Significados atribuídos por puérperas adolescentes à maternidade: autocuidado e cuidado com o bebê

The objectives of this study were to identify the meanings attributed by adolescent mothers towards care for their babies and self-care, and to identify what they recognize as difficulties during their postpartum period and concerning caring for their respective babies. This study used a qualitative approach that took place in a maternity hospital that attends women through the Brazilian National Health Care System (Sistema Unico de Saude) from January to March of 2005. The subjects were 22 adolescent first time pregnant women. The data was collected via semi-structured interviews, recorded, and submitted to posterior thematic content analysis. Maternity is understood as a psychological, social, and physical change with maternal care as a duty to be exercised, taking priority when compared with self-care. The lack of experience, information, and their financial situations were pointed out as the main difficulties to care, with family listed as support. Society underestimates the adolescents capacity to care for the child, needing to create strategies for developing the adolescents autonomy in order that she actively assume her maternal responsibilities. O espaço social das mulheres e a referência para o cuidado na prática da amamentação

This study aimed to identify agents or institutions taken as reference by women when breastfeeding. A qualitative study was carried out on 20 primiparous who were assisted, for reasons not related to breastfeeding, in the five health services selected by this study. Data were collected by semi-structured interviews carried out in the participants’ households and were analyzed by content analysis in the thematic mode. We identified that health professionals play a standardize role of breastfeeding based on scientific knowledge. In the daily breastfeeding routine, the family is the first reference for women, transmitting beliefs, habits and behaviors. We believe in the valorization of the family context by the health professional, in which actions and interactions in the breastfeeding issue are developed in order to constitute the foundations for a new care model in breastfeeding. This model should, therefore, consider the practice diversity, adapting actions to the multiple roles of being mother/fortress/wife/worker in the social context.

Adolescent pregnancy: a comparative study between mothers who use public and private health systems

This is a comparative and descriptive study of adolescent mothers who were attended in three maternities of the public health system and three private maternities in a city in Sao Paulo, Brazil, between 2000 and 2002. This study aimed to compare the profile of mothers attended in both systems. The database of Ribeirao Preto was used and 5,286 adolescent mothers between 10 and 19 years old were selected according to type of delivery, level of instruction, number of prenatal consultations and parity. We found that the users of the public health system had less prenatal consultations, lower level of education, higher parity and the vaginal delivery was most frequent. Amamentação e o seio feminino: uma análise sob a ótica da sexualidade e dos direitos reprodutivos

A pesar del conocimiento sobre el amamantamiento, reunido por cientificos y profesionales de la salud, a lo largo del siglo XX, el destete precoz persistio en este periodo como una causa importante para el aumento de la morbilidad y la mortalidad infantil. Desde el ano 1980, las politicas publicas en pro del amamantamiento empezaron a ser desarrolladas, principalmente en los paises de tercer mundo. Sin embargo, las mujeres principales actoras en el proceso del amamantamiento, muchas veces no son escuchadas en el momento de cumplir la funcion de amamantar. Este estudio aporta algunas reflexiones sobre el amamantamiento, enfocando la sexualidad y los derechos reproductivos de las mujeres en la cultura occidental. Esta dirigido a los profesionales de la Salud que trabajan con las mujeres en el ciclo gravido puerperal, para permitirles comprender y apoyar a las mujeres en sus decisiones sobre el proceso de amamantamiento.

Percepção das mulheres acerca do contato precoce e da amamentação em sala de parto

OBJETIVO: conocer y analizar las vivencias de las mujeres relacionadas al cuarto paso de la Iniciativa Hospital Amigo da Crianca, en una maternidad de Ribeirao Preto. METODOS: Fue utilizado el abordaje cualitativo. Los datos recolectados por medio de entrevistas semi-estructuradas fueron analizados a traves del analisis de contenido, modalidad tematica. RESULTADOS: Participaron 23 mujeres las cuales relataron que, aun estando en situacion inadecuada debido al cansancio y al dolor, aceptan con resignacion y entienden el momento como importante para el bebe. Ademas, recibir al nino por primera vez causa impacto y sorpresa. CONSIDERACIONES FINALES: La asistencia se muestra limitada a los aspectos practicos del cumplimiento del cuarto paso, sin considerar los reales sentimientos de las mujeres, siendo necesario el desarrollo de habilidades de comunicacion y empatia del profesional de salud.Objective: To describe women’s experiences with the fourth step of the Hospital Baby Friendly Initiative. Methods: A qualitative study was undertaken to collect data from 23 women admitted to a maternity hospital, in Ribeirao Preto, Brazil. Data were collected through semistructured interviews and submitted to content analysis. Results: Women stated that earlier contact and breastfeeding the newborn in the delivery room was very important for the newborn. In addition, women stated that receiving the newborn for the first time made them surprised and emotional. Final Considerations: There are several limitations in the application of the fourth step in clinical practice. Health care providers must be sensitive to women’s feelings, and must develop empathy and therapeutic communication.

Mensuração e características da dor perineal em primíparas submetidas à episiotomia

OBJETIVO: Mensurar e caracterizar a percepcao dolorosa das puerperas primiparas submetidas a episiotomia. METODOS: Trata-se de uma pesquisa descritiva com abordagem quantitativa, realizada com 40 puerperas primiparas submetidas ao parto normal com episiotomia. Para mensuracao da dor foi utilizada a escala de categoria numerica e para caracterizacao a versao brasileira do questionario McGill - Br-MPQ. RESULTADOS: Os valores encontrados na avaliacao da dor foram de media 4,2 e os descritores que melhor caracterizaram a dor foram: dolorida; que repuxa; incomoda; chata; ardida; pica como uma agulhada; latejante; em pressao. CONCLUSAO: A intensidade da dor perineal foi considerada como moderada pelas puerperas. Na caracterizacao da queixa dolorosa os descritores mais citados foram da dimensao sensorial. Este estudo possibilitou observar a necessidade do reconhecimento dos aspectos qualitativos e quantitativos da dor na pratica clinica obstetrica.

Mortalidade materna na perspectiva do familiar

Maternal mortality is one of the health and social development indicators for countries. It is a tragedy for the family, as the death of a mother deprives the child of breastfeeding and maternal contact and also due to the fact that women maintain family unity. This study is aimed at understanding the meaning of death for the family members of women who have died. The population was formed by 10 family members of 7 women who died as a result of maternal problems in the municipality of Ribeirão Preto. The authors visited their homes and collected data through interviews. For analyzing the data the authors used thematic analysis. Three thematic categories were found: the meaning of maternal mortality; living maternal mortality in the family; and living maternal mortality in the health services. The visit confirmed that there are secondary factors that influenced in the occurrence of maternal deaths.Maternal mortality is one of the health and social development indicators for countries.
