The use of an integral tissue expander® for primary breast reconstruction

Ron Lapin; Martin Elliott; Hugo Juri

The authors emphasize the recent advances in breast reconstruction provided by use of the skin expander device. A Silastic percutaneous skin expander developed by the authors and the Dow Corning Corporation has been used by them in breast reconstruction for the past 7 years with satisfactory results. Important design differences in the device suggested by the authors include the use of a reinforced backing which consists of Dacron Silastic sheeting extending beyond the inflatable teardrop-shaped envelope for approximately 3/8 of an inch, thus adding strength and rigidity to the envelope and the rim, and making it possible for the rim of the expander implant to be sutured in an appropriate orientation.

Physics is the science that studies the interaction between matter and energy. This discipline studies the general properties of bodies, the forces that modify the transfer of energy and the interaction between particles. Physics now has many branches, and one of them is the Biomedical Physics. Biomedical Physics in Medicine applies the principles and methods of physics. This will generate new knowledge and progress towards new horizons in the management of certain diseases. Thus the study of radiation led to progress in the field of diagnosis and therapy of some diseases. Radiation its called all energy that propagates as a wave through space. Radiation can be classified as ionizing radiation (cosmic rays, gamma rays, x-rays) and non-ionizing radiation. The concept of non-ionizing radiation includes ultraviolet (UV), infrared (IR) and others lasers. Non-ionizing radiations are those that radiation interaction with matter does not generate ions due to its energy content is relatively low. Non-ionizing radiation (laser, ultraviolet and infrared) are commonly used in medicine for diagnosis and therapy. However, they have a deleterious effect on organogenesis (organ formation).

CO2 Laser Effects on Dentin Root Canals: Studies with the Scanning Electron Micrograph

Sonia L. Ferreyra; Marta E. Crosa; Hugo Juri; Alejandro Ferreyra

ABSTRACT The aim of this work was to study the structural aspects of dentin surface by CO2 laser irradiation. One hundred specimens were lased with 3 W for 1, 3, 5, 10, and 15 sec. The irradiated hemisections were processed for SEM observation. With 3 W power emission during 1 sec the samples revealed a crusty wavy aspect. At the edges of the crusts, the dentine had been melted, and was slightly separated from the wall showing an actual cutting effect. When the irradiation time (IT) was increased to 10 and 15 sec fungi formations were observed. Melted recrystalized dentin rims were visible in certain areas. CO2 laser irradiation induces coagulation, vaporization, and melted tissue with cracks and carbonizations areas.

In previous investigations it was found that prostaglanding E1 (PGE1) and Bradiquinine (B), liberated in the inflammatory process, produced a significant increment on the Plasma fibrition Level (PFL), indicative of inflammatory process. The mentioned increment was completely abolished by the irradiation with HeNe laser in the area of infection of the mentioned substances. In the current investigation it was studied the effect of HeNe laser radiation on the P.F.L. of rats injected with another substance related with the inflammatory process and tissular injury: Thromboxane (Tx). It is though that the signal to increase P.F.L. is though an adrenal and an extra adrenal pathways. To study it we injected normal and medullectomized animals and both showed marked increment of P.F.L. Then we repeated the experiment but followed immediately by HeNe laser radiation and we noted a complete blockage of the P.F.L. increment in both groups which suggest that the effect is extra-adrenal. All substances were injected I.M. once daily X 3 days. Immediately after injection the area was irradiated with HeNe laser, 1.5 J total energy. In the normal, non injected non irradiated animals the P.F.L. reached 210.3 + 1.15 mg%. The single injection of Tx did not modify the P.F.L. compared with the previous group. The HeNe irradiation alone did not modify the P.F.L. in the animals injected with Tx only. The animals injected with PGE1+B showed a marked increment of P.F.L. to 337.6 + 14.5 mg%; the HeNe laser radiation completely abolished the increment (231 + 22.3 mg%). But in the animals injected with PGE1+B+Tx, the P.F.L. reached even larger values: 375.2 + 15.3 mg%. The HeNe laser radiation produced a partial blockage in P.F.L. increment (270.3 +/- 13.4 mg%). Showing a significant difference (p < 0.001) compared with normal rats or with rats injected with PGE1+B+Tx without radiation. In conclusion Tx potentiate the effect of PGE+B on the P.F.L. The HeNe laser blocks completely the interaction of PGE1+B on the P.F.L. but only partially the interaction PGE(subscript 1+B+Tx.
