Web-based application of Community Health Center (Puskesmas) for patient electronic medical records

This research provides the design and implementation of a web-based software application of Community Health Center/ CHC (Pusat Kesehatan Masyarakat = Puskesmas). The application is mainly used for recording patients personal identity and medical treatment at medical services (polyclinics) of CHC. Generally, the polyclinics at Puskesmas provides are General Medical Treatment Service (Poli Umum), Maternal and Infant Service (Poli Ibu dan Anak), Dental Service (Poli Gigi), and Drug Service. With the application, all data of patients personal identity and medical treatment at medical services of Puskesmas are recorded and integrated in an attempt to create an accurate report based on the reporting format standard set by Ministry of Health, Indonesia. Furthermore, on technical side, the application is built up with open source-based technology, so a license is not required to use it. Relational Database Management System, MySQL, Apache web server and PHP are aids software which is operasionalized to initially implement the system. On the other hand, to implement the application a web browser is needed. The application is equipped with user authentication, That is used to ensure that the application is accessed only by appropriate medical personnel. Users categorized in administrator level have a higher authority than the ones categorized as operator. In fact, the application has been implemented at some of CHCs in Depok Municipality. Moreover, the initiative of the application has been developed for infant malnutrition surveilance. Some of the data obtained from Puskesmas are going to be seriously followed up to make effective decisions to take curative actions on infant malnutrition cases.

The Development of Nursing Care Services Model for Low Birth Weight Infants

Introduction: Low birth weight (LBW) infants deal with various problems during transitional period from intra-uterine and extra-uterine because of immature organs’ functions. This leads to LBW as the second death cause in Indonesia, particularly in the fi rst seventh days of infants’ lifes. The problem continues to occur at home when the infants have discharged. This research was aimed to develop the nursing care services model for LBW infants and to test the model. Method: The research design was an action research using quantitative and qualitative approach. This design was chosen as it facilitated improvement in health care system, which was involving nurses and other health providers. Results: Nursing care services provided by the nursing team are hindered by several factors, such as various level of nurses’ knowledge, not optimal health education activities, incomplete standard operational procedure, ethical dilemma, paramedic functions, and documentation system. This model was developed based on conservation and becoming a mother/maternal role attainment theory, family-centered care principles, and input from the experts through focus group discussion. Conclusion: The result of this research is going to increase the quality of nursing care for LBW infants by achieving nurses’ and parents’ satisfaction in giving care for their infants which can lead to lower infant death rate.

Heat Loss Prevention Education Aids Nurses’ Knowledge in Prevention of Hypothermia in Newborns

ABSTRACT Newborns are at risk of developing hypothermia due to the instability of their thermoregulation systems and inability to adjust to changes in ambient temperature. This risk increases during hospitalization. Education is a strategy to improve nurses’ understanding of the prevention of hypothermia in newborns in the hospital. This study aimed to analyze the impact of education about preventing heat loss on nurses’ knowledge and behavior in preventing hypothermia in newborns. The study design was a quasi-experiment with one group pre-test–post-test and a total sample of 21 neonatal nurses who were selected using the consecutive sampling method. The results indicated that there was significant difference in nurses’ knowledge and behavior pre- and post-education intervention (p < .001; α = .05). This study suggests that education on prevention of heat loss is an effective strategy to promote nurses’ knowledge and behavior in preventing hypothermia.

The Quarter Prone Position Increases Oxygen Saturation in Premature Infants Using Continuous Positive Airway Pressure

ABSTRACT A primary problem that occurs in premature infants is oxygenation disorders, thus requiring respiratory support, including continuous positive airway pressure (CPAP). The effectiveness of CPAP can be improved by adjusting the body’s position, so the aim of this study was to examine the effect of the quarter prone position on the oxygenation status of preterm infants using CPAP. This study used a randomized controlled trial with a crossover design. A group of 15 preterm infants receiving CPAP was selected, and randomization of allocation was done to divide the respondents into the intervention group (quarter prone) or the control group (supine). Oxygenation status was measured using an observation form, and the result showed a significant difference in the oxygen saturation levels of premature infants using CPAP in the quarter prone group compared to that in the supine group (p = .045). The quarter prone position was effective for improving the oxygenation status of premature infants using CPAP. It is recommended that the quarter prone position be applied as part of nursing care in neonatal nursing.

Sensitivity, Specificity, and Accuracy of Kramer Examination of Neonatal Jaundice: Comparison with Total Bilirubin Serum

ABSTRACT Neonatal jaundice often occurs in the first week after birth. Early detection of neonatal jaundice can prevent kernicterus as a complication of high levels of bilirubin in neonates. Visual assessment is one of the examinations introduced by Kramer to assess the presence of neonatal jaundice. The purpose of this study was to determine the sensitivity, specificity, and accuracy of Kramer’s visual assessment in the examination of neonatal jaundice. This study used cross-sectional design to examine 102 infants, with the sample selected by the convenience sampling method. A 2 × 2 table diagnostic test was employed in this study to analyze the data. The results of this study showed that visual assessment of neonatal jaundice obtained a sensitivity value of 76.92%, a specificity value of 89.47%, and an accuracy value of 86.27%. This research can contribute to the development of other non-invasive methods for assessing neonatal jaundice, and thereby contribute to a decrease in neonatal morbidity.
