Antifibrinolytic activity in middle ear effusion

No activity of the plasmin and plasminogen activators could be detected in middle ear effusion (MEE). A streptokinase proactivator, which was not plasminogen and markedly enhanced the activation of plasminogen by streptokinase, could be detected. MEE had an apparent inhibitory activity against both thiol protease (papain) and serine proteases (plasmin and trypsin). All these findings indicate that MEE contains unsaturated protease inhibitors and is in a state of predominance of inhibitors over fibrinolytic activity.

Quantitation of Nasal Secretory IgA by Enzyme-Linked Immunosorbent Assay

A simple and sensitive technique, enzyme-linked immunosorbent assay (ELISA) was used for the quantitation of secretory IgA (sIgA) in order to study the synthesis and transport of sIgA in the nasal secretion from patients with chronic sinusitis. Rough-surface glass beads coated with antisecretory component antibody were employed as the solid phase. The minimal detectable concentration of sIgA was 50 ng/ml in this study. sIgA averaged 226.3 +/- 217.4 mg/dl and total IgA averaged 302.0 +/- 137.5 mg/dl.

Analysis by electrophoretic transfer blotting of Japanese cedar pollen allergens which react with IgG and IgE antibodies in the serum of patients.

Crude extract of Japanese cedar pollen was fractionated by DEAE-cellulose chromatography to separate the major allergenic components. In order to analyze which antigens react with IgG or IgE antibodies in the serum of Japanese cedar pollinosis patients, an electrophoretic transfer blotting was performed. Both IgE and IgG antibodies reacted with several common antigens, while IgG additionally reacted with some low-molecular-weight proteins.

A Case of Subglottic Hemangioma: A New Technique for Surgical Removal

Congenital malformation of the larynx are life-threatening anomaly for newborns and infants. Although tumorous lesions in these malformations (especially subglottic hemangioma) are extremely rare, when they do occur some definitive attempts should be made to treat them in infancy. This report concerns a case of subglottic hemangioma with marked stridor and sternal retractions which had begun in the third month of age in a six month-old male. In the excision of the hemangioma a new technique is employed. That is: the excision through the laryngofissure was made under surgical microscope. Assuming that careful removal of the subepithelial hemangioma and closure of the overylaying epithelia would help to maintain the mucosa and thereby avoid excessive scarring, the hemangiomatous tissue was completely removed while preserving the overlaying epithelial layer.

Purification of glandular kallikrein in maxillary mucosa from humans suffering from chronic inflammation.

Glandular kallikrein was purified from human maxillary mucosa with chronic inflammation and its biochemical properties were characterized. The purification procedure consisted of extraction with 3 mol/l KCl, saturation of ammonium sulfate (66%), DEAE-Sepharose, arginine-Sepharose and Sephadex G-150 chromatographies. Maxillary mucosa with chronic inflammation contains considerable activity of glandular kallikrein, which is a serine protease with limited proteolytic activity and its biochemical properties resemble those of pancreatic kallikrein.

Partial purification and characterization of the tissue plasminogen activator in nasal mucosa and nasal polyp

Tissue plasminogen activator was partially purified from the inferior turbinate and nasal polyp, and its biochemical properties were investigated. Similar TPA peak positions were seen in the gel filtration chromatography of both tissues, and the molecular weight was approximately 65,000, which was comparable to TPA of pig heart (55,000-60,000). Activity of TPA from inferior turbinate was higher than that from nasal polyp. TPA from both tissues was completely inhibited by trans-aminomethyl cyclohexane carboxylic acid, dithiothreitol, and diisopropylfluorophosphate and had similar inhibition profiles to TPA from pig heart. All these findings indicate that TPA from both tissues is undoubtedly a plasminogen-activating enzyme and serine-type protease and would be biochemically identical.

A Clinical Study of Oral Cancer

One hundred and fortynine patients with carcinoma of the oral cavity were treated at the Department of Otolaryngology, Mie University Hospital from 1961 to 1980. There were 80 cases of tongue cancer, 19 of cancer of the buccal mucosa, 9 of cancer of the gingiva, 7 of cancer of the lips. The average age of these patients was 57.4 years old; ninety-four (63%) were male and 55 (37%) were female. According to the T classification proposed by the UICC, 43 (28.9%) were classified as T1, 66 (44.3%) as T2, 31 (20.8%) as T3, and 6 (4.0%) as T4. Although there were no cases of distant metastasis, cervical lymphnodal metastasis was found in 45 (30%). Of all the patients, the 1-, 3- and 5-year survival rates were 81.6%, 64.5% and 48.9%, respectively.In the analyses of the cases of tongue cancer, the following results were obtained: as the principal method of treatment of the tongue cancer, surgery and/or irradiation were employed. Especially, in the last 10 years, irradiation was preferred as the initial treatment of this cancer. The 5-year survival rate of tongue cancer was 66.7% in T1, 61.9% in T2 and 10.0% in T3. In the present study, we recommended radiation therapy for early cases of tongue cancer and combined therapy of irradiation and surgery for advanced cases.

Effects of Immunotherapy on Nasal Allergy with or without Extrinsic Asthma

昭和53年1月より12月までの1年間に三重大学耳鼻咽喉科アレルギー外来にて検査を受けた 435名を対象に問診より喘息合併群, 喘息既往群, 鼻アレルギー単独群の3群に分類し一元拡散法又はRIST法による血清IgE値, RAST SCORE, 血中好酸球, 鼻汁中好酸球, 自覚症状の5項目について鼻アレルギーの重症度や減感作治療効果を検討した. その結果鼻アレルギーに気管支喘息を合併しておればRIST, RASTが共に高値を示す症例や血清IgE値, 血中好酸球が共に高値を示す症例がそれぞれ鼻アレルギー単独群に比べ多く認められたが, 血中好酸球, 鼻汁好酸球が共に高値を示す症例は両群間で差がなかった. 鼻アレルギーに対して1年以上の減感作治療を行い有意に治療効果を認めたものは, 鼻アレルギー単独群の鼻汁好酸球と遮断抗体陽性率のみで, これらは喘息合併群においても同様の傾向が認められた. しかし血中好酸球, 血清IgE値, RAST SCORE, 自覚症状等においては両群間で1年以上の減感作治療を行っても治療効果は認められなかった.

Two Gases of Warthin's Tumor

Warthins tumor (papillary cystadenoma lymphomatosum) is a rare benign tumor of the salivary glands. This tumor consists of histologic components, one epithelial, the other lymphoid.This report concerns two cases of Warthins tumor. The first case was an 18 year old female who first noted a small lump near the left angle of the mandible. The lump was removed and was histologically demonstrated to be Warthins tumor. The second case concerned a 67 year old male who first noted a lump near the left angle of the mandible and a swelling in the nasopharyngeal region. The tumor was diagnosed as an epipharynx malignancy with left neck metastasis and radiation therapy of 1200 rads was given, when this tumor proved to be benign, the neck tumor and epipharynx tumor were removed and were histologically demonstrated to be Warthins tumor for the former and the oncocytoid tumor for the latter.Reports of papillary cystadenoma lymphomatosum in Japan were reviewed and histogenesis of this tumor was discussed.
