Effect of a video on developing skills in undergraduate nursing students for the management of totally implantable central venous access ports

BACKGROUND Handling Totally Implantable Access Ports (TIAP) is a nursing procedure that requires skill and knowledge to avoid adverse events. No studies addressing this procedure with undergraduate students were identified prior to this study. Communication technologies, such as videos, have been increasingly adopted in the teaching of nursing and have contributed to the acquisition of competencies for clinical performance. OBJECTIVE To evaluate the effect of a video on the puncture and heparinization of TIAP in the development of cognitive and technical competencies of undergraduate nursing students. METHOD Quasi-experimental study with a pretest-posttest design. RESULTS 24 individuals participated in the study. Anxiety scores were kept at levels 1 and 2 in the pretest and posttest. In relation to cognitive knowledge concerning the procedure, the proportion of correct answers in the pretest was 0.14 (SD=0.12) and 0.90 in the posttest (SD=0.05). After watching the video, the average score obtained by the participants in the mock session was 27.20. CONCLUSION The use of an educational video with a simulation of puncture and heparinization of TIAP proved to be a strategy that increased both cognitive and technical knowledge. This strategy is viable in the teaching-learning process and is useful as a support tool for professors and for the development of undergraduate nursing students.

Tendências da pesquisa envolvendo o uso do toque terapêutico como uma estratégia de enfermagem

Therapeutic touch (TT) is one of the oldest vibrational therapies still in use. Dolores Krieger and Dora Kunz, in the 1970s, pioneered the application of the method in nursing. Since then, research involving the use of therapeutic touch has been developed in order to demonstrate the effectiveness of the intervention to reduce the signs and symptoms related to various diseases. Many studies have evaluated the use of TT in isolation, however, a large part of these research studies compare this strategy with other complementary therapies, such as: music, guided imagery and relaxation. The results of these studies indicate improvement or reduction of signs and symptoms investigated in patients undergoing TT. However, methodological biases make it impossible to generalize the results, making it essential to conduct well-designed clinical trials capable of determining the effectiveness of the intervention, in addition to studying populations that have not yet been addressed, such as the elderly and newborns.O toque terapeutico (TT) e uma das mais antigas terapias vibracionais ainda em uso. Dolores Krieger e Dora Kunz, na decada de 1970, foram precursoras da aplicacao do metodo na enfermagem. Desde entao, as pesquisas envolvendo o uso do toque terapeutico tem sido desenvolvidas com o intuito de demonstrar a efetividade da intervencao na reducao de sinais e sintomas relacionados a diversas doencas. Muitos estudos avaliam o uso do TT de forma isolada, porem, grande parte das pesquisas compara esta estrategia com outras terapias complementares, tais como: musica, imagem guiada e relaxamento. Os resultados desses estudos apontam melhora ou reducao dos sinais e sintomas investigados nos pacientes submetidos ao TT. Entretanto, vieses metodologicos impossibilitam a generalizacao dos resultados, tornando indispensavel a realizacao de estudos clinicos bem delineados capazes de comprovar a efetividade da intervencao, alem de estudar populacoes que por ora nao sao abordadas, como os idosos e recem-nascidos.

Pharmacological interventions to treat phlebitis: systematic review.

This study presents a systematic review for evaluating effective pharmacological actions for the treatment of phlebitis stemming from infusion therapy. The studies reviewed were categorized according to the type of therapeutic approach proposed by the author and by the level of evidence presented. The review found that topical nitroglycerin and notoginseny were more effective in the reduction of the inflammatory process when compared with other proposed alternatives. Nevertheless, the development of research related to possible alternatives for the treatment of phlebitis is important.

Topical interventions to prevent acute radiation dermatitis in head and neck cancer patients: a systematic review

PurposeThe purpose of the study is to evaluate the effects of pharmacological and non-pharmacological topical controls in the prevention of radiation dermatitis.MethodsRelevant clinical trials were identified through electronic searching databases CINAHL, CENTRAL, LILACS, PubMed, Scopus, and Web of Science. Handsearching and gray literature searches were also performed to find additional references. Primary outcomes of interest were the development of radiation dermatitis and the time of occurrence of radiation dermatitis.ResultsThirteen randomized clinical trials were included in this review. The trials were published in Chinese, English, or French, from 1980 to 2015. Pharmacological interventions used in the trials were trolamine, aloe vera, allantoin, Lianbai liquid, sucralfate, Na-sucrose octasulfate, olive oil, hialuronic acid, and dexpanthenol. Non-pharmacological topical controls were usual care/institution routine, aqueous cream, mild soap, water thermal gel, placebo, and no intervention.ConclusionsThere was no strong evidence that indicates differences between topical pharmacological interventions or non-pharmacological topical controls in the prevention of acute radiation dermatitis among patients with head and neck cancer undergoing radiotherapy.

Gel of chamomile vs. urea cream to prevent acute radiation dermatitis in patients with head and neck cancer: a randomized controlled trial

AIM To compare a gel made with chamomile (Chamomilla recutita) with a cream of urea as an intervention to delay the time to occurrence of radiation dermatitis. BACKGROUND Radiation dermatitis is one of the most common adverse effects of radiotherapy in patients with head and neck cancer. It is characterized by erythema, itching, pain, skin breakage and burning sensation, and there is no consensus on how to prevent it. DESIGN The study is a randomized controlled clinical trial. METHODS We will recruit 48 individuals with head and neck cancer who will be starting their radiotherapy and randomize them to receive either gel of chamomile or cream of urea, as an intervention for prevention of radiation dermatitis. Social-demographic data will be collected at baseline, and clinical data will be collected before the initiation of radiotherapy. Participants will be followed weekly to assess development of radiation dermatitis. The protocol is funded by Conselho Nacional de Pesquisa e Desenvolvimento Científico (Brazil). The study was approved by a research ethics committee. DISCUSSION Given the clinical relevance of preventing radiation dermatitis and the lack of evidence supporting specific preventive interventions, it is important to study new products that might be efficacious to prevent this complication. This article presents the protocol of a randomized controlled trial comparing a gel made with chamomile (intervention) with a cream of urea (control) to prevent radiation dermatitis in patients with head and neck cancer undergoing radiotherapy.

Validation of an educative manual for patients with head and neck cancer submitted to radiation therapy.

Objetivo: validar el contenido y la apariencia de manual educativo dirigido a los pacientes con cancer de cabeza y cuello, sometidos a radioterapia. Metodo: investigacion metodologica de caracter descriptivo. Se utilizo la Teoria de la Psicometria para el proceso de validacion, el cual fue realizado por 15 peritos en el area tematica del manual educativo y por dos profesionales de letras y publicidad. Fue considerado un indice de concordancia que fuese, por lo menos, de 80%, para garantizar la validacion del material. Resultados: los items abordados en el instrumento de evaluacion del manual educativo fueron divididos en tres bloques: objetivos, estructura y presentacion, y relevancia. Apenas un item, relacionado al nivel sociocultural del publico-objetivo, obtuvo un indice de concordancia <80%, habiendo sido reformulado con base en las sugestiones de los participantes. Todos los otros items fueron considerados adecuados y/o totalmente adecuados en los tres bloques propuestos: objetivos (92,38%), estructura y presentacion (89,74%), y relevancia (94,44%). Conclusion: el manual educativo propuesto fue considerado valido en lo que se refiere al contenido y a la apariencia. Se sugiere que puede contribuir para la comprension del proceso terapeutico, al cual el paciente con cancer de cabeza y cuello es sometido al realizar la radioterapia, ademas de auxiliar la practica clinica por medio de la consulta de enfermeria.

Conhecimento de pacientes acerca da quimioterapia antineoplasica

Objetivo : avaliar o entendimento e a percepcao dos pacientes com câncer em relacao a quimioterapia antineoplasica, no que concerne aos efeitos colaterais e as medidas de prevencao/controle destes. Metodo : estudo transversal, com abordagem qualitativa, realizado em um ambulatorio de quimioterapia de um hospital universitario de Brasilia, Distrito Federal. Resultados : foram avaliados 50 pacientes, no periodo de janeiro de 2012 a fevereiro de 2013, que seriam submetidos a primeira infusao de quimioterapia antineoplasica. Evidenciou-se deficit de conhecimento dos entrevistados acerca da quimioterapia antineoplasica bem como do manejo de seus efeitos colaterais. Entretanto, nausea/vomito, alopecia, mucosite, diarreia, perda do apetite e fadiga foram relatados como efeitos frequentes. A maioria dos entrevistados conseguiu identificar apenas a atuacao da equipe medica e o uso de terapias farmacologicas para controle dos sinais e sintomas relacionados ao tratamento. Conclusao : a partir dos resultados apresentados, verificou-se importante deficit de conhecimento acerca da quimioterapia antineoplasica e prevencao/controle de seus efeitos colaterais. Tal fato motivou a elaboracao de manuais de orientacoes direcionados aos pacientes submetidos a quimioterapia antineoplasica sobre os cuidados necessarios ao controle de sinais e sintomas decorrentes deste tratamento.
