Maxillary sinus volumes of patients with unilateral cleft lip and palate

BACKGROUND AND OBJECTIVE Studies about maxillary sinuses of cleft lip-palate patients have increased since sinusitis is commonly observed in these patients. It is evident that maxillary sinus will be morphologically affected in these patients. And anatomic differences may be a cause or at least a contributor of sinusitis. The aim of this study was to compare maxillary sinus volumes of the non-syndromic patients with unilateral cleft lip-palate and control group by using Cone-Beam computed tomography. METHODS Tomography scans of 44 unilateral cleft lip-palate patients (18 right and 26 left) with age and gender matched 45 control patients were evaluated for the study. The images used in the study were part of the diagnostic records collected due to dental treatment needs. All tomographs were obtained in supine position by using Cone-Beam computed tomography (NewTom 5G, QR, Verona, Italy). The patient-specific Hounsfield values were set to include the largest amount of voxels in the sinuses volume calculation individually. All data were measured in mm(3). RESULTS There was no statistically difference between the gender and age distributions of the groups. No statistically significant difference was found on the cleft and non-cleft side, the right and left side of the unilateral cleft lip-palate patients and the control group (P>0.05). For the inter group comparison, mean maxillary sinus volumes volume of unilateral cleft lip-palate patients (9894.55±4171.44mm(3)) was statistically smaller than the control group (11,977.90±4484.93mm(3)) (P<0.05). CONCLUSION Maxillary sinus volumes were effected negatively in unilateral cleft lip-palate patients when compared with the healthy control group. No difference was found on the cleft, non-cleft side and the right-left side of the unilateral cleft lip-palate patients.

Evaluation of tooth number anomalies in a subpopulation of the North‑East of Turkey

Objective: The aim was to evaluate the prevalence and distribution of tooth number anomalies in a Turkish subpopulation. Materials and Methods: A population of 2722 patients (1532 females and 1190 males; mean age, 12.33 ± 2.5 years) was retrospectively examined to determine the prevalence and distribution of the hypodontia, oligodontia, and hyperdontia using panoramic radiographs. All permanent teeth were investigated except thirds molars and the data obtained were recorded as unilateral (left or right) or bilateral according to gender. Pearson′s Chi-squared and Fisher exact tests were used for difference comparisons (P < 0.05). Results: Permanent tooth anomalies were found in at least 132 (4.84%) of 2722 patients with no statistically difference between the genders, consists of 4.63% females and 5.12% males. Distribution of hyperdontia was statistically significant difference between genders, whereas distribution of hypodontia and oligodontia was no significant difference between genders. Hypodontia was the most frequently observed anomaly (3.67%), followed by hyperdontia (0.96%), and oligodontia (0.21%). Oligodontia and hypodontia were more frequent in females (3.98% and 0.26%, respectively), whereas hyperdontia was more frequently observed in males (1.68%). Maxillary lateral incisors were most common missing teeth (2.27%), while the frequency of hyperdontia was most common in premolars. Conclusion: The prevalence of teeth number anomalies was 4.84% of dental patients. Maxillary lateral incisors were most common missing teeth, while the frequency of hyperdontia was most common in premolars.

Mandibular changes during initial alignment with SmartClip self-ligating and conventional brackets: A single-center prospective randomized controlled clinical trial

Objective To test the null hypothesis that SmartClip self-ligating brackets are more effective than conventional brackets for initial mandibular alignment and identify influential factors. Methods Fifty patients were randomly allocated to two equal treatment groups by using an online randomization program: self-ligating group (SmartClip brackets) and conventional group (Gemini brackets). The archwire sequence was standardized. Changes in anterior irregularity index, intercanine width, and intermolar width were assessed on plaster models at 8th and 16th weeks. Changes in incisor position and inclination were assessed on lateral cephalometric radiographs at 16 weeks. Intragroup and intergroup comparisons were performed with paired t-test and Students t-test, respectively. Multiple linear regression was performed to identify variables affecting improvement in anterior ambiguity. Results Data of 46 patients were analyzed; those missing an appointment (n = 2) or showing bracket breakage (n = 2) were excluded. Incisor inclination (p < 0.05), intercanine width (p < 0.05), and intermolar width (p > 0.05) increased at 8 and 16 weeks in both the groups; no significant intergroup differences were noted (p > 0.05). Initial anterior irregularity index and intercanine width change were significantly associated with improvement in anterior irregularity (p < 0.001). Conclusions The null hypothesis was rejected. Bracket type has little effect on improvement in anterior ambiguity during initial mandibular alignment.

Three-dimensional evaluation of the mandibular third molars’ development in unilateral crossbite patients: A cone beam computed tomography study

Objectives: The aim was to investigate mandibular third molar (3M)′s maturation in the crossbite and normal sides by two- and three-dimensional analyses using cone beam computed tomography (CBCT). Materials and Methods: A retrospective study was performed using CBCT of 25 patients (16 females and 9 males; mean age: 16.8 ± 2.9 years) with unilateral posterior crossbite. The formation stages and the volume of the mandibular 3Ms were evaluated by means of CBCT data of the patients without knowing the crossbite side of the patients. Results: Statistically no significant differences were found in the development of the 3Ms between the crossbite and the control sides, whereas the volume of 3M was found to be less in the crossbite side than in the normal side (P = 0.021). Conclusions: A volume of 3M was found to be less in the crossbite side than in the normal side.

Evaluation of temporomandibular fossa and mandibular condyle in adolescent patients affected by bilateral cleft lip and palate using cone beam computed tomography

The purposes of this study were to investigate the position of the mandibular condyle and temporomandibular fossa between the adolescent patients affected by bilateral cleft lip and palate (BCLP) and well-matched controls without any cleft by using cone-beam computed tomography (CBCT). The study sample consisted of 17 patients (7 females and 10 males; mean age, 14.27 ± 2.83 years) affected by BCLP and 17 patients (6 females and 11 males; mean age, 14.27 ± 2.12 years) as age-and sex-matched control group without any cleft. Using cone-beam computed tomography segmented three-dimensional temporomandibular fossa and mandibular condyle images were reconstructed and angular, linear, and volumetric measurements of the patients in both sides of the groups were examined using Paired and Students t-tests. Comparison of the sides showed that both sides were found to be similar in BCLP and control groups, except the condylar angulation of the right side was found to be higher compared to that of the left side in both groups (p < 0.05). Comparison of the groups showed that the patients affected by BCLP and non-cleft patients had similar values for all parameters, except for the condylar angulation in the right side (BCLP group had less angulation compared to controls; p < 0.05). The condylar volume was found to be slightly less in the BCLP group in both sides compared to the controls (p > 0.05). The positions of the mandibular condyle and temporomandibular fossa were found to be similar in patients affected by BCLP and control group of without any cleft. SCANNING 38:720-726, 2016.

Evaluation of dehiscence and fenestration in adolescents affected by bilateral cleft lip and palate using cone-beam computed tomography

Introduction We evaluated the dehiscence and fenestration presence in maxillary and mandibular anterior teeth of patients affected by bilateral cleft lip and palate (BCLP) and compared the findings with a well‐matched control group of noncleft patients using cone‐beam computed tomography. Methods Cone‐beam computed tomography images of 51 patients were divided into 2 groups (group 1, 21 patients affected by BCLP; mean age; 14.62 ± 2.89 years; and group 2, 30 patients as the noncleft control group; mean age, 14.22 ± 1.05 years) and assessed them for dehiscence and fenestration in the anterior maxillary and mandibular teeth. Data were analyzed with the Student t test, Pearson chi‐square test, and Fischer exact test. Results The prevalences of dehiscence in patients affected by BCLP were 61.11% in the maxillary and 48.41% in the mandibular anterior teeth, whereas the rates in the noncleft group were 7.78% and 16.67%, respectively (P < 0.001). The presence of fenestration was found to be statistically significantly higher in the maxillary central incisors of the BCLP group compared with the noncleft controls (P < 0.05), and almost similar rates were noted for the other teeth, with no statistically significant differences (P > 0.05). Conclusions Our data suggest that patients affected by BCLP may have higher prevalences of dehiscence in the maxillary and mandibular anterior teeth and of fenestration in the maxillary central incisors. Investigation of the peg-shaped maxillary lateral incisors in a Turkish orthodontic subpopulation

Aim: The following study aims to investigate the prevalence and distribution of the peg-shaped maxillary lateral incisors in a Turkish orthodontic subpopulation. Materials and Methods: A study sample consisted of 2925 patients of which 1746 were females and 1179 were males with aged range from 10 to 18 years; (mean age, 13.40 ± 1.9 years) were retrospectively examined using panoramic radiographs, dental cast and pre-treatment records for the presence of the peg-shaped maxillary lateral incisors. Pearson′s Chi-square and Fisher exact tests were used for statistical comparisons. Results: Peg-shaped maxillary lateral incisors were found in 2.15% of the patients (63/2925; 41 females and 22 males) with no statistically significant difference between the genders. However, females had more peg-shaped maxillary left lateral incisors compared with the males (P = 0.007). It was almost equally distributed as bilateral (1.09%, 32/2925) or unilateral (1.06%, 31/2925). Conclusion: It was found to be 2.15% (63 out of 2925 patients) with no gender difference except for the peg-shaped maxillary left lateral incisors. Clinicians should be aware of this anomaly prior to treatment for possible complications.
