Protective effect of thymoquinone against testicular torsion induced oxidative injury

We aimed to determine the protective effects of thymoquinone (TQ), against ischaemia–reperfusion (I/R) injury in the testis tissue of rats. Twenty‐seven male Wistar albino rats were randomly divided into three equal groups as follows: Group I, sham group; Group II, torsion group; and Group III, torsion + thymoquinone group. The ischaemia period was 2 h, and orchiectomy was performed after 30 min of detorsion. Testis tissue sections were analysed with the terminal transferase mediated dUTP‐nick end labelling (TUNEL) assay to determine in situ apoptotic DNA fragmentation. Additionally, Caspase 3 and Bax proteins were analysed immunohistochemically. The superoxide dismutase (SOD), glutathione peroxidase (GSH‐Px) and malondialdehyde (MDA) activity levels in the testis tissue were also measured. The superoxide dismutase activity and malondialdehyde levels in the torsion group were significantly higher than those of the sham group (P < 0.05). Thymoquinone administration significantly reduced these levels. Torsion significantly increased active‐Caspase 3 and Bax expression, which was decreased by thymoquinone. The apoptotic index of the torsion group was significantly higher than that of the control group. However, thymoquinone significantly reduced the apoptotic index (P < 0.05). Our results indicate that thymoquinone plays a protective role in oxidative stress induced ischaemia–reperfusion in the testis tissue of rats.

Peripapillary retinal nerve fiber layer and ganglion cell–inner plexiform layer thickness in adult-onset familial Mediterranean fever

ObjectiveThe purpose of the present study was to evaluate the thickness of the peripapillary retinal nerve fiber layer (RNFL) and retinal ganglion cell–inner plexiform layer (GCIPL) in adult-onset familial Mediterranean fever (FMF).MethodsForty two adult-onset FMF patients and forty two healthy controls were included in the present study. Detailed ocular examination was performed, and then the thickness of the peripapillary RNFL and GCIPL was measured by Spectral domain optical coherence tomography. The patients were divided into two groups according to their disease severity score, M694V gene mutation, colchicine dosage used per day, colchicine usage time period and number of FMF attacks per year.ResultsThere were no statistically significant differences in peripapillary RNFL and retinal GCIPL thickness in patients with adult-onset FMF and controls.ConclusionAccording to our study, it looks like that neither adult-onset FMF nor colchicine has any effect on the RNFL and GCIPL thicknesses. Further studies with a large sample size are needed.

OBJECTIVES The aim of this study was to compare the efficiency of diclofenac sodium to paracetamol using a visual analog scale in the patients presenting to the emergency room with primary dysmenorrhea. METHODS Group I (n=40) patients were diagnosed with primary dysmenorrhea and treated with paracetamol (1 gr intravenous) and Group II (n=40) patients were diagnosed with primary dysmenorrhea and treated with diclofenac sodium (75 mg intramuscular). In both groups, patients were 19-30 years old. In all groups, the intensity of the pain was ranked from 0 (no pain) to 10 (intolerable) using VAS. The VAS scores were compared between treatment groups. RESULTS Between two groups, there was no statistically significant difference in age, mean arterial pressure and pulse values. The initial VAS values of the first group were higher than that of 2nd group. Following treatment, in the 10th and 30th minutes, the VAS values were lower in Group I than Group II (p=0.00). The VAS values of each group were significantly different from each other on the 10th and 30th minutes. VAS values at the 10th and 30th minutes were lower compared to the initial values and the values in the 30th minute were lower relative to the 10th minute (p=0.00) in both treatment groups. CONCLUSION We can suggest that paracetamol is more efficient than diclofenac sodium in the treatment of primary dysmenorrhea.

Epidemiological investigation of traumatic upper extremity fractures in children who applied to emergency department

A B S T R A C T Introduction: Serious and potentially fatal multi-trauma in childhood can rarely be seen. However, each one of 5 children is exposed to minor trauma . Our aim is to reveal upper extremity traumas, trauma mechanisms and seasonal relationship in children under 18 years of age. Patients and methods: In this retrospective study, the files and graphs of 500 patients under 18 years old who applied to Tokat Gaziosmanpasa University Medical Faculty Emergency Service after trauma were examined through Enlil and PACS system. 328 patients with upper extremity pathology were included in the study. These patients were analyzed according to different age groups (0-4,5-8,9-12,13-17). Results: 500 patients were screened and 328 patients (65.6%) with upper extremity trauma were included. 109 (33.23%) patients were female and 209 (66.77%) patients were male. The most frequent 0-4 year upper extremity traumas (n = 39) in girls were found to decrease with age. In boys, it was found that the upper extremity traumas increased with age, the most common age was 13-17 (n = 79). The distal radius fracture (n = 64) and distal humerus fracture (n = 61) were the most common in upper extremity traumas. The highest falling down in the etiology of 328 patients with upper extremity pathology (n = 149, 45.43%) was detected. It was determined that traumatic upper extremity pathologies were followed most in summer (36.58%) followed by autumn, spring and winter respectively. Conclusion: Two - thirds of children under 18 years old who applied for trauma were admitted due to upper extremity trauma. It was found that fracture of the distal radius (n = 87) was the most (33.85%) when the isolated radius fractures, bilateral forearm fractures, and upper extremity multiple fractures were evaluated together in children.

Evaluation of Vertebral Fractures in Patients Presenting to Emergency Department with Trauma

Although vertebral fractures are observed in only a minority of patients presenting with trauma, they are serious injuries in terms of mortality and morbidity. Apart from the osteoporotic fractures seen in the elderly people, vertebral fractures generally occur as a result of high energy traumas3. These fractures may have dire consequences, therefore it is best to take preventive measures before they happen. There is a marked increase in the number of vertebral fractures and spinal cord injuries that are in parallel with the increase in motor vehicle accidents and falls from a height. Although treatment of vertebral fractures is one of the most discussed topics in orthopedic surgery, time to diagnosis has been shortened with advanced imaging techniques, and it has been possible to perform early surgical interventions. Considering patient density and limited time in the emergency departments, it is very important to evaluate patients for vertebral fractures promptly ABSTRACT
