This study aims at characterizing the social representations of the nurse in the intensive care unit (ICU) by identifying the central core and the peripheral system. It was carried out in five ICUs from both public and philanthropic teaching, research and assistance hospitals in Salvador (Bahia, Brazil). Data were gathered from ninety nurses by means of free evocation from the phrase: nurses work in an ICU and then processed with the EVOC software. Analysis was performed by building a four-digit chart based on the structural approach to the theory of social representations. Results point out stress, responsibility, integral care and gratification as the core elements. The peripheral system comprised personal and professional attitudes deemed necessary to perform the work. This is a retrospective case study of a patient affected by toxic epidermal necrolysis in the intensive care unit of a public hospital, with the goal to apprehend, starting from the clinical judgments of the nurses, theirs nursing diagnoses. Thirteen nursing diagnoses were evidenced and, also, it was evidenced the necessity of the theoretical improvement of those professionals about the Systematization of Nursing Care, and on the sense of value that this practice may add to nursing in the pursuit of individualized assistance to the patients under their care.Es un estudio de caso clinico, retrospectivo, de un paciente afectado por necrolisis epidermica toxica, internado en una unidad de terapia intensiva de una organizacion hospitalaria publica, con el objetivo de aprehender, a partir de los juicios clinicos de las(los) enfermeras(os), los diagnosticos de enfermeria. Fueran evidenciados trece diagnosticos de enfermeria, asi como la necesidad de actualizacion teorico-practica de estas profesionales, sobre la Sistematizacion de la Asistencia de Enfermeria y el sentido de valor que esta practica puede anadir a la Enfermeria, en la conduccion de la asistencia individualizada a los pacientes bajo sus cuidados.

O trabalho das enfermeiras de unidade de tratamento intensivo (UTI) e complexo, devido a diversidade de tecnologiase atencao a pessoas em risco de vida, e exige a capacidade de decidir e agir com autonomia. Objetivou-se apreendere analisar as representacoes sociais de enfermeiras intensivistas sobre sua autonomia no trabalho na UTI. Estudoexploratorio descritivo com abordagem quantitativa e qualitativa realizado em dois hospitais de grande porte, nacidade de Salvador, Bahia, de maio a junho de 2006, com 24 enfermeiras. Os dados coletados nas entrevistas foramanalisados segundo a tecnica de analise de conteudo, a luz da teoria das representacoes sociais. Identificaram-secinco categorias simbolicas: Manifestacoes, Fatores intervenientes, Valorizacao, Atributos associados e Aspectospsicossociais da autonomia. Concluiu-se que a autonomia, considerada um instrumento de trabalho da enfermeiraem processo de consolidacao, esta condicionada ao conhecimento, discernimento, atitude, postura profissional,respeito e reconhecimento da equipe de saude.(AU) The objective of this study was to seize and analyze the social representations of critical care nurses about theirautonomy in the intensive care unit. The work of the nurses at the intensive care unit is complex due to the diversityof technologies and attention to people at risk of life; furthermore, it requires the ability of making decisions and dealwith autonomy. This is a descriptive exploratory study based on quantitative and qualitative approach implementedat two major hospitals in Salvador, Bahia. The data were collected with 24 nurses selected randomly from May to Junein 2006. These findings were analyzed according to the content analysis, based on the social representation theory. Inthis study five symbolic categories were identified: Manifestations, Intervening factors, Valuation, Associated attributesand Psychosocial aspects of autonomy. It can be concluded that autonomy, which is considered a work instrumentof the nurse in consolidation process, is conditioned to knowledge, judgment, attitude, professional behavior, respect,and acknowledgment of the health team.(AU) El trabajo de las enfermeras de la unidad de cuidados intensivos (UCI) es complejo, debido a la diversidad detecnologias y a la atencion a personas con riesgo de vida y, exige, la capacidad de decidir y actuar con autonomia.Se objetivo aprehender y analizar las representaciones sociales de las enfermeras intensivistas sobre su autonomiaen el trabajo en la UCI. Estudio exploratorio, descriptivo con enfoque cuantitativo y cualitativo, realizado con24 enfermeras, en dos hospitales de grande porte, en la ciudad de Salvador, Bahia, durante el periodo de mayoa junio de 2006. Los datos recolectados en las entrevistas fueron analizados a traves de la tecnica del analisisde contenido, a la luz de la teoria de las representaciones sociales. Se identificaron cinco categorias simbolicas:Manifestaciones, Factores de intervencion, Valorizacion, Atributos asociados y Aspectos psicosociales de laautonomia.
