Aptidão física relacionada à saúde de escolares: programa fitnessgram

The study identified the proportion of children and adolescents of the school population from Montes Claros, Minas Gerais, Brazil, who meet the health-related criteria determined by physical fitness components. The sample was composed of 2,849 subjects (1,457 girls and 1,392 boys), with age range between 6 and 18 years. The physical fitness components were assessed by a test battery of five items: back-saver sit-and-reach; curl-up; trunk-lift; push-up and endurance run (PACER), following cut off-points for gender and age suggested by the Fitnessgram. It was possible to observe with the obtained results that the proportion of schoolchildren that reached the minimum motor requirements established was not higher than 8%, and the global amount able to meet the health-related criteria was lower from 10 years of age in both genders. The data also show that great part of the studied youngsters demonstrate physical fitness components that could compromise a better health status, indicating the urgent need of implementing intervention programs addressed to the improvement of the practice of physical exercise in the school population.

Physical Activity, Sedentary Behavior, Cardiorespiratory Fitness and Metabolic Syndrome in Adolescents: Systematic Review and Meta-Analysis of Observational Evidence

Background Metabolic syndrome (MetS) has been diagnosed in adolescents and among the associated factors are low levels of physical activity, sedentary behavior over long periods and low cardiorespiratory fitness. However, specifically in adolescents, studies present conflicting results. The aim of the present study was to conduct a systematic review and meta-analysis of observational studies, in order to map the association between physical activity, sedentary behavior, cardiorespiratory fitness and MetS in adolescents. Methods A search was performed in the databases PubMed, SPORTDiscus, LILACS and the Cochrane Library. For the meta-analysis, the odds ratio (OR) was calculated together with the respective confidence intervals (95% CI), in which the measures of effect were analyzed by dichotomous data (exposure variables) with MetS used as events. Results Eighteen studies were included in the meta-analysis. Primary analysis demonstrated that low levels of physical activity (OR = 1.35 [1.03 to 1.79]; p = 0.03) and low cardiorespiratory fitness (OR = 4.05 [2.09 to 7.87]; p < 0.01) were significantly associated with the development of MetS, while for sedentary behavior, represented by screen time > 2 hours/day, a significant association was not identified (OR = 1.20 [0.91 to 1.59]; p = 0.20). Subgroup analyses demonstrated that the association between low physical activity and MetS was dependent on the use of the accelerometry technique (OR = 2.93 [1.56 to 5.47]; p < 0.01). Screen time > 2 hours/day was significantly associated with MetS only on weekends (OR = 2.05 [1.13 to 3.73]; p = 0.02). With respect to cardiorespiratory fitness, a significant association with MetS was found independent of the maximal oxygen uptake (VO2max) measurement method. Conclusions Low levels of physical activity, low indices of cardiorespiratory fitness and sedentary behavior, represented by screen time > 2 hours/day on weekends, were significantly associated with the development of MetS in adolescence.

MEDIDA DA ATIVIDADE FÍSICA EM JOVENS BRASILEIROS: REPRODUTIBILIDADE E VALIDADE DO PAQ-C E DO PAQ-A

Introducao: O Physical Activity Questionnaire for Children (PAQ-C) e o Physical Activity Questionnaire for Adolescents (PAQ-A) foram originalmente propostos para oferecer medidas autorrelatadas da pratica de atividade fisica de jovens. Objetivos: Traduzir para o idioma portugues, realizar adaptacao transcultural e analisar reprodutibilidade e validade concorrente do PAQ-C e do PAQ-A em amostra de jovens brasileiros. Metodos: Versoes originais foram traduzidas de acordo com recomendacoes internacionais. Os questionarios traduzidos foram administrados em amostra de 528 jovens (285 mocas e 243 rapazes) com idades entre oito e 18 anos. Reprodutibilidade foi obtida por intermedio de replicas de aplicacao dos questionarios, com intervalo de uma semana, envolvendo coeficiente de correlacao intraclasse (CCI) e tecnica de Bland-Altman. A consistencia interna foi analisada mediante coeficiente alfa de Cronbach. Para analise da validade concorrente foram consideradas informacoes apresentadas pelo acelerometro RT3 durante sete dias como criterio de referencia. Atividade fisica total (AFtotal) e atividade fisica de intensidades moderada a vigorosa (AFMV) derivadas do acelerometro foram comparadas mediante coeficiente de correlacao de Spearman com pontuacoes apresentadas pelos questionarios. Resultados: Apos discretas modificacoes apontadas no processo de traducao, um comite de juizes considerou que as versoes traduzidas do PAQ-C e do PAQ-A apresentaram equivalencias semântica, idiomatica, cultural e conceitual. Reprodutibilidade teste-reteste mostrou CCI entre 0,68 e 0,88. A tecnica de Bland-Altman evidenciou elevada capacidade de concordância entre replicas de aplicacao dos questionarios. A consistencia interna mostrou α = 0,71 e α = 0,76, respectivamente. A pontuacao dos questionarios demostrou correlacoes moderadas com AFtotal (rho = 0,40; rho = 0,50) e AFMV (rho = 0,48; rho = 0,54) estimadas pelo acelerometro. Conclusao: A traducao, adaptacao transcultural, reprodutibilidade e validade concorrente do PAQ-C e do PAQ-A foram satisfatorias, o que viabiliza sua aplicacao em futuros estudos no Brasil.

Participation Motivation Questionnaire: tradução e validação para uso em atletas-jovens brasileiros

Os objetivos do estudo foram traduzir para o idioma portugues, realizar a adaptacao transcultural e identificar as propriedades psicometricas para atletas-jovens brasileiros do Participation Motivation Questionnaire (PMQ). A versao original foi traduzida de acordo com recomendacoes internacionais. Um comite de juizes foi formado para analisar as versoes traduzidas do questionario. O comite utilizou como criterio de analise as equivalencias semântica, idiomatica, cultural e conceitual. A versao final do questionario traduzido foi administrada em uma amostra de 1517 atletas-jovens (714 mocas e 803 rapazes) com idades entre 12 e 18 anos. Para identificar as propriedades psicometricas foi realizada analise fatorial confirmatoria com rotacao varimax e, na sequencia, para analise da consistencia interna de cada fator associado a motivacao para a pratica de esporte, foi empregado coeficiente alfa de Cronbach. Apos discretas modificacoes apontadas no processo de traducao, o comite de juizes considerou que a versao para o idioma portugues do PMQ apresentou equivalencias semântica, idiomatica, cultural e conceitual. A analise fatorial confirmou a estrutura de oito fatores originalmente proposta, explicando 67% da variância total com satisfatorios valores de consistencia interna. O alfa de Cronbach apresentou coeficientes entre 0,543 e 0,827. Concluindo, a traducao, a adaptacao transcultural e as qualidades psicometricas do PMQ foram satisfatorias, o que viabiliza sua aplicacao em futuros estudos no Brasil.

Performance of different diagnostic criteria of overweight and obesity as predictors of metabolic syndrome in adolescents

OBJECTIVE To analyze the performance of three different diagnostic criteria of overweight and obesity (WHO, IOTF and Conde and Monteiro) using body mass index (BMI) as predictors of metabolic syndrome (MetS) in a representative sample of adolescents. METHODS A sample of 1035 adolescents aged 12-20 years (565 girls and 470 boys) was used in the study. BMI was calculated through the quotient of weight (kg)/height squared (m)2, and MetS was defined according to the criteria of the International Diabetes Federation. Sensitivity, specificity, and overall accuracy (area under the curve) were estimated using the receiver operating characteristic (ROC) curves method and used to describe the predictive performance. RESULTS The three diagnostic criteria showed higher absolute values of sensitivity and specificity for predicting MetS in boys and older adolescents. The highest sensitivity to identify MetS was found using the IOTF criterion (60-85%), while specificity values ≥ 90% were found for the three criteria. The Conde and Monteiro diagnostic criterion pointed to a significantly lower overall accuracy (0.52-0.64) than that of the WHO (0.70-0.84) and IOTF (0.75-0.89) diagnostic criterion. CONCLUSIONS Overweight and obesity using BMI showed a moderate association with MetS, regardless of the diagnostic criteria used. However, the IOTF criterion showed better predictive capacity for the presence of MetS than the WHO and the Conde and Monteiro criteria.

Physical Fitness and Metabolic Syndrome in Brazilian Adolescents: Validity of Diagnostic Health Criteria

We tested health criteria cutoff points from physical fitness variables proposed by the Fitnessgram program for detecting metabolic syndrome (MetS) in a representative sample of 1,035 Brazilian adolescents (565 girls and 470 boys) aged 12-20 years. We studied five fitness tests: (a) back-saver sit and reach, (b) trunk lift, (c) curl-up, (d) push-up, and (e) progressive aerobic cardiovascular endurance run (PACER), and we defined MetS through the criteria of the International Diabetes Federation. We described the predictive performance of each fitness test in terms of sensitivity, specificity, and area under the curve (AUC) estimates of the receiver operating characteristic method. The AUCs for four tests (back-saver sit and reach, trunk lift, curl-up, and push-up) revealed low accuracy for identifying MetS in both sexes (0.50 ≥ AUC ≤ 0.60), but AUCs for the PACER test demonstrated good accuracy values between 0.72 (0.68–0.76) and 0.83 (0.78–0.89). Thus, cutoff points proposed in the Fitnessgram program for the PACER test offer an effective means of screening adolescents at higher cardiometabolic risk, which, for the age-group used in this study, should be from 40.3-44.3 ml/kg/min for boys and 38.6-40.1 ml/kg/min for girls.

Anthropometric Nutritional Status in Children from a School Community in Southern Brazil: Comparison with WHO-2007 Reference

Identify nutritional inadequacies in specific segments of the child population may offer subsidies for public health intervention. The aim of this study was to assess the anthropometric nutritional status in a sample of children from a city in the southern region of Brazil. The sample comprised 23 thousand subjects of both sexes who ranged in age from 5 to 10 years. The anthropometric nutritional status was identified by height and body weight. The comparisons were based on the corresponding distributions of percentiles of the Word Health Organizations, 2007 reference. Analysis of results indicated that the distributions of percentiles of the height and BMI values in both sexes were statistically different of those from WHO-2007 reference. The data showed high proportion of children with low height ( 90 Percentile). The nutritional impairment translated by the high proportion of low height and excess body weight alerts to the need to implement public policies for healthy eating practices and physical activity. Resumo Identificar inadequacoes nutricionais em segmentos especificos da populacao infantil pode oferecer subsidios para intervencao em saude publica. O estudo analisou estado nutricional antropometrico em amostra de criancas escolares de uma cidade da regiao sul do Brasil. A amostra foi constituida por 23 mil sujeitos de ambos os sexos, com idade entre 5 e 10 anos. O estado nutricional antropometrico foi identificado mediante medidas de estatura e peso corporal. As comparacoes estatisticas foram estabelecidas com base nas correspondentes distribuicoes de percentis da referencia proposta pela Organizacao Mundial da Saude, 2007. Os resultados encontrados mostraram que, em ambos os sexos, as distribuicoes de percentis equivalentes aos valores de estatura e peso corporal foram estatisticamente diferentes da referencia OMS- 2007. Foram identificadas elevadas proporcoes de criancas com baixa estatura ( 90 Percentil). O comprometimento nutricional traduzido pela elevada proporcao de baixa estatura e excesso de peso corporal alerta para necessidade de implantar politicas publicas direcionadas as praticas de alimentacao saudavel e atividade fisica. “Burnout” em uma amostra de profissionais de Educação Física brasileiros

Burnout is the result of prolonged exposure to chronic work stress with insufficiente recovery. This study examined the presence of burnout in a sample of physical education professional from the metropolitan region of Londrina, Parana, Brazil. A total of 588 subjects (273 women and 315 men) were included in the study. To define the presence of burnout we used the version translated into Portuguese of the Maslach Burnout Inventory. Burnout was defined by superposition of high scores in emotional exhaustion and depersonalization and low scores on personal accomplishment at work. We also studied the effect of gender, age and job characteristics (working experience, academic qualification, area of professional practice, weekly workload, workplaces and income) on the three dimensions of burnout. The results indicated the presence of burnout in 10.2% of the selected sample. Men reported scores on reduced personal accomplishment indicative of greater severity than women. Scores equivalent to emotional exhaustion and depersonalization were significantly higher in older professionals. Regarding to job characteristics, more working experience, qualification only at graduation, professional practice in primary education, weekly workload ≥ 40 hours/week, multiple employment and lower income increased significantly the odds of the presence of burnout. In conclusion, the present findings can be used to design programs of intervention and implement changes in the work environment aimed at improving occupational health and overall well-being of profissional physical education.
