Dirofilariasis involving the oral cavity: report of the first case from South America

Oral dirofilariasis is very rare with non-specific clinical manifestations. Here, we report the case of a 65-year-old South American woman with a submucosal nodule on her right buccal mucosa. The nodule was slightly tender and painful. Differential diagnoses included mesenchymal (lipoma or fibrolipoma, solitary fibrous tumor, and neurofibroma) or glandular benign tumors (pleomorphic adenoma) with secondary infections. We performed excisional biopsy. A histopathological examination revealed a dense fibrous capsule and a single female filarial worm showing double uterus appearance, neural plaque, well-developed musculature and intestinal apparatus. Dirofilariasis was diagnosed, and the patient was followed-up for 12 months without recurrence.

Development of a mobile application for oral cancer screening

PURPOSE To develop a mobile application (app) for oral cancer screening. METHODS The app was developed using Android system version 4.4.2, with JAVA language. Information concerning sociodemographic data and risk factors for oral cancer development, e.g., tobacco and alcohol use, sun exposure and other contributing factors, such as unprotected oral sex, oral pain and denture use, were included. We surveyed a population at high risk for oral cancer development and then evaluated the sensitivity/specificity/accuracy and predictive values of clinical oral diagnosis between two blinded trained examiners, who used movies and data from the app, and in loco oral examination as gold-standard. RESULTS A total of 55 individuals at high risk for oral cancer development were surveyed. Of these, 31% presented homogeneous/heterogeneous white lesions with potential of malignancy. The clinical diagnoses performed by the two examiners using videos were found to have sensitivity of 82%-100% (average 91%), specificity of 81%-100% (average 90.5%), and accuracy of 87.27%-95.54% (average 90.90%), as compared with the gold-standard. The Kappa agreement value between the gold-standard and the examiner with the best agreement was 0.597. CONCLUSION Mobile apps including videos and data collection interfaces could be an interesting alternative in oral cancer research development.

NASOLABIAL CYST: A CASE REPORT

The nasolabial cyst is considered as lesion a little frequent and of etiology still unknown. The nasolabial cyst is a non-odontogenic cyst that happens in the contiguous area to the wing of the nose committing just soft tissues, responsible fact for the ala nosi elevation constituting important clinical sign for the diagnosis. The present article report a case of nasolabial cyst in a 28-years-old white woman, with complaint of partial obstruction of the left nostril, directed by otorhinolaryngologist with the information that to the radiographic exam of the sinus of the face there was mild mucous thickly in maxillary, frontal and ethmoidal sinus with good aeration of the nasal fossa and discreet deviation of the bony nasal septum to left. The lesion was painless and to the physical exam mild camber of the left anterior upper vestibular fornix, besides the elevation of the ala nosi on the same side. To the occlusal radiographic exam, it was noticed mild deviation of the left nasal floor. Aspiration was accomplished for citopathologic exam and surgical enucleation, being observed to the microscope a cavity covered by columnar epithelium and fibrous capsule, compatible with nasolabial cyst. EFICÁCIA DO USO DE CORTICOSTEROIDE SISTÊMICO NO TRATAMENTO DO PÊNFIGO VULGAR ORAL

Introduction: Pemphigus vulgaris is an autoimmune disease characterized by ulcerative and bullous lesions on skin and mucous membranes. The aim of this paper is to report a case of oral pemphigus vulgaris, emphasizing the treatment and discuss some clinical and histopathologic features of this disease. Case Report: The patient is a 38year-old male, feodermic, who was referred to a specialized service in oral diagnosis in the city of João Pessoa – Paraíba State, complains of multiple ulcerative lesions in the oral cavity, dysphagia, dysphonia, and marked weight loss. The diagnostic hypotheses were pemphigus vulgaris and mucous membrane pemphigoid. An incisional biopsy was performed on buccal mucosa and the diagnosis of the pemphigus vulgaris was made. Initially, the treatment of choice was systemic administration of prednisone 80 mg. In the first weeks, a remission of the oral lesions occurred. The dosage was gradually reduced to the current dose of 2.5 mg/day. Conclusion: Treatment was effective, once there was remission of all oral lesions. Introdução O pênfigo representa um grupo de doenças autoimunes e incomuns que se caracterizam pela presença de lesões bolhosas e ulceradas na pele e em mucosas(1). O termo “pênfigo” geralmente engloba quatro subtipos: vulgar, vegetante, eritematoso e foliáceo. Desses, o primeiro se destaca na Odontologia em razão da maior prevalência de acometimento da cavidade oral (aproximadamente 70%). A doença é mais comum em indivíduos de meia idade e não há predileção por sexo(1-2).
