abandono do tratamento da tuberculose , segundo a dimensao do elenco de servicos. Metodo : pesquisa avaliativa de natureza qualitativa, que utilizou a Historia Oral Tematica como referencial metodologico. Participaram do estudo nove usuarios que tiveram o abandono como criterio de encerramento do tratamento da tuberculose em dois municipios da regiao metropolitana de Joao Pessoa, Paraiba, apos aprovacao do Comite de Etica em Pesquisa do Centro de Ciencias da Saude da Universidade Federal da Paraiba (CEP/CCS) sob registro de protocolo numero 067/08. O material empirico foi produzido no periodo de julho a setembro de 2008 e analisado mediante a tecnica de analise do discurso. Resultados : foram verificados como fatores relacionados ao abandono do tratamento: o etilismo, a falta de informacoes sobre a doenca, a ilusao de cura antes do termino do tratamento e a grande quantidade de comprimidos tomados. Como fragilidades na oferta do elenco de servicos, constatou-se que ha dificuldade do usuario ter acesso a exames, insumos e incentivos sociais (cestas basicas, vale alimentacao). Conclusao : considerando a dimensao do elenco de servicos, acredita-se que para prevenir o abandono do tratamento da TB, seja necessario resgatar a nocao de subjetividade do cuidado, bem como fortalecer o principio da integralidade e a diretriz da intersetorialidade nas acoes e servicos de saude. RESUMEN Objetivo : evaluar los factores relacionados con la falta de cumplimiento con el tratamiento para la tuberculosis, de acuerdo con el tamano de la gama de servicios. Metodo : evaluacion cualitativa, que utilizo la historia oral tematica como referencia metodologica. El estudio incluyo a nueve usuarios que habian abandonado como criterio para la finalizacion del tratamiento de la tuberculosis en dos distritos de la zona metropolitana de Joao Pessoa, Paraiba d espues de la aprobacion por el Comite de Etica de la Universidad Federal de la Paraiba con numero de protocolo 067/08). El material empirico fue elaborado entre julio y septiembre de 2008 y analizados por la tecnica de analisis del discurso. Resultados : se observo en relacion con la interrupcion del tratamiento: el alcoholismo, la falta de informacion sobre la enfermedad, la ilusion de curacion antes del final del tratamiento y el gran numero de pastillas. ?Como deficiencias en la prestacion de los servicios disponibles, se constato que el usuario es dificil de las pruebas de acceso, los insumos y los incentivos sociales (estampillas de comida, estampillas de comida). Conclusion : considerando el tamano de los servicios disponibles, se cree que es prevenir el abandono de tratamiento de la tuberculosis, es necesario rescatar la nocion de subjetividad de la atencion, y fortalecer el principio de enfoque integrado e intersectorial en la orientacion de las acciones y los servicios de salud. Tuberculosis is an infectious contagious disease that remains at a high illness rate in several countries and the non-adhesion to the treatment has reduced the healing rate. Objective: The study aimed at identifying, through the discourse of people infected by tuberculosis, factors and meanings that benefit the adhesion to the specific treatment. Method: For the data collection an in-depth interview was applied. The analysis was based on the theoretical-analytical framework of the French school of Discourse Analysis. Results: The results revealed that the manner in which professionals welcome the patient, mainly the nurse, as well as the relatives’ support, are decisive factors to the adhesion of the treatment. Concerning the subject’s stand regarding the care, one observes that it is linked to the concept of integrality as attributed to the organization of health practices. Conclusion: In this aspect, the importance of the singularities of the people infected by tuberculosis stands out in the elaboration of the therapeutic projects.

El diagnóstico de la tuberculosis em las personas mayores: las barreras relacionadas con el acceso a los servicios de salud

Este estudo tem como objetivo analisar as barreiras ao diagnostico da tuberculose em idosos relacionadas aos servicos de saude no municipio de Joao Pessoa-PB. Trata-se de pesquisa qualitativa que envolveu sete pessoas idosas doentes de tuberculose. Entrevistas foram utilizadas para coletar informacoes. O material empirico foi organizado com o software Atlas.tiversao 6.0 e analisado conforme a tecnica de analise de discurso. As barreiras relacionadas ao acesso para confirmacao diagnostica foram: horario de funcionamento das unidades de saude da familia; transferencia de responsabilidades; visitas domiciliares sem controle de comunicantes; demora do servico de saude em suspeitar da doenca e repetidas idas do doente ao servico de saude para a obtencao do diagnostico. Apesar de terem sido identificadas barreiras comuns as encontradas por doentes de tuberculose de um modo geral, em virtude da vulnerabilidade dos idosos, sugerem-se que acoes de controle sejam empreendidas pelos servicos de saude de modo a evitar que a doenca torne-se um agravo comum para essa populacao.This study was performed with the objective to analyze the barriers to diagnosing tuberculosis in the aged and access to health services in the city of João Pessoa, Paraíba, Brazil. This qualitative study included the participation of seven aged women with tuberculosis. Interviews were used for data collection. The empirical material was organized using Atlas.ti 6.0, and analyzed according to the techniques of discourse analysis. The identified barriers related to the access to health services to confirm the diagnosis were: the operating hours of family health units; transferred responsibilities; home visits without controlling communicants; delay of the health service in suspecting the disease and the patients repeated visits to the health center before being informed about the diagnosis. Despite the identification of common barriers that tuberculosis patients of all ages must deal with, because of the vulnerability of the elderly, health services should implement control actions so as to prevent the disease becoming a common condition in this population.
