A respiração oral em crianças e suas repercussões no estado nutricional

PURPOSE: identify nutritional state, masticatory function, respiratory mode, sense of smell and sense of taste among mouth breathing children. METHODS: nasal aeration, mastication, nutritional state of 77 mouth breathing children and 154 nasal breathers children between six and ten years were evaluated. RESULTS: mouth breathers exhibited alterations in the masticatory pattern when compared to the nasal-breathers, but did not have statistical significance in nutritional state. CONCLUSION: the oral-breathing children had more alteration in sleep, reduction in the ingestion of carbohydrate and increase in ingestion of lipids, however, no consistent data were found pointing out the association between mouth breathing and nutritional condition.

Adaptações na mastigação, deglutição e fonoarticulação em idosos de instituição de longa permanência

PURPOSE: to identify extant adaptations into stomatognathic functions of chew, deglutition and speech in aged people at a long permanence Institution. METHODS: 34 aged people, of both genders, being 15 feminine and 19 masculine, with ages ranging from 65 to 88 years, took part in the study. Interviews were carried through containing questions for the aged people on general health and feeding and speech therapy evaluations were performed in order to characterize functions of chew, deglutition and speech. RESULTS: as for chew, it was observed that the majority of aged people showed an adapted chew, with exaggerated participation of perioral muscles, heaves of jaw and predominant chew standard was bilateral simultaneous. As for deglutition, it was observed that this function as well as chew were an adapted form into the majority of volunteers majority, we also observed the occurrence of exaggerated participation of perioral muscles and noise during deglutition. In speech evaluation we noted that in more than 50% of the aged people, there was great air escape occurring during speaks, as well as a rise of reduced larynx and the presence of substitution, omission, distortion and imprecision. CONCLUSION: it was observed that aged people get adaptations during chew process in order to keep their possible diet next to when they were young. It was also noted that the studied aged people carry through some compensations during deglutition. The found speech characteristics do not limit severely aged people communication currently. It was still possible to observe that attendance to aged people needed to be carried through by an interdisciplinary team and that speech therapist can and must be part of this team assisting in preventing and/or rehabilitating issues related to stomatognathic functions.

Eletromiografia de superfície da musculatura supra-hióidea durante a deglutição de idosos sem doenças neurológicas e idosos com Parkinson

PURPOSE: to investigate differences between electromyographic samples of right and left suprahyoid muscles during deglutition of thickened liquids and liquid consistencies by elderly people with Parkinson disease and elderly people without any neurological disease. METHODS: 30 subjects, from both genders, took part in this study. Fifteen were elderly without neurological disease whereas the others showed Parkinson disease. All of them were submitted to a clinical evaluation protocol and surface electromyography. RESULTS: significant amplitude difference was found between subjects (f=14.4 and p=0.000) and between sides (t=4,4 and p=0.0037). As for muscular contraction time, the differences were just significant between the consistencies (f=8.2 and p=0.005). CONCLUSION: the electromyographic amplitude was shorter for subjects with Parkinson disease and this amplitude was higher in both groups at the right side. The duration of the muscular contraction did not display significant difference between the subjects with Parkinson disease and elderly without neurological disease. This duration was shorter during the deglutition of liquid consistencies.

Antropometria e mastigação em crianças asmáticas

OBJETIVOS: caracterizar os padroes da antropometria facial em criancas asmaticas; identificar a presenca de assimetrias faciais em criancas asmaticas e nao asmaticas; e relacionar o lado de predominio mastigatorio com a presenca de assimetria facial em criancas asmaticas e nao asmaticas. METODOS: participaram da pesquisa 60 criancas com idade entre 6 e 10 anos. Destas, 30 possuiam diagnostico em prontuario de asma moderada ou grave e 30 criancas nao apresentavam asma, fazendo parte do grupo controle. Foram realizadas avaliacoes antropometrica facial e da mastigacao dessas criancas. RESULTADOS: em relacao as mensuracoes antropometricas faciais nas criancas asmaticas e nao-asmaticas nao foram reveladas diferencas significativas entre os grupos. No que diz respeito a presenca de assimetrias faciais, observou-se que estas ocorreram no grupo controle, assim como, no grupo asmatico. O padrao mastigatorio predominante em ambos os grupos foi o bilateral simultâneo e quando relacionados a assimetria facial e o predominio mastigatorio, nao foram encontradas associacoes significantes. CONCLUSAO: nao foram encontradas diferencas significativas entre o grupo controle e o grupo asmatico em relacao as mensuracoes antropometricas. A assimetria facial foi observada nos dois grupos avaliados. Em ambos o padrao mastigatorio bilateral simultâneo foi predominante, porem quando realizada a relacao entre assimetria facial e o lado de predominio mastigatorio, nao se observou relacoes significativas.

Gustatory and olfactory dysfunction in laryngectomized patients

UNLABELLED After total laryngectomy surgery, nasal airflow is moved permanently to the tracheostomy opening, compromising the contact of odorant molecules with the nasal cavity, which may reflect changes in the olfactory and gustatory perception in these individuals. OBJECTIVE To evaluate the functions of smell and taste in total laryngectomized patients. STUDY DESIGN a study of series. METHOD The sample included a group of 25 patients submitted to total laryngectomy and another group of 25 patients who did not underwent the procedure. The taste function was evaluated by gustatory strips of filter paper. To assess the olfactory function we employed the Brief Smell Identification Test. RESULTS Among the laryngectomized patients there was more hypogeusia (80%, p < 0.05), as well as hyposmia (88%, p < 0.001), alone and concomitant (72%, p < 0.001). Concerning flavor discrimination, the bitter taste did not differ between the groups - which was different from the other flavors. In the olfactory aspect, laryngectomized patients performed worse in detecting warning and food-related odors. We found that a history of smoking and alcohol consumption were significantly more frequent among laryngectomized patients. CONCLUSION We found a decrease of gustatory and olfactory functions in total laryngectomized patients in this study.

Surface electromyography: proposal of a protocol for cervical muscles

Purpose: to present a proposal of a surface electromyography evaluation method for cervical muscles specific and detailed protocol, with a standardized collection method of electrical signal in these muscles. Method: the researchers took as reference the existing publications about this subject which evidenced a need for standardization, clarity, better reproducibility and greater specificity for the surface electromyography evaluation of the upper trapezium and sternocleidomastoid muscles fibers. The proposal preparation process for the current protocol included the cleaning of the target area, placing the electrodes, required tasks in order to collect and register the electrical signal and interpretation of the electromyography signal parameters. This evaluation method was carried out in 24 healthy volunteers of both genders, with an average age of 26 years. We used the electromyography Miotool 400 with 4 channels. Result: an evaluation surface electromyography method for upper trapezium and sternocleidomastoid muscles fibers was developed and tested in order to determine the best form of electrical signal data collection for these muscles. Conclusion: we submitted a protocol proposal to evaluate the cervical muscles by a surface electromyography, allowing the healthy professionals and researchers to get more information about this electrical potential evaluation method for the sternocleidomastoid and the upper trapezium muscles fibers. This knowledge will be an adjuvant in a more specific therapy.

Quantitative instruments used to assess children's sense of smell: a review article

PURPOSE To systematically gather from the literature available the quantitative instruments used to assess the sense of smell in studies carried out with children. RESEARCH STRATEGY The present study included a survey in the Pubmed and Bireme platforms and in the databases of MedLine, Lilacs, regional SciELO and Web of Science, followed by selection and critical analysis of the articles found and chosen. SELECTION CRITERIA We selected original articles related to the topic in question, conducted only with children in Portuguese, English, and Spanish. We excluded studies addressing other phases of human development, exclusively or concurrently with the pediatric population; studies on animals; literature review articles; dissertations; book chapters; case study articles; and editorials. DATA ANALYSIS A book report protocol was created for this study, including the following information: author, department, year, location, population/sample, age, purpose of the study, methods, and main results. RESULTS We found 8,451 articles by typing keywords and identifiers. Out of this total, 5,928 were excluded by the title, 2,366 by the abstract, and 123 after we read the full text. Thus, 34 articles were selected, of which 28 were repeated in the databases, totalizing 6 articles analyzed in this review. CONCLUSION We observed a lack of standardization of the quantitative instruments used to assess childrens sense of smell, with great variability in the methodology of the tests, which reduces the effectiveness and reliability of the results.

Respiratory muscle strength in asthmatic children

Summary Introduction: Changes in the respiratory system of asthmatics are also due to the mechanical disadvantage caused by the increased airway resistance. Objective: The study aims to evaluate the respiratory muscle strength and nutritional status of asthmatic children. Method: This is a prospective descriptive and transversal study with 50 children aged 7 to 12 years, who were placed into 2 groups, asthmatic and non-asthmatic. Respiratory muscle strength was evaluated on the basis of maximal inspiratory pressure (MIP) and maximal expiratory pressure (MEP). The nutritional status was evaluated by measuring the anthropometric data, including height, weight, and body mass index (BMI). The findings were subjected to analysis of variance, chi-square, and Students t test, and p-values < 0.05 was considered statistically significant. Results: In our comparisons, we observed statistically significantly lower values for age, weight, and height in asthmatic patients: 8.52 ± 1.49 years, 30.62 ± 7.66 kg, and 129.85 ± 10.24 cm, respectively, vs. non-asthmatic children(9.79 ± 1.51 years, 39.92 ± 16.57 kg, and 139.04 ± 11.62 cm, respectively). There was no significant increase in MIP and MEP between the groups: MIP was -84.96 ± 27.52 cmH2O for the asthmatic group and -88.56 ± 26.50 cmH2O for the non-asthmatic group, and MEP was 64.48 ± 19.23 cmH2O for asthmatic children and +66.72 ± 16.56 cmH2O for non-asthmatics. Conclusion: There was no statistically significant difference between groups, but we observed that MIP and MEP were slightly higher in the non-asthmatic group than in the asthmatic group.

The Use of Electrognathography in Jaw Movement Research: A Literature Review

Abstract The measurement of mandibular movements is a complex task that can be simplified through the use of electrognathography (EGN), an examination that can be applied as an auxiliary method for the diagnosis or follow-up of the evolution of specific therapeutics applied to the stomatognathic system. The aim of the present study was to review the literature pertaining to the use of EGN as a method for monitoring jaw movements in the research and treatments related to the stomathognathic system. A literature review was made by searching for articles in PubMED, MEDLINE (1997-2010), MEDLINE (1966-1996), LILACS, Ovid, BBO and SciELO databases. In this systematic review, 373 articles were found, of these, 23 articles were selected. The study concluded that EGN can be used with advantages in varied clinical situations, although it is a technique still not widely applied.

Repercussões da respiração oral no estado nutricional: por que acontece?

INTRODUCAO: Algumas criancas que respiram pela boca e apresentam apneias obstrutivas noturnas podem apresentar retardo do crescimento pondero-estatural. OBJETIVO: O objetivo deste artigo e analisar as alteracoes miofuncionais orofaciais presentes no individuo respirador oral e as repercussoes sobre o estado nutricional. Enfoca a importância da equipe interdisciplinar no acompanhamento das alteracoes globais presentes na respiracao oral. METODO: O metodo utilizado foi uma revisao da literatura, a partir de artigos publicados em revistas cientificas indexadas, livros e trabalhos de pos-graduacao. A maioria dos artigos foi identificada a partir das bases de dados, LILACS, MEDLINE e SCIELO. RESULTADOS: Percebe-se relacao da respiracao oral com a modificacao no processo geral de alimentacao, associada as dificuldades no olfato, paladar e disturbios miofuncionais orofaciais, repercutindo assim no estado nutricional. Comentarios Finais: A diversidade de causas envolvidas na respiracao oral requer uma equipe interdisciplinar treinada para identificar estas alteracoes, possibilitando a implementacao de medidas preventivas, que evitem alteracoes na saude geral, no desenvolvimento normal da face e no estado nutricional em importantes fases do crescimento desses individuos.
