Assistência à mäe de recém-nascido internado na UTI neonatal: experiências, sentimentos e expectativas manifestadas por mäes

The porpuse of the study was to examine the experiences, feelings and expectation of mothers of high risk newborns. The population was a group of 20 mothers of high risk newborns of three hospitals in the City of Sao Paulo. Interview with the mothers was the method of data colletion containing opened and structured questions . It was verified that most of the mothers had none or only a little interaction with the newborn after delivery; the eye contact was the most referred during the staying of the newborn in the Intensive Care Unity; all of them demonstrated interest in participating in the care of the newborn and expressed the need of information concerning to the health status of the newborn, the Intensive Care Unity environment and the hospital team. Several were the feelings expressed and the motives that indicated the needs of the mothers.

Violência doméstica na gravidez

Objective: To characterize domestic violence in pregnancy. Method: Cross-sectional, exploratory and analytical study of domestic violence with 385 women who attended a public maternity. The Chi-square test of Pearson and Fisher exact test were used to verify associations and considering significant results p<0.05. Data of the sociodemographic characteristics of women, partners and family members and items of “Abuse Assessment Screen-AAS” were collected. Results: Domestic violence compromised 36.9% of women at some point in life and 34.6% during pregnancy. Prevalence rates were due to psychological (97.1%), physical (48.7%) and sexual (4.9%) violence and the partner was the main aggressor. The following variables were signifcantly associated with domestic violence: protestant religion (p=0.0022), lack of planning of pregnancy (p=0.0196), low family income (p=0.0215) and partner drinking habit (p=0,0002). Conclusion: Domestic violence should be systematically investigated during pregnancy, with special attention to protestant pregnant women, women who did not plan their pregnancy and women whose partners are alcoholics.

Assessment of the newborn respiratory function in the immediate neonatal period

The aim of the study was to identify the predictive factors and the signals of transient tachypnea of the newborn valued by the nurses who work at neonatal ward and rooming-in when they assess the newborn respiratory function during the immediate neonatal period in the first six hours after birth. A cross sectional study carried out at two public hospitals of São Paulo city. The sample consisted of 28 nurses who replied a structured questionnaire about the newborn respiratory function assessment practices. Data showed they valuated to know the evaluation of labor, delivery and Apgar score when they evaluate the newborn respiratory function. About the signals of respiratory distress they valued to evaluate the respiratory rate and auscultation, evaluation of the respiratory distress using the Silverman-Andersen Index besides, the neonatal nurses valued to evaluate de oxygen saturation.O objetivo do estudo foi Identificar os fatores preditivos e os sinais de taquipneia transitoria neonatal valorizados por enfermeiras de unidade neonatal e alojamento conjunto ao avaliar a funcao respiratoria do recem-nascido no periodo neonatal imediato. Estudo transversal realizado em duas instituicoes hospitalares publicas da cidade de Sao Paulo. Participaram do estudo 28 enfermeiras que responderam um questionario estruturado relacionado as praticas de avaliar a funcao respiratoria do neonato. As enfermeiras consideram relevantes conhecer a evolucao do trabalho de parto, parto e o Apgar ao avaliar a funcao respiratoria do neonato. Com relacao aos sinais clinicos valorizam o monitoramento da frequencia respiratoria, ausculta pulmonar, avaliacao do nivel de desconforto respiratorio utilizando o Indice de Silverman-Andersen, alem desses, as enfermeiras de unidade neonatal valorizaram o monitoramento da saturacao de oxigenio.

In most maternity units, the physician classifies, evaluates, and determines which unit will receive the newborn (NB) after birth. Evaluation occurs in the delivery room, taking into consideration the following factors: birth weight, gestational age, and behaviors that define the clinical picture and disease. This observational study evaluates the reliability of an NB classification instrument. The study was conducted at the nursery of a public hospital in São Paulo. Nine nurses applied the instrument to 63 NB, with two of the nurses working simultaneously in each of the nurserys five sectors. The Kappa level of agreement among the nurses was found to be excellent for most care areas (69.0%). It was concluded that there was a consensus and agreement among the nurses that the instrument was complete, easy to understand and applicable, but was very time consuming.
