Validation of indicators of the nursing outcomes classification for hospitalized adults at risk of infection

This study aimed to validate the indicators of the Nursing Outcomes proposed by the Nursing Outcomes Classification for the diagnosis Risk of Infection. Content validation was performed according to 12 nurse experts from the clinical, surgical and intensive care units of a university hospital. The analysis was based on the weighted arithmetic average of the scores the experts assigned to each indicator assessed and scores that reached at least 0.80 were validated. Out of 132 proposed indicators, 67 were validated for eight nursing outcomes described for the diagnosis Risk of Infection, which had been validated in a previous study. The content validation process identified that the Nursing Outcomes Classification presents feasible results and indicators to evaluate and identify the best care practices. This study will support the implementation of the Nursing Outcomes Classification in clinical practice, teaching and research.

Nursing Outcomes for the Diagnosis Impaired Tissue Integrity (00044) in Adults With Pressure Ulcer.

Purpose The study aims to validate nursing outcomes from the Nursing Outcomes Classification (NOC) related to the nursing diagnosis of impaired tissue integrity (00044) in adults with pressure ulcer (PU). Methods It was a consensus validation study using the focus group technique. Data were analyzed by descriptive statistics, and NOCs that obtained 100% consensus were considered validated. Findings Of the 16 NOCs evaluated, nine were validated. Conclusions The NOCs validated may identify the effectiveness, impact, and quality of interventions, making it possible to maintain or change the care planning of PU patients. Implications for Nursing Practice This work may serve to contribute to the future development of NOC, making it possible to evaluate the outcomes of PU patients in terms of the interventions performed in nursing practice. Objetivo O estudo tem como objetivo validar os resultados de enfermagem da Nursing Outcomes Classification (NOC) relacionados ao diagnostico de enfermagem Integridade Tissular Prejudicada (00044) em adultos com ulcera por pressao (UP). Metodos Trata-se de um estudo de validacao por consenso utilizando-se a tecnica de grupo focal. Os dados foram analisados pela estatistica descritiva e os resultados NOC que obtiveram 100% de consenso foram considerados validados. Resultados Dos 16 resultados NOC avaliados, nove foram validados. Conclusoes Os resultados NOC validados podem identificar a efetividade, o impacto e a qualidade das intervencoes, possibilitando manter ou alterar o planejamento da assistencia de pacientes com UP. Tempo despendido na execução do processo de enfermagem em um centro de tratamento intensivo

Este estudio tuvo como objetivo medir el tiempo dedicado por el personal de enfermeria en una Unidad de Cuidados Intensivos (UCI) en la aplicacion de los pasos del Proceso de Enfermeria (PE). Se trata de una investigacion descriptiva, observacional y exploratoria, utilizando el metodo cuantitativo. Fueron cronometradas las etapas del PE llevadas a cabo por seis enfermeros en 29 pacientes internados en la UCI. El estudio fue aprobado por la Comision de Etica en Investigacion de la Institucion. Se midio el tiempo en cada fase: evaluacion diaria del paciente: 8,34 (3,78); diagnostico y la prescripcion de enfermeria: 3.65 (2.27-5.45); programacion de la prescripcion en enfermeria: 2.30 (1, 14); y la evolucion de asistencia: 11,29 (2,55). El enfermero paso 25.58 minutos por paciente en la realizacion del PE. Los datos fueron importantes para dimensionar los recursos humanos necesarios para el cuidado de pacientes criticos, con el fin de calificar la asistencia.Resumen This study had as a goal the measure of time spent on an Intensive Care Unit (ICU) by nurses when carrying out the steps of the Nursing Process (NP). It is a descriptive research, observational and exploratory, using quantitative methodology. Steps of the NP as carried out were timed by six nurses on 29 ICU patients. Research was approved by the Institutional Ethics Review Board. Time was measured in the following steps: daily patient assessment: 8.34 (3.78); diagnosis and nursing prescription: 3.65 (2.27-5.45); scheduling nursing prescription: 2.30 (1.14) and evaluation of care: 11.29 (2.55). Nurses spent 25.58 minutes per patient when carrying out the NP. The data provide a way to measure human resources required for caring for critical patients, so as to qualify care giving.

Aplicabilidade da classificação dos resultados de enfermagem em pacientes com déficit no autocuidado: banho/higiene

O objetivo da pesquisa e verificar a aplicabilidade da Classificacao dos Resultados de Enfermagem (NOC) em pacientes ortopedicos hospitalizados com o diagnostico de enfermagem Deficit no Autocuidado: banho/higiene. Estudo quantitativo, descritivo, desenvolvido em hospital universitario com pacientes submetidos a artroplastia total de quadril em 2008. O instrumento de coleta de dados continha 24 indicadores dos resultados da NOC Autocuidado: Atividades de Vida Diaria, Autocuidado: Banho, Autocuidado: Higiene, Autocuidado: Higiene Oral e uma escala Likert onde 1=dependente; 2=nao participa; 3=necessita de pessoa auxiliar; 4=independente com mecanismo auxiliar; 5=completamente independente. Foi observado o banho de 25 pacientes no primeiro e terceiro dia de pos-operatorio. Os aspectos eticos foram respeitados. Em 21 indicadores houve variacao estatisticamente significativa entre as medias de pontuacao do primeiro para o segundo banho. As escalas da Classificacao NOC apresentaram consistencia interna, mostrando-se confiaveis para mensurar a evolucao dos pacientes durante a implementacao dos cuidados de enfermagem.The purpose of this research is to explore the Nursing Outcomes Classification (NOC) applicability on hospitalized orthopedic patients diagnosed with Self-care Deficit: bathing/hygiene. This is a quantitative, descriptive study developed in a university hospital with patients submitted to total hip replacement in 2008. The data collection instrument was composed of 24 NOC Self-care outcome indicator: Self-care: Activities of Daily Living; Self-care: Bathing; Self-care: Hygiene; Self-care: Oral Hygiene; and a Likert scale in which 1 = dependent 2 = does not participate; 3 = requires an assistant; 4 = independent but requires an assistant mechanism; 5 = completely independent. 25 patients bathing on first and third post-operative day were observed. Ethical aspects were respected. In 21 cases there was significant statistical variation between average scores from first to second bath. NOC classification scales presented internal consistency, proving their reliability to measure the patient evolution during nursing care implementation.

Clinical Indicators for Evaluation of Outcomes of Impaired Tissue Integrity in Orthopedic Patients: Consensus Study

PURPOSE To select outcomes from the Nursing Outcomes Classification (NOC) to evaluate impaired tissue integrity in patients undergoing orthopedic surgeries, and develop conceptual and operational definitions for their indicators. METHODS A consensus study involving 10 nurses from clinical practice. A 100% consensus was established in the outcomes selection. FINDINGS Wound Healing: Primary intention (1102) with two indicators: (110214) Scar formation, and (110201) Skin approximation were selected. Four indicators were grouped to evaluate inflammatory signs together with foul wound odor and six for drainage. CONCLUSIONS These indicators direct the outcomes evaluation for wound healing. IMPLICATIONS FOR NURSING PRACTICE Support to the definition the priority interventions in nursing process documentation.
