Use of Punica granatum as an antifungal agent against candidosis associated with denture stomatitis

The aim of this study was to evaluate the use of a gel containing the extract of Punica granatum as an antifungal agent against candidosis associated with denture stomatitis. Sixty patients with denture stomatitis confirmed by clinical and mycologic examination were selected. The patients were randomly allocated in two groups of 30 individuals each according to the medication prescribed: group A used miconazole (Daktarin® gel oral) and group B used a gel of P. granatum Linné (pomegranate). Both groups used the medicines three times per day for 15 days. Forty‐eight hours after finishing the treatment the patients were re‐examined and a second set of samples was collected for mycologic examination. The medicines were evaluated for their clinical response and negativity for Candida. The clinical results showed a satisfactory and regular response in 27 and 21 subjects of groups A and B, respectively. Negativity of yeasts was observed in 25 subjects of group A and 23 of group B. It can be concluded that the extract of P. granatum may be used as a topical antifungal agent for the treatment of candidosis associated with denture stomatitis.

Efeito antibacteriano e antiaderente in vitro do extrato da Punica granatum Linn. sobre microrganismos do biofilme dental

Nesta pesquisa foi avaliada a atividade antimicrobiana e a capacidade de inibicao da sintese de glucano in vitro do extrato da casca do fruto da roma (Punica granatum Linn.) sobre linhagens bacterianas de Streptococcus mitis, Streptococcus mutans, Streptococcus sanguis, Streptococcus sobrinus e Lactobacillus casei. Os ensaios foram realizados pelas tecnicas de agar-difusao em placas para determinacao da Concentracao Inibitoria Minima (CIM) e tecnica dos tubos inclinados para determinacao da Concentracao Inibitoria Minima de Aderencia (CIMA) ao vidro, na presenca de 5% de sacarose. Os mesmos procedimentos foram realizados com a clorexidina a 0,12%. As CIMs (mg/mL) do extrato da P. granatum frente ao S. mitis, S. mutans, S. sanguis, S. sobrinus e L. casei foram 1:32, 1:8, 1:8, 1:512, e 1:128, respectivamente. Para as CIMAs o extrato da P. granatum mostrou resultados melhores que a clorexidina, exceto para o S. mutans com achados semelhantes na diluicao de 1:256. Os resultados mostram a potencialidade da P. granatum na inibicao do crescimento bacteriano e sintese de glucano representada pela aderencia ao vidro, sugerindo o emprego do extrato da roma, como meio alternativo, no controle desses microrganismos na formacao do biofilme.

Goldenhar's syndrome: case report

Goldenhars syndrome is a rare condition described initially in the early 1950s. It is characterized by a combination of anomalies: dermal epibulbar cysts, auricular appendices and malformation of the ears. In 1963, Gorlin suggested the name oculo-auriculo-vertebral (OAV) dysplasia for this condition and also included vertebral anomalies as signs of the syndrome. The etiology of this rare disease is not fully understood, as it has shown itself variable genetically and of unclear causes. This work reports a case of Goldenhars syndrome in an 11-year-old female, who presented all classical signs of this rare condition

Evaluation of mandibular implant sites: correlation between panoramic and linear tomography

The reliability of the linear tomography and panoramic radiography made with X-ray equipment was evaluated (Vera View Scope X-600; Morita). The sample was composed of 20 dry human hemimandibles, in which the area selected for analysis was 1.5 cm distal from the limit set before the mental foramen. Four measurements were made. The images obtained were drawn on acetate paper and the hemimandibles cut at the demarcated area. The measurements were made using a digital electronic pachymeter. The values found for the radiographic images were compared to those obtained in the mandibular specimens and submitted to statistical evaluation by the Wilcoxon test. It was concluded that both techniques were reliable for the accomplishment of vertical linear measurements in the selected area. However, a 2.0 mm safety margin is recommended.

Leucoplasias bucais: relação clínico-histopatológica

A relacao entre o aspecto clinico e as caracteristicas histologicas das leucoplasias bucais foi avaliada em 28 pacientes adultos, que apresentaram lesoes leucoplasicas na mucosa bucal. As lesoes foram divididas quanto ao aspecto clinico em homogeneas e nao-homogeneas, e classificadas segundo suas caracteristicas histopatologicas em seis grupos: hiperceratose com ausencia de displasia epitelial; displasia epitelial leve; displasia epitelial moderada; displasia epitelial severa, carcinoma in situ e carcinoma invasivo. Os resultados clinicos mostraram maior ocorrencia de leucoplasias homogeneas (78,6%) do que nao-homogeneas (21,4%). Os achados histopatologicos demonstraram que 32,2% dos casos apresentaram hiperceratose com ausencia de displasia epitelial, 53,5% evidenciaram displasia epitelial (39,3% leve, 7,1% moderada e 7,1% severa) e 14,3% diagnosticados como carcinoma invasivo. As leucoplasias homogeneas apresentaram alteracoes celulares discretas, enquanto as nao-homogeneas evidenciaram displasia epitelial severa e carcinoma invasivo. Os resultados sugerem um maior cuidado no diagnostico e controle de leucoplasias dada a possibilidade de transformacao maligna.

Radiographic diagnosis of incipient proximal caries: an ex-vivo study

The aim of this ex vivo study was to compare visual clinical and radiographic examinations to the histological analysis for proximal caries diagnosis in extracted permanent molars and premolars. The relationship between clinical aspects and carious lesions was also evaluated. Eighty-eight proximal surfaces (44 freshly extracted teeth) were longitudinally sectioned with a 370-microm diamond disk, thinned with wet silicon carbide paper and observed with a stereomicroscope at x40 magnification. Sensitivity and specificity were 65.6% and 83.3% for clinical examination and 29.7% and 95.8% for radiographic examination, respectively. Kappa values ranged from 0.64 to 0.91. The white spots corresponded to lesions restricted to enamel, while the dark spots corresponded to lesions that reached the dentinoenamel junction. In most cases, cavitation corresponded to dentin lesions. It may be concluded that interproximal radiographic examination is not a reliable method for detection of incipient proximal carious lesions.

Accuracy of linear measurements before and after digitizing periapical and panoramic radiography images

The aim of this study was to evaluate the accuracy of linear measurements made on conventional and digitized periapical and panoramic radiographic images of dry human hemi-mandibles. Images from the posterior region of 22 dry human hemi-mandibles were obtained by conventional panoramic and periapical radiography technique. Using a digital caliper, 3 vertical measurements were marked directly on the dry hemi-mandibles (reference measurements) as well as on the tracing from the conventional radiographic images of the specimens made onto acetate paper sheet: Distance 1: between the upper limit of the alveolar ridge and the lower limit at the mandible base; Distance 2: between the upper limit of the alveolar ridge and the upper limit of the mandibular canal; Distance 3: between the lower limit of the mandibular canal and the lower limit of the mandible base. Next, the radiographs were digitized and the three measurements were made on the digital images using UTHSCSA Image Tool software. Data were analyzed statistically by one-way ANOVA (α=0.05). There was no statistically significant differences (p>0.05) between periapical and panoramic radiographs or between the measurements recorded using the digital caliper and UTHSCSA software compared with dry mandible specimens for Distances 1 (p=0.783), 2 (p=0.986) and 3 (p=0.129). In conclusion, the radiographic techniques evaluated in this study are reliable for vertical bone measurements on selected areas and the UTHSCA Image Tool software is an appropriate measurement method.

Streptococcus mutans in denture stomatitis patients under antifungal therapy

PURPOSE: To assess the number of Streptococcus mutans in saliva of patients with denture stomatitis before and after antifungal therapy. METHODS: After examining 93 patients, 47 were selected for fungal test. Then, from this sample, thirty patients were selected: 15 with positive and 15 with negative diagnosis for candidiasis that were evaluated for S. mutans counting, salivary flow and buffer capacity evaluation. Oral hygiene and prosthesis hygiene, period using prosthesis, lesion type and salivary data were related with clinical laboratorial characteristics of the patients with Candida. RESULTS: The most frequent lesions were type I (43.5%) and II (53.5%). The amount of S. mutans was six times higher in patients with candidiasis and it was associated with low salivary flow and poor oral hygiene. After therapy, a reduction of S. mutans was verified particularly in patients with normal salivary flow. The values ranged from 0.01 to 3.88 x 104 cfu/mL. CONCLUSION: The data suggest that Streptococcus spp collaborates with Candida spp in the etiology and pathogenesis of denture stomatitis. The use of oral antimicrobial agents may provide a beneficial effect for denture stomatitis patients that are under antifungal therapy and that have poor oral hygiene and unfavorable salivary parameters.
