All about compression: A literature review

Lower extremity ulcers represent a significant public health problem as they frequently progress to chronicity, significantly impact daily activities and comfort, and represent a huge financial burden to the patient and the health system. The aim of this review was to discuss the best approach for venous leg ulcers (VLUs). Online searches were conducted in Ovid MEDLINE, Ovid EMBASE, EBSCO CINAHL, and reference lists and official guidelines. Keywords considered for this review were VLU, leg ulcer, varicose ulcer, compressive therapy, compression, and stocking. A complete assessment of the patients overall health should be performed by a trained practitioner, focusing on history of diabetes mellitus, hypertension, dietetic habits, medications, and practice of physical exercises, followed by a thorough assessment of both legs. Compressive therapy is the gold standard treatment for VLUs, and the ankle-brachial index should be measured in all patients before compression application.

The importance from relationship with the patient surgical for implementation of nursing psychosocial diagnoses: report study

Objectives: to apply the nursing process using the language of classification and to show the effectiveness of care systematization. Methodology: report of clinical case conducted with a female patient of 26 years old, admitted in a General Hospital at the city of Niteroi, Rio de Janeiro, during the period from March to April 2008. We used the technique of semi-structured interview based on Carpenito and the documentary survey of medical records analysis. The analysis proceeded second clinical reasoning of Risner. Results: nursing diagnosis were identified according to North American Nursing Diagnosis Association-NANDA, the main are: denial ineffective; hypothermia, acute pain, impared skin integrity, risk for infection, risk for fluid volume deficit, risk for situational low self-steem. Then was built a plan of care with the interventions proposed by Nursing Intervention Classification-NIC and the results were evaluated by Nursing Outcomes Classification-NOC. And to allow evaluation of nursing actions the results of the NOC were described in two phases, before and after intervention. Conclusion: we concluded that the nursing care based on NANDA, NIC and NOC is effective and transparent to the team of professionals. Descriptors: nursing process; nursing assessment; perioperative care. resumo Objetivos: aplicar o processo de enfermagem utilizando das linguagens de classificacao e evidenciar a efetividade da sistematizacao do cuidado. Metodologia: estudo de caso realizado com paciente de 26 anos, sexo feminino, internada num Hospital Geral do Municipio de Niteroi-Rio de Janeiro, no periodo de Marco a Abril de 2008. Para tanto utilizamos da tecnica de entrevista semi-estruturada baseada em Carpenito e do levantamento documental do prontuario. A analise procedeu segundo raciocinio clinico de Risner. Resultados: foram identificados os diagnosticos de enfermagem segundo North American Nursing Diagnosis Association-NANDA, sendo os principais: Negacao ineficaz; Hipotermia; Dor aguda; Integridade da pele prejudicada; Risco para infeccao; Risco para volume de liquidos deficiente; Risco para baixa autoestima situacional. Em seguida construimos o plano de cuidados contendo as intervencoes propostas pela Nursing Intervention Classification-NIC e a avaliacao dos resultados utilizando a Nursing Outcomes Classification-NOC. E para permitir a avaliacao das acoes de enfermagem os resultados da NOC foram descritos em dois momentos, pre e posintervencao. Conclusao: concluimos que a assistencia de enfermagem baseada em Nanda, Nic e Noc demonstram-se eficaz e transparente para a equipe de profissionais.
