Human mandibular trabecular bone density correlation with mechanical strength: implications for implant dentistry.

Purpose:The prognosis of a dental implant depends on the mechanical properties of the bone. The preoperative diagnosis of bone quality has become more important, because the immediate loading procedure is now widely used, and a firm initial stability is required. Quantitative computed tomography (QCT) is an effective method of determining selectively the bone mineral density (BMD) of the trabecular bone mostly responsible for the survival of the implant. In this project, we investigated the relationship between the BMD measured by QCT and the mechanical strength of the mandible. Materials and Methods:The BMD (mg/mL) of 58 trabecular bone specimens from 6 embalmed human cadaver mandibles were obtained using QCT. The compressive strength to break point was performed with a mechanical device. Each specimen was washed and weighed. Results:The mean values of BMD for the anterior, premolar, and molar region were 633.3, 571.0, and 518.3 mg/mL, respectively. The mean values of compressive strength were 0.237, 0.216, and 0.196 kN, and the ash weights were 0.047, 0.044, and 0.039 g, respectively. There was a positive correlation between the BMD and the mechanical strength (R2 = 0.77) and between the ash weight and the mechanical strength (R2 = 0.84). Conclusion:There was strong correlation of bone strength, bone mineral, and bone density by QCT. QCT is an excellent preoperative diagnostic tool to select the most mechanically appropriate implant for initial stability and improve the survival prognosis.

Proteolytic activity of mycoplasmas and ureaplasmas isolated freshly from human saliva

Mycoplasma andUreaplasma strains freshly isolated from human saliva were examined for proteolytic activity. All the strains tested (101 strains ofMycoplasma salivarium, 97 strains ofMycoplasma orale, and 3 strains each ofMycoplasma hominis andUreaplasma urealyticum) decomposed horse serum proteins, bovine albumin, and casein. Ten of theM. salivarium and 8 of theM. orale strains were tested for aminopeptidase activity as well, using L-leucine-p-nitroanilide as a substrate, and were shown to possess the activity. Thus proteolytic activity was suggested to be one of the biological characteristics of these two species. The level of both caseinolytic and aminopeptidase activity was significantly higher inM. salivarium thanM. orale strains.

Clinical Observation of Submandibular and Sublingual Gland Tumors at Beijing Stomatological Hospital.

われわれは1987年から1996年までの間に, 北京口腔医院 (首都医科大学附属) において44例の顎下腺および舌下腺腫瘍を経験した。44例中39例は顎下腺腫瘍, 5例は舌下腺腫瘍であった。44例の腫瘍のうち, 良性腫瘍は29例 (顎下腺が27例, 舌下腺が2例) で, 悪性腫瘍は15例 (顎下腺が12例, 舌下腺は3例) であった。組織型は顎下腺の良性腫瘍27例中26例が多形性腺腫, 1例は血管腫で, 舌下腺の2例は乳頭状嚢腺腫であった。顎下腺悪性腫瘍の組織型は粘表皮癌が3例, 腺癌および腺房細胞癌が各2例, 腺様嚢胞癌, 筋上皮癌, 多形性腺腫内癌, 唾液腺導管癌, および扁平上皮癌が各1例であった。舌下腺の悪性腫瘍はそれぞれ腺房細胞癌, 腺様嚢胞癌および筋上皮癌であった。良性顎下腺腫瘍は前医で顎下腺を切除されていた2次症例の1例を除き, 全例顎下腺を含めて腫瘍を切除した。舌下腺腫瘍の1例は舌下腺および顎下腺を含め腫瘍切除, もう1例は術前に顎下腺腫瘍と診断されたため顎下腺と腫瘍が切除された。悪性腫瘍では, 症例により頸部郭清および下顎骨切除を併用し, 比較的広汎な腫瘍切除を行ったが, 顎下腺では原発巣の再発がやや高頻度にみられた。

根治手術後に即時再建手術を行った口底癌19症例の術後の構音機能と手術との関連性を分析した。評価方法は日本語100語音による発語明瞭度検査である。対象症例の内訳を軟組織の再建方法別にみると, 遊離前腕皮弁移植症例が12例, 大胸筋皮弁移植症例と腹直筋皮弁移植症例がそれぞれ3例, 前腕皮弁と大胸筋皮弁の併用症例が1例であった。舌の切除範囲別では, 舌部分切除症例が14例, 可動部半側切除症例が1例, 半側切除症例が1例, 亜全摘出症例が3例であった。これらの症例について原発腫瘍の大きさ, 切除範囲, 切除様式, 軟組織の再建方法, 下顎骨の再建方法と術後の構音機能の関連について検討した。その結果, 原発腫瘍の大きさによる術後の構音機能の差は少なかったが, 舌の切除範囲が大きくなると構音機能は低下していた。切除様式については側方型の切除を受けた症例の明瞭度は前方型の切除を受けた症例よりも明らかに高く, 切除様式による発音機能の差異が明らかであった。前方型の切除を行った症例の術後の発音障害を防ぐためには, 手術法の改善が必要であり補綴物の応用が有用であることが示唆された。

Clinical evaluation of Granisetron against nausea and vomiting induced by Carboplatin in oral cancer patients. Multi-center study.

カルボプラチン (CBDCA) により誘発される悪心嘔吐に対するグラニセトロンの予防効果を検討する目的で, 多施設による共同研究を実施した。6施設からの30名の口腔癌患者をこの研究の対象とし, 各患者に対しCBDCA (200~350mg/m2) 投与直前にグラニセトロン3mgを静脈内に投与した。悪心嘔吐の発現頻度と, その程度を, 5日間にわたり観察した。悪心は30名中8名に出現したが, 5名の患者ではCBDCA投与当日に軽度の悪心が出現したのみであった。残り3名では2~3日間悪心が継続し, うち1名は3日目に中等度の悪心を訴えた。嘔吐が認められたのは1名のみで, CBDCA投与後第3日目に3回, 第4日目に1回嘔吐した。この患者は第3日目に中等度の悪心を訴えたものと同一の患者であった。本研究の結果, 73.3%の患者で悪心の発現はなく, 96.7%の患者で嘔吐は認められず, 最終効果判定ではグラニセトロンは96.7%の患者において著効, 残りの3.3%は有効と判定された。
