Maxillary sinus volumes of patients with unilateral cleft lip and palate

BACKGROUND AND OBJECTIVE Studies about maxillary sinuses of cleft lip-palate patients have increased since sinusitis is commonly observed in these patients. It is evident that maxillary sinus will be morphologically affected in these patients. And anatomic differences may be a cause or at least a contributor of sinusitis. The aim of this study was to compare maxillary sinus volumes of the non-syndromic patients with unilateral cleft lip-palate and control group by using Cone-Beam computed tomography. METHODS Tomography scans of 44 unilateral cleft lip-palate patients (18 right and 26 left) with age and gender matched 45 control patients were evaluated for the study. The images used in the study were part of the diagnostic records collected due to dental treatment needs. All tomographs were obtained in supine position by using Cone-Beam computed tomography (NewTom 5G, QR, Verona, Italy). The patient-specific Hounsfield values were set to include the largest amount of voxels in the sinuses volume calculation individually. All data were measured in mm(3). RESULTS There was no statistically difference between the gender and age distributions of the groups. No statistically significant difference was found on the cleft and non-cleft side, the right and left side of the unilateral cleft lip-palate patients and the control group (P>0.05). For the inter group comparison, mean maxillary sinus volumes volume of unilateral cleft lip-palate patients (9894.55±4171.44mm(3)) was statistically smaller than the control group (11,977.90±4484.93mm(3)) (P<0.05). CONCLUSION Maxillary sinus volumes were effected negatively in unilateral cleft lip-palate patients when compared with the healthy control group. No difference was found on the cleft, non-cleft side and the right-left side of the unilateral cleft lip-palate patients.

A survey of ponticulus posticus: Radiological analysis of atlas in an orthodontic population based on cone-beam computed tomography.

Background: The ponticulus posticus (PP; Latin for little posterior bridge) has become an important anomaly of the atlas, as the use of the C1 lateral mass screw has become common in treating atlantoaxial instability. Aim: The purpose of this study was to retrospectively determine the prevalence and morphologic characteristics of PP in an orthodontic patient population using cervical three-dimensional (3-D) cone beam computed tomography (CBCT) images. Material and Methods: A retrospective study was conducted by selecting cervical 3-D CBCT images of 374 adolescent population and examining them for the presence and type of PP. Results: 161 patients with 298 complete or partial or bilateral or unilateral PPs were identified based on the 374 cervical 3-D CBCT scans. The prevalence of posticulus ponticus was founded 43.04% in adolescent population. Conclusion: Our results show that the presence of this anomaly should be carefully examined using preoperative lateral radiographs before lateral mass screw placement. If a PP is suspected or confirmed on radiographs, 3-D CBCT scanning should be considered before lateral mass screw placement into the posterior arch because of the variation in the size and shape of PPs and the possibility of injury.

Evaluation of Mineral Content and Photon Interaction Parameters of Dental Enamel After Phosphoric Acid and Er:YAG Laser Treatment

OBJECTIVE The purpose of this study was to evaluate the effects of laser and acid etching on the mineral content and photon interaction parameters of dental enamel in human teeth. BACKGROUND DATA The composition of dental enamel may vary, especially at the surface, depending on the reactions that occur during dental treatment. MATERIALS AND METHODS Forty maxillary premolars were divided randomly into 2 groups of 20 teeth. In the first group, half of teeth crowns were etched by using 37% phosphoric acid; in the second group, half of teeth crowns were etched by using an erbium:yttrium-aluminum-garnet (Er:YAG) laser. The remaining half crowns in each group were used as untreated controls. We characterized the calcium (Ca), phosphorus (P), magnesium (Mg), sodium (Na), and potassium (K) contents in each specimen by using wavelength dispersive X-ray fluorescence spectrometry. The total atomic cross-section ([Formula: see text]), effective atomic number ([Formula: see text]), and electron density (Ne) of the tooth samples were determined at photon energies of 22.1, 25, 59.5, and 88 keV by using a narrow beam transmission method. Data were analyzed statistically by using the Mann-Whitney U test. RESULTS The mineral contents after Er:YAG laser and phosphoric acid etching did not differ significantly (p > 0.05), and no significant variation in [Formula: see text], [Formula: see text], or Ne was observed. CONCLUSIONS Therefore, we conclude that the Er:YAG laser and phosphoric acid systems used in this study did not affect mineral composition or photon interaction parameters of dental enamel.

Evaluation of temporomandibular fossa and mandibular condyle in adolescent patients affected by bilateral cleft lip and palate using cone beam computed tomography

The purposes of this study were to investigate the position of the mandibular condyle and temporomandibular fossa between the adolescent patients affected by bilateral cleft lip and palate (BCLP) and well-matched controls without any cleft by using cone-beam computed tomography (CBCT). The study sample consisted of 17 patients (7 females and 10 males; mean age, 14.27 ± 2.83 years) affected by BCLP and 17 patients (6 females and 11 males; mean age, 14.27 ± 2.12 years) as age-and sex-matched control group without any cleft. Using cone-beam computed tomography segmented three-dimensional temporomandibular fossa and mandibular condyle images were reconstructed and angular, linear, and volumetric measurements of the patients in both sides of the groups were examined using Paired and Students t-tests. Comparison of the sides showed that both sides were found to be similar in BCLP and control groups, except the condylar angulation of the right side was found to be higher compared to that of the left side in both groups (p < 0.05). Comparison of the groups showed that the patients affected by BCLP and non-cleft patients had similar values for all parameters, except for the condylar angulation in the right side (BCLP group had less angulation compared to controls; p < 0.05). The condylar volume was found to be slightly less in the BCLP group in both sides compared to the controls (p > 0.05). The positions of the mandibular condyle and temporomandibular fossa were found to be similar in patients affected by BCLP and control group of without any cleft. SCANNING 38:720-726, 2016.

Dental anomalies in an orthodontic patient population with maxillary lateral incisor agenesis.

ABSTRACT Introduction: The purpose of this study was to evaluate the prevalence of dental anomalies in a subpopulation of orthodontic patients with agenesis of maxillary lateral incisors (MLI). Methods: The material of the present study included the records of the 1964 orthodontic patients. Panoramic radiographs and dental casts were used to analyze other associated eight dental anomalies, including agenesis of other teeth, dens invaginatus, dens evaginatus, peg shaped MLI, taurodontism, pulp stone, root dilaceration and maxillary canine impaction. Results: Out of the 1964 patients examined, 90 were found to have agenesis of MLI, representing a prevalence of 4.6%. The most commonly found associated-anomalies were agenesis of other teeth (23.3%), peg-shaped MLIs (15.6%), taurodontism (42.2%), and dilacerated teeth (18.9%). Conclusion: Permanent tooth agenesis, taurodontism, peg-shaped maxillary lateral incisor, and root dilacerations are frequently associated with maxillary lateral incisor agenesis.

The relationship between frontal sinus morphology and skeletal maturation

BACKGROUND The aim of this study is to evaluate the relationship between frontal sinus morphology and hand-wrist bone maturation by using postero-anterior (PA) cephalometric radiographs. MATERIALS AND METHODS The study sample consisted of 220 patients divided into 11 groups based on the hand-wrist radiographs. The right and left maximum height, width and area of the frontal sinus parameters were measured in PA cephalometric radiographs of 220 subjects aged 8-18 years. The hand-wrist skeletal maturation stages were evaluated on the hand-wrist radiographs using the method of Fishman. The Kendall tau-b values were analysed to evaluate the correlation between the hand-wrist skeletal maturation stages and the frontal sinus parameters. RESULTS The right and left frontal sinus areas and widths were found to be larger in males than in females (p < 0.05). In males, a significant difference was observed in all frontal sinus parameters in different maturation stages (p < 0.001), while a statistically significant correlation was found in females between the left frontal sinus area, right frontal sinus height, right frontal sinus width and different maturation stages (p < 0.05). CONCLUSIONS The relationship between frontal sinus dimensions obtained from PA cephalometric radiographs and hand-wrist maturation stages suggests that frontal sinuses can be used in determining growth and development.

Evaluation of the skeletal and dental effects in orthodontic patients with maxillary lateral incisor agenesis

OBJECTIVE The purpose of this study was to evaluate the transverse dimension of the dentoalveolar and skeletal widths in both unilateral (UMLIA) and bilateral (BMLIA) maxillary lateral incisor agenesis groups. MATERIALS AND METHODS The study sample consisted of 59 patients (34 females and 25 males) divided into three groups as UMLIA group (16 patients; 14.94 ± 1.37 years), BMLIA group (21 patients; 14.46 ± 1.53 years) and control group (22 patients; 15.28 ± 1.70 years) without tooth agenesis. Transversal measurements (maxillary and mandibular intercanine, interpremolar, intermolar, intercanine alveolar, interpremolar alveolar, intermolar alveolar widths) were done on dental casts. Craniofacial skeletal measurements were done with posteroanterior cephalometric radiographs. Statistical testing was performed with the one-way variance analysis, Kruskal-Wallis and post hoc tests. RESULTS UMLIA and BMLIA groups had statistically significantly narrower skeletal apical base of the maxilla, maxillary intercanine and intercanine alveolar widths compared with the control group (P < .05). Moreover, UMLIA and BMLIA groups had statistically significantly similar dental, alveolar and skeletal features (P > .05). CONCLUSIONS The UMLIA and BMLIA groups showed statistically significantly smaller values for maxillary intercanine, maxillary intercanine alveolar and skeletal maxillary widths compared with the control group. CLINICAL SIGNIFICANCE This study provides information the transverse dimension of the dental, alveolar and skeletal widths with maxillary lateral incisor agenesis patients. Knowledge of transverse dimension of the dental, alveolar and skeletal widths with maxillary lateral incisor agenesis patients is important for diagnosis and treatment planning.

Evaluation of dehiscence and fenestration in adolescents affected by bilateral cleft lip and palate using cone-beam computed tomography

Introduction We evaluated the dehiscence and fenestration presence in maxillary and mandibular anterior teeth of patients affected by bilateral cleft lip and palate (BCLP) and compared the findings with a well‐matched control group of noncleft patients using cone‐beam computed tomography. Methods Cone‐beam computed tomography images of 51 patients were divided into 2 groups (group 1, 21 patients affected by BCLP; mean age; 14.62 ± 2.89 years; and group 2, 30 patients as the noncleft control group; mean age, 14.22 ± 1.05 years) and assessed them for dehiscence and fenestration in the anterior maxillary and mandibular teeth. Data were analyzed with the Student t test, Pearson chi‐square test, and Fischer exact test. Results The prevalences of dehiscence in patients affected by BCLP were 61.11% in the maxillary and 48.41% in the mandibular anterior teeth, whereas the rates in the noncleft group were 7.78% and 16.67%, respectively (P < 0.001). The presence of fenestration was found to be statistically significantly higher in the maxillary central incisors of the BCLP group compared with the noncleft controls (P < 0.05), and almost similar rates were noted for the other teeth, with no statistically significant differences (P > 0.05). Conclusions Our data suggest that patients affected by BCLP may have higher prevalences of dehiscence in the maxillary and mandibular anterior teeth and of fenestration in the maxillary central incisors. Effect of 3 cements on white spot lesion formation after full-coverage rapid maxillary expander: A comparative in-vivo study.

INTRODUCTION The aim of this study was to assess the effects of 3 luting agents (glass ionomer cement, compomer, and polycarboxylate cement) on white spot lesion formation in patients with full-coverage bonded acrylic splint expanders. White spot lesion formation was assessed with quantitative light-induced fluorescence. METHODS Full-coverage rapid maxillary expanders were cemented with glass ionomer cement, compomer, and polycarboxylate cement in groups 1, 2, and 3, respectively. A control group comprised patients who never had orthodontic treatment. Quantitative light-induced fluorescence images taken before and after rapid maxillary expansion treatment were analyzed for these parameters: the percentages of fluorescence loss with respect to the fluorescence of sound tooth tissue (ΔF) and maximum loss of fluorescence intensity in the whole lesion; lesion area with ΔF equal to less than a -5% threshold; and the percentage of fluorescence loss with respect to the fluorescence of sound tissue times the area that indicated lesion volume. RESULTS All 3 groups showed statistically significantly greater demineralization than the control group. The 3 experimental groups differed from each other in half of the parameters calculated. Teeth in the polycarboxylate group developed the most white spot lesions. CONCLUSIONS With the highest rate of white spot lesion formation, polycarboxylate cements should not be used for full-coverage bonded acrylic splint expanders. Compomers may be preferred over glass ionomer cements, based on the findings of this study.

OZET Amac: Unilateral mandibular ikinci premolar eksikligi olan hastalarda kondiler ve ramal vertikal asimetriyi arastirmaktir. Bireyler ve Yontem: Unilateral mandibular ikinci premolar eksikligi olan 30 kisiyi iceren calisma grubu (25 kadin, 35 erkek) ve 57 kisilik kontrol grubun (41 kadin, 16 erkek) hem iskeletsel hem de dental Sinif I iliskiye sahipti. Kondiler, ramal ve kondiler ramal yukseklikler toplami Paired t testi kullanilarak her iki cene segmenti icin hesaplandi. Ayrica; calisma ve kontrol gruplari icin asimetri indekslerinin hesaplanmasi icin Student t testi kullanildi. Bulgular: Asimetri indeks olcumleri cinsiyetler arasinda istatistiksel olarak farkli degildi ve kiz ve erkeklere ait veriler istatistiksel degerler birlestirilerek degerlendirildi. Calisma grubunda kondiler, ramal ve kondiler-ramal yukseklikleri toplami dis eksikligi olan tarafta normal taraftan daha dusuk bulundu. Ancak, bu farklilik istatistiksel olarak anlamli degildi.
