Content validation of current and new defining characteristics of the nursing diagnosis: decreased cardiac output

Objective: To validate the content defining characteristics of the currently approved and of those identified through reviewing the literature, for the nursing diagnosis: decreased cardiac output. Methods: Is a content validation study using the model proposed by Fehring. The defining characteristics were validated by 18 experts (eight doctors and ten nurses), using the five points Likert scale and according to the following classification: scores higher than 0.80 were considered as the main features; those with weight between 0, 50 and 0.79 as secondary; and, those with average less than or equal to 0.50 as irrelevant. Results: Of 79 defining characteristics investigated, 38 (48.1%) were validated, among which 17 were present in NANDA-I, and 41 (51.9%) were considered as irrelevant to this diagnosis, of these, eight are listed in NANDA-I. Defining Characteristics of Decreased Cardiac Output: A Literature Review

OBJECTIVE.  The study aims to proceed a literature review of defining characteristics (DCs) of decreased cardiac output (DCO). METHODS.  Medline database was used to perform this study. The descriptors used were “low cardiac output” and “nursing diagnosis.” RESULTS.  Seventy-nine DCs were identified. Among them, 28 have already been approved by NANDA-I. Some data from microcirculation assessment such as high levels of serum lactate and decreased oxygen venous saturation were identified as indicators of this nursing diagnosis. CONCLUSIONS.  Some of the approved DCs were identified through literature review but others seem to be new as they have not been cited in the NANDA-I classification. PRACTICAL IMPLICATIONS.  Further content and clinical validations are needed to confirm if data from microcirculation might be considered as DCs of DCO. OBJETIVO.  Realizar uma revisao da literatura das caracteristicas definidoras (CD) do Debito Cardiaco Diminuido (DCD). METODOS.  A base de dados do Medline foi utilizada neste estudo. Os descritores utilizados foram “baixo debito cardiaco” e “diagnostico de enfermagem”. RESULTADOS.  Foram identificadas 79 CD. Dentre estas, 28 ja estavam contidas na classificacao de diagnosticos da NANDA-I. Alguns dados referentes a avaliacao da microcirculacao como os niveis de lactato aumentados e diminuicao da saturacao venosa de oxigenio foram identificados como indicadores deste diagnostico. CONCLUSOES.  Algumas CD aprovadas pela NANDA-I foram identificadas na revisao da literatura, entretanto, outras parecem ser novas, uma vez que nao encontram-se citadas nesta classificacao. IMPLCIACOES PARA PRATICA.  Novos estudos de validacao de conteudo e clinica devem ser realizados para confirmar se os dados sobre a microcirculacao podem ser considerados como CD do DCD.

Special report: Nursing diagnoses and interventions for a child with dilated cardiomyopathy requiring intra-aortic balloon pump support--case report.

OBJECTIVE.  The study aims to identify the nursing diagnoses and interventions for a child requiring intra-aortic balloon pump support. METHODS.  This was a case study of a 7-year-old child. The nursing diagnoses and interventions were obtained by three cardiology and pediatric specialist nurses, familiar with NANDA-International and Nursing Intervention Classification taxonomies. RESULTS.  Many nursing diagnoses and interventions were identified, but the main diagnosis for this patient was decreased cardiac output. CONCLUSIONS.  The nursing care for a child with an intra-aortic balloon requires well-qualified professionals, prepared to handle the diagnoses and interventions identified in this study. PRACTICAL IMPLICATIONS.  The use of a standard language system is essential to guide professionals in the caring process, providing essential elements to optimize interpretations. Resumo OBJETIVO.  Identificar os diagnosticos e as intervencoes de enfermagem de uma crianca com balao intra-aortico. METODOS.  Estudo de caso de uma crianca de sete anos em uso de balao intra-aortico. A coleta de dados foi realizada retrospectivamente por meio do prontuario. Os diagnosticos e intervencoes de enfermagem foram levantados por tres enfermeiras especialistas em cardiologia e pediatria e com dominio nas taxonomias da NANDA-I e NIC. RESULTADOS.  O principal diagnostico de enfermagem encontrado para este paciente foi o debito cardiaco diminuido relacionado a contratilidade alterada. Outros diagnosticos de enfermagem encontrados foram: risco de infeccao, risco de aspiracao, constipacao, integridade da pele prejudicada, nutricao desequilibrada: menos do que as necessidades corporais, risco de perfusao renal ineficaz, protecao ineficaz, risco de sangramento e volume de liquidos excessivo. Para cada diagnostico de enfermagem foram encontradas muitas intervencoes e atividades. CONCLUSOES.  A assistencia de enfermagem a uma crianca portadora de balao intra-aortico exige experiencia, profissionais qualificados e preparados para lidar com os diagnosticos e intervencoes identificados neste estudo. IMPLICACOES PARA PRATICA.  A utilizacao de um sistema padronizado de linguagem e fundamental para guiar os profissionais no processo de cuidar, possibilitando a organizacao dos nossos pensamentos, observacoes e interpretacoes. Para assegurarmos a efetividade das intervencoes apresentadas, sugerimos a realizacao de novos estudos, visando a aplicacao e avaliacao destas atividades de enfermagem. Escala de diferencial semântico para avaliação da percepção de pacientes hospitalizados frente ao banho

Objecti ve: To construct and validate a semantic differential scale to assess patients’ perceptions in regarding bathing. Methods: The first stage consisted of constructing a scale, conforming to specific theoretical parameters, and the second stage consisted of factorial validation procedures and calculation of the measure of reliability coefficients. One hundred thirty patients admitted to the Intensive Care Unit participated in the validation study. Results: The resulting measure of semantic differential presented as a product of a two-dimensional scale with Cronbach’s alpha reliability coefficients greater than 0.90. Conclusion: The scale can be considered a valid and reliable instrument for assessing patients’ perceptions regarding showers and bed baths.OBJETIVO: Construir y validar una escala de diferencial semantico que evalue la percepcion de los pacientes en relacion al bano. METODOS: La primera etapa, consto de la construccion de la escala, conforme los niveles teoricos especificos y la segunda etapa, estuvo compuesta por procedimientos de validacion factorial y el calculo de los coeficientes de confiabilidad de la medida. En el estudio de validacion participaron 130 pacientes internados en una Unidad de Cuidados Intensivos. RESULTADOS: La medida del diferencial semantico resultante presento como producto una escala bidimensional con coeficientes de confiabilidad alfa Cronbach superiores a 0,90. CONCLUSION: La escala puede ser considerada un instrumento valido y confiable para la evaluacion de la percepcion de los pacientes frente a los banos de ducha y en la cama.
