A 12-month follow-up of an influenza vaccination campaign based on voluntary adherence: report on upper-respiratory symptoms among volunteers and non-volunteers

CONTEXT Routine immunization of groups at high risk for influenza has been progressively implemented as a matter of Brazilian public health policy. Although the benefits of the vaccination for healthy young adults are still controversial, it has been offered yearly to hundreds of thousands of Brazilian workers, generally as part of wellness initiatives in the workplace. OBJECTIVE To study the characteristics of subjects that accepted or refused to be vaccinated against influenza and to report on respiratory symptoms in both groups, one year after the campaign date. DESIGN A prospective observational study. SETTING Workers at a subsidiary of an international bank in São Paulo, Brazil. PARTICIPANTS 124 persons that did not accept and 145 that voluntarily accepted the vaccine completed 12 months of follow-up. MAIN MEASUREMENTS Data concerning gender, age, tobacco use, and any history of chronic respiratory illness such as asthma, bronchitis, rhinitis, and repetitive upper-respiratory infections, were recorded at the time of vaccination. After that, workers were asked monthly by questionnaire or telephone about respiratory symptoms, days of work lost and medical consultations. RESULTS The results showed statistically significant differences regarding age (P = 0.004) with the vaccinated group (V) being younger than the non-vaccinated (NV) one, and with reference to previous repetitive upper-respiratory infections being higher among the V group (P < 0.0001). During the follow-up, the V group reported more occurrences of upper respiratory symptoms (P < 0.0001), due to both non-influenza (P < 0.0001) and influenza-like illness (P = 0.045). Differences were also found between V and NV groups concerning days off work and number of medical consultations due to upper-respiratory symptoms and non-influenza illness. Gender and history of repetitive upper-respiratory infections were the best predictors of influenza-like illness-related events. CONCLUSIONS The making of previous reference to repetitive upper-respiratory infections was a major difference between those who accepted or rejected the vaccine. The vaccination itself was not sufficient to reduce the number of occurrences of respiratory symptoms and related absenteeism to levels similar to those found among non-vaccinated people.

Relações disciplinares em um centro de ensino e pesquisa em práticas de promoção da saúde e prevenção de doenças

O trabalho em equipe interdisciplinar e considerado um importante pressuposto para reorganizacao do processo de trabalho com praticas de promocao da saude e prevencao de doencas, visando a uma abordagem mais integral e resolutiva. Neste artigo avaliam-se as relacoes disciplinares entre profissionais de saude a partir do estudo de caso de um centro universitario voltado ao ensino e pesquisa em praticas de promocao da saude e prevencao de doencas. Com base na visao humanista das relacoes disciplinares, procurou-se identificar as principais premissas que determinam o trabalho interdisciplinar. Utilizaram-se os metodos da observacao participante e entrevistas semi-estruturadas com profissionais de saude, seguidas da analise de conteudo na modalidade tematica, para a coleta e interpretacao dos dados. A analise revelou que o servico de saude avaliado apresentava uma situacao possivelmente transitoria da pluri para a interdisciplinaridade. Partindo-se do estudo de caso, discutem-se os fatores sociais, culturais, educacionais, institucionais e subjetivos que podem agir tanto como facilitadores quanto como obstaculos a interdisciplinaridade.

Tele-educação e monitoria ativa no ensino da saúde bucal a estudantes de medicina

OBJECTIVES: To verify the baseline level of knowledge among medical students in the fifth year of the degree course regarding the practice of oral health and to measure the increase in knowledge and counseling skills after training. METHOD: 148 students, divided into 4 groups - 2007-A (control group), 2007-B, 2008-A and 2008-B - received progressive training interventions, face to face (2007) and online (2008). The 2008-B group also had active contact with a specialist in oral health. Increases in knowledge were measured by written assessments applied before and after the respective intervention, and counseling skills were assessed through the Objective Structured Clinical Examination (OSCE) . RESULTS: 91% of the students found their baseline knowledge of oral health promotion insufficient. The growth in knowledge of groups 2008-A and 2008-B was significant when compared to the control group. The 2008-B group also demonstrated an increased ability to provide advice on oral health. CONCLUSION: the information obtained in this study may be useful for developing strategies for interdisciplinary teaching in complementary areas of medical training.

Alcohol and tobacco use and the diseases treated in general practice

Objective: To characterize the use of alcohol and tobacco and correlate both to the diseases of outpatients in a general practice outpatient clinic. Method: The ASSIST (Alcohol, Smoking and Substance Involvement Screening Test) questionnaire was answered by 300 randomly chosen subjects assigned to different groups according to the diseases being treated at the Outpatient General and Teaching Clinic of the Department of Internal Medicine, Hospital das Clínicas of the University of São Paulos School of Medicine (HC-FMUSP, in the Portuguese acronym), São Paulo, Brazil. The consumption of tobacco and alcohol was characterized and its correlation with the groups of diseases being treated was calculated using Chi-square and Pearson test statistics. Results: Compared to alcohol, tobacco use was more prevalent, more intense and showed more health-, social-, legal- and financial-related damage. Tobacco smoking presented a positive significant (p<0.0001) correlation with respiratory diseases. According to the questionnaires criteria, few alcohol users would be referred to clinical interventions in comparison to smokers. Conclusion: Respiratory diseases and tobacco use were well correlated based on the ASSIST questionnaire. The preventive value of the questionnaire was more evident in relation to tobacco than alcohol consumption.

0292 Comparison of personal habits and baseline knowledge of health promotion among students of the Faculdade de Medicina da Universidade de São Paulo

Objectives Because of the importance given to the implementation of health promotion programs nowadays generating positive outcomes important to society and especially to businesses, employees and the financial market this study was designed to assess students’ baseline knowledge of expertise in occupational medicine on health promotion and correlate it with the students’ baseline knowledge Faculdade de Medicina da Universidade de São Paulo (internal fifth-year residents and first-year internal medicine) as well as evaluate their own health habits. Method We conducted a cross-sectional study in which we applied two questionnaires that had been used in previous studies by Ferreira Junior, 2011. The questionnaires so called 1) “Survey of attitudes and behaviour” and 2) “Questionnaire assessment of knowledge in health promotion.” Regarding the epidemiological data to submit descriptive statistics and Excel filters the table. Results The higher scores among graduate students, were related to nutrition and oral hygiene, both at present as expected in a year. Questions about professional attitude in their 13 items’ scores were higher post-graduate training in the item. In other items, students’ grades and graduation from residency and postgraduate students were equal. When comparing the mean of graduate students called in questionnaire 2, all items showed lower values in relation to the notes of interns and residents. Conclusions The loss in quality of care is undeniable, since there is awareness on the part of the respondents own lack of technical knowledge updated with regard to health promotion that contrasts with the self-perception of adequate training to do so.

Objectives: To introduce a basic webliography to access highly qualified evidence-based material on health promotion and disease prevention, aiming at the continuing education of health professionals. Methods: By means of Google® browser, applying the descriptors in sequence to progressively refine the search on Internet and key concepts to be learned, all previously defined by the authors themselves, we proceeded a qualitative analyses of the 20 first listed links for each searched issue and the final selection of the most scientifically relevant ones. Results: The 34 selected links are presented in 4 groups: 23 portals, 5 guides and recommendations, 4 scientific journals and 3 blogs that allow free access to health promotion and disease prevention related subjects, such as: concepts; national and international public policies; epidemiology, statistics and health indicators; diseases screening and prophylaxis; counseling for behavior change of health related habits; and interdisciplinary work. Among the selected links 10 (29%) are written in English while the others are in Portuguese. Conclusions: The identification of reading materials on health promotion and disease prevention available on Internet, many in Portuguese, allowed us to select relevant scientifically qualified literature and turn it accessible to health professionals, enabling the acquisition of new knowledge or quick update.
