Risk factors for Toxoplasma gondii infection in women of childbearing age

OBJECTIVES Determine the risk factors involved in toxoplasmosis transmission and determine whether pregnancy is a risk factor for toxoplasmosis infection. STUDY DESIGN Cross-sectional study carried out on 2,242 women at childbearing age. An indirect immunofluorescence reaction was used to identify immunity to Toxoplasma gondii. Previous gestations were also analyzed as a possible risk factor. The results were analyzed by chi2 and OR tests, and by variance analysis. The sample was statistically balanced according to social-economic risk factors. RESULTS Previously pregnant women were 1.74 times more frequently infected with toxoplasmosis, regardless of environmental conditions. Pregnant women living under unfavorable environmental conditions had an approximately two times increased risk of being infected for each risk factor (contact with host animals, presence of vehicles of oocyst transmission). Previous pregnancy was the risk factor that had the strongest influence on acquiring toxoplasmosis (variance analysis and statistical balancing). DISCUSSION The prevalence of this zoonosis is high in Goiânia-GO, Brazil (65.8%). Inadequate environmental sanitation was not significantly correlated with toxoplasmosis infection, except when associated with previous pregnancy, showing that the fundamental cause for infection is not environmental. CONCLUSION The finding that pregnancy makes women more vulnerable to this protozoan, makes it important to implement prophylactic control of at-risk pregnant women.

Freqüência de soropositividade para antígenos de Toxocara canis em crianças de classes sociais diferentes

Frequency of seropositivity for Toxocara in children from different socioeconomic strata in the city of Brasilia (Brazil) was measured. Six hundred and two children of both sexes, aged one to 12 years were distributed in two socioeconomically distinct groups. The samples of sera of the first group were obtained from blood drawn for routine tests in the laboratory of a public hospital attending children from low-income families. Samples from the second group were obtained from private laboratories attending children from middle-class families. Anti-toxocara antibodies were detected by ELISA, using Toxocara canis excretory-secretory antigens previously absorbed with Ascaris suum extract. The prevalence of seropositivity was 21.8% (66/302) in the first group and 3% (9/300) in the second (p< 0.0001). No differences in frequency according to age or sex could be detected. Our results suggest a high prevalence of childhood toxocariasis in Brasilia, with children from lower income brackets being the most affected.

Autism spectrum disorder and celiac disease: no evidence for a link

OBJECTIVE To evaluate the possible association between celiac disease (CD) and/or gluten sensitivity (GS) and autism spectrum disorder (ASD). METHODS Occurrences of CD were determined in a group of children and adolescents affected by ASD and, conversely, occurrences of ASD were assessed in a group of biopsy-proven celiac patients. To detect the possible existence of GS, the levels of antigliadin antibodies in ASD patients were assessed and compared with the levels in a group of non-celiac children. RESULTS The prevalence of CD or GS in ASD patients was not greater than in groups originating from the same geographical area. Similarly the prevalence of ASD was not greater than in a group of biopsy-proven CD patients. CONCLUSION No statistically demonstrable association was found between CD or GS and ASD. Consequently, routine screening for CD or GS in all patients with ASD is, at this moment, neither justified nor cost-effective.

Emerging fluoroquinolone-non-susceptible group A streptococci in two different paediatric populations

Clonal emergence of group A streptococci (GAS) with reduced susceptibility to fluoroquinolones (FQs) has been increasingly reported. Non-susceptibility is associated with various point mutations in the target-encoding genes and has only been described in a few emm types. We used a well-characterised GAS clinical paediatric collection from Brussels (Belgium) and Brasília (Brazil) to analyse the molecular basis of FQ non-susceptibility. GAS strains were tested for ciprofloxacin susceptibility and were screened for mutations in DNA gyrase- and topoisomerase IV-encoding genes. Genetic relationships between the different emm types were assessed by phylogenetic analysis of the whole surface-exposed part of the M protein. A high proportion (22.5%) of ciprofloxacin-non-susceptible isolates (minimal inhibitory concentration > or = 2mg/L) was found among the Belgian strains. They belonged mostly to emm type 6 (87%). In Brazil, 6% of the isolates, belonging to seven distantly related emm types, were non-susceptible. Our phylogenetic analysis showed that non-susceptibility may arise in various genetic backgrounds. Sequence comparison of the quinolone resistance-determining regions (QRDRs) of the ParC- and ParE-encoding genes from susceptible and non-susceptible isolates revealed that most of the mutations were found in both classes of isolates, indicating an emm type-linked polymorphism. In conclusion, we observed a clonal spreading of non-susceptible emm type 6 GAS strains in Brussels and a polyclonal distribution of non-susceptible isolates in Brazil. All the Brazilian and Belgian emm type 6 strains displayed a S79A/F mutation in parC that convincingly explains the non-susceptible phenotype.

Sleep deprivation, pain and prematurity: a review study.

The aim was to describe current reports in the scientific literature on sleep in the intensive care environment and sleep deprivation associated with painful experiences in premature infant. A systematic search was conducted for studies on sleep, pain, premature birth and care of the newborn. Web of Knowledge, MEDLINE, LILACS, Cochrane Library, PubMed, EMBASE, Scopus, VHL and SciELO databases were consulted. The association between sleep deprivation and pain generates effects that are observed in the brain and the behavioral and physiological activity of preterm infants. Polysomnography in intensive care units and pain management in neonates allow comparison with the first year of life and term infants. We have found few references and evidence that neonatal care programs can influence sleep development and reduce the negative impact of the environment. This evidence is discussed from the perspective of how hospital intervention can improve the development of premature infants.

Zinc supplementation may recover taste for salt meals

OBJECTIVE: To evaluate the effect of zinc on the appetite for salt foods in children aged 8 months to 5 years. METHOD: Double-blind, placebo-controlled study. Two groups of 20 children refusing to eat salt foods were followed during six months. The children in the first group received zinc chelate 1 mg/kg daily for three months. The second group received a placebo solution. The two groups were similar in terms of age, sex, weight, duration of breastfeeding, age at weaning, biochemical and hematological data. The response of children to treatment was informed by their mothers. RESULTS: 17/20 (85%) of the children receiving zinc chelate and 10/20 (50%) of the children receiving placebo improved their appetite for salt foods. The difference was statistically significant (p < 0.05, chi-square test). CONCLUSION: Zinc supplementation may improve the acceptance of salt foods by children.

Sintomas e sinais de respiração predominantemente oral em adolescentes com transtorno de déficit de atenção/hiperatividade e queixa de prejuízo escolar

PURPOSE: to characterize symptoms and signs of Mouth Breathing in children with Attention Deficit Hyperactivity Disorder and with complaints about school underachievement. The specific objectives were to relate the found characteristics to the Attention Deficit Hyperactivity Disorder types, gender and age of this population. METHODS: data were collected at ADOLESCENTRO; a public institution of the State Secretary of Health of the Federal District - Brazil. Nocturne and daytime symptoms that appeared in the age ranging between two and twelve-years-old were reported by the parents and were analyzed, as well as some signs cited on literature as being common characteristics of Mouth Breathing. RESULTS: a high frequency of Mouth Breathing characteristics were found to be associated with snoring, nocturne mouth breathing, rhinitis, sialorrhoea, dark undereye circles and dry lips. Related to the symptoms, an association was found among tonsillitis and Attention Deficit Hyperactivity Disorder types; sialorrhoea on the pillow with masculine gender and daytime mouth breathing; nocturne mouth breathing, sialorrhoea on the pillow with age. An association was found between non-functional lips with age and non-functional tongue with age. CONCLUSION: the possibility of simultaneous existence of Mouth Breathing on children with Attention Deficit Hyperactivity Disorder and complaints about school underachievement requires a deeper diagnostic investigation.

The formation of citizens : the pediatrician’s role

OBJECTIVE This review article aims to define the fundamental role of the pediatrician in the formation of citizens in the 21st century. SOURCE OF DATA Significant bibliographical contributions produced by neuroscience, ecology, and epigenetics in the early childhood scenario. SYNTHESIS OF DATA Many diseases that impair the lives of adults result from severe and often uncontrollable disorders that occur in early childhood, an irreplaceable period for the safe construction of the human brain, personality, and intelligence. There is noteworthy scientific evidence that has become unquestionable, according to which abuse and neglect and other forms of violence to which children are exposed during the course of their lives, are the genesis of many physical ailments and other mental diseases, including depressive morbidity and schizophrenia. Conversely, it is also emphasized that healthy practices such as reading and listening to/playing music are able to intensively contribute to the exercise of cognitive capacity inherent to this period of life, as a prerequisite for the acquisition of learning indispensable to the high educational performance during the schooling period. CONCLUSION In the light of the disclosed scientific evidence, the pediatrician emerges as the most differentiated professional to provide preventive and curative care indispensable to the skilled formation of a healthy citizen.

Indicadores da escolha de pediatria por graduandos da universidade de Brasília: estudo longitudinal em série histórica

The authors analyzed Pediatrics as career choice in a study involving 877 graduates from the University of Brasilia from 1994 to 2007. Data included demographics, learning attributes, early career preference, peer-tutoring experience, academic achievement, selective training in the last semester, and choice of residency training. Contingency and logistic regression analyses were performed with the graduates grouped according to choice of Pediatrics versus all other options. Overall, 9.7% of graduates had chosen Pediatrics, of whom 44.7% had indicated early attraction to the specialty, while another 45.9% did not express interest until their clinical training. No significant trend was shown for early attraction, but a lower proportion of graduates chose Pediatrics in the last 6 years, in parallel with fewer applications for tutoring experience and selective training. Logistic regression identified five predictors of choice in decreasing order of effect size: selective training, overall achievement, reflective learning style, ranking of early preference, and time of graduation. Independent ef fects of gender and peer tutoring were not statistically significant. In conclusion, the predictors of choice of Pediatrics comprised personal and curricular features in distinct patterns of interest and influence, within an overall context of decreasing choice of the specialty in recent years.

Medidas do tendão do calcâneo no primeiro ano de vida

OBJECTIVE: To determine the thickness and width of the calcaneal tendon (Achilles tendon) in both male and female, eutrophic children at 2, 6, 9 and 12 months of age. MATERIALS AND METHODS: The present prospective and descriptive study included 38 boys and 31 girls. Measurements of the tendons were performed by means of ultrasonography, with a linear, 14-MHz probe, at the level of the medial malleolus. RESULTS: Tendon thicknesses observed in the boys were 2.4 mm, 2.3 mm, 2.4 mm, and 2.4 mm, respectively at 2, 6, 9 and 12 months. Widths were, respectively, 6.0 mm, 6.4 mm, 6.7 mm and 7.1 mm. In the girls, the tendon thicknesses observed at 2, 6 and 9 months were 2.4 mm, and 2.5 mm at 12 months. Widths were, respectively, 5.7 mm, 6.2 mm, 6.5 mm and 6.5 mm. CONCLUSION: No difference was observed in the calcaneal tendon thickness between boys and girls along the first year of their lives; however, width measurements demonstrated differences.
