Development of secondary dental care: a study on specialized dental clinics in Pernambuco State, Brazil

This exploratory study aimed to evaluate the Specialized Dental Care Centers in Pernambuco State, Brazil, based on secondary outpatient treatment data from 2006 and standards and criteria adopted to implement these services. The study describes the overall achievement of targets by these specialized dental clinics and variables for characterization of the dental services and municipalities (counties). The methodology included descriptive analysis and non-parametric correlation analysis (Spearman coefficient), with significance set at 5%. Of the 22 specialized dental clinics, 40.9% showed good performance, and 81.8% met the target for basic dental care, as compared to 13.6% that met the target for minor oral surgery. Among the independent variables, only population size and Municipal Human Development Index (MHDI) were correlated with the performance of the specialized dental clinics: the smaller the city (r = 0.678; p < 0.001) and the lower the MHDI (r = 0.599; p < 0.001), the worse the performance. The findings suggest that standardization of the implementation and functioning of specialized dental clinics should be monitored and evaluated in order to ensure better quality of services for the population.

Comparação do acesso aos serviços de saúde bucal em áreas cobertas e não cobertas pela Estratégia Saúde da Família em Campina Grande, Paraíba, Brasil

Although there are various studies on access to health studies, few have dealt specifically with access to oral health services. The aim of the current study was to evaluate factors associated with access to oral health services in Campina Grande, Paraiba State, Brazil, comparing the areas covered versus not covered by the Family Health Program (FHP). This was a cross-sectional, population-based study with a sample consisting of individuals over 18 years of age (n = 827) from urban census tracts that included health units of the Unified National Health System (SUS), stratified in areas covered versus not covered by the FHP. The statistical analysis used Pearsons chi-square and logistic regression. Individuals living in areas not covered by the FHP had 1.5 times greater odds of having access to oral health services (OR = 1.5; 95%CI: 1.1-1.9; p = 0.004) when compared to those living in areas covered by the FHP. However, this probability decreased, losing its significance (OR = 1.1; 95%CI: 0.8-1.6; p = 0.337), after adjusting for gender, age, individual income, schooling, and self-perceived health. Thus, no association was proven between coverage versus non-coverage by the FHP and access to oral health services.

Impacts of dental pain on daily activities of adolescents aged 14-15 years and their families

Objective. To assess the prevalence and severity of dental pain on the daily lives of adolescents and their families in Brazil, and, in terms of socio-economic status, whether there are differences in the prevalence of impacts. Material and Methods. In a cross-sectional survey of a random sample of 14 to 15-year-olds in Brazil, the prevalence and severity of dental pain, and how dental pain affects daily functioning, were assessed using a child-related questionnaire. A family-related questionnaire included questions on a child pain inventory and the pain-related impacts on the daily lives of the childs family members. Results. 14.5% of children reported high impact levels on their daily lives related to dental pain, the more so among girls than among boys, i.e. 16.7% compared to 11.6%. Significantly more impacts occurred in lower socio-economic class children than in higher, i.e. 19.7% versus 7.8%. The prevalence of impacts among children who reported dental pain (n=354) was high; 64.7% reporting that concentration at school was affected, 55.6% that home activities were affected, and 51.4% that leisure activities were affected. There was a significant relationship between impacts of dental pain on children and families. Overall prevalence of impacts on childrens families was 15.2%. The most frequent family dimensions affected were: in carrying out normal household activities (19.7%) and in visiting friends and relatives (17.7%). Emotional stability was the most frequent psychological activity dimension affected (14.1%); 9.6% of parents were woken up as a result of their childs dental pain on an average of 3.7 nights per week. Conclusions. Dental pain has considerable impact on the daily lives of adolescents and their families in Brazil. There are many more dental pain-related impacts in girls and in lower social class children.

Fatores associados à automedicação em dor de dente: análise a partir dos profissionais dos estabelecimentos farmacêuticos da cidade do Recife, PE

This paper aims to describe the factors associated with self-medication related to toothache in the city of Recife, Pernambuco, Brazil. The investigation was designed as a cross-sectional study. The sample was random selected among the pharmacies registered in the Regional Pharmacy Council of Pernambuco and distributed over the Health Districts. The level of knowledge of the pharmacy personnel about self-medication related to toothache was analyzed. One hundred and seventy nine professionals from 120 establishments were interviewed. The data were collected by means of a questionnaire. According to the results, 67.0% of the interviewees had attended individuals relating orofacial pain in the last six months, among them 91.6% relating toothache; 83.7% male and 73.3% female personnel had indicated medications for toothache without prescription; personnel with high-school level indicated more medications without prescription (48.6%) The study concludes that to buy pain relievers without prescription including for toothache is common and reinforces the need of informing the population about the correct use of these medicaments.

Raça e o uso dos serviços de saúde bucal por idosos

We analyze if race can be considered a limiting factor in the use of dental services by the elderly. This study is of an analytical nature, with the use of secondary data collected by the National Survey of Oral Health in 2003. Those examined who declared themselves as being white, brown or black in the 65 to 74-year-old age bracket were included. The sample was composed of 5,108 elderly people: 2,575 whites and 2,533 blacks. Of the whites, 3.8% stated that they had never been to the dentist, while this figure was 7.8% for the blacks. Even after the adjustment for interception for prosthetics and dental pain, the chance of elderly blacks not having used dental services at least once in their life is 0.62 OR less than for elderly whites. Of those who used the services, 21.2% of the elderly whites visited the dentist in the last year, while for elderly blacks the figure was 14.2%, in the adjusted model for interception for prosthetics and dental pain the OR was 0.60. All the relations were statistically significant (p<0,001). Race is a limiting factor in the use of dental services by the elderly and even after the adjustments the elderly blacks continue to manifest greater resistance to the use of oral health services.

Validação inicial do índice de necessidade de atenção à saúde bucal para as equipes de saúde bucal na estratégia de saúde da família

This survey set out to validate the Index of Oral Healthcare Needs (IOHN), based on a pre-defined algorithm of the social status of families. The validation process was divided into two phases, namely a face validation and a construct validation. In the latter, data on caries experience, toothache and access to oral health services was collected. To validate the index a random, stratified sample of 412 children aged 3-5 and 7-12 was obtained, based on the IOHCN algorithm, all the children being from the areas of Recife covered by the family healthcare program. The analysis consisted of a descriptive and an analytical phase, adopting a 5% level of significance. The index was considered by an expert committee to have good face validity. The convergent construct validation was associated with a decay component of dmft (p = 0.03) and DMFT (p = 0.01); the divergent construct validation was associated with access to oral care (p = 0.001) and filled component of dmft (p = 0.05), showing no association with the filled component of DMFT. The Index of Oral Healthcare Needs was shown to have good initial validation and canbe used as a useful tool in the planning of dental care at a local level.

Sociodemographic Factors and Oral Health Conditions Related to the Impact on the Quality of Life of Adolescents

Objective: To identify the sociodemographic factors and the oral health conditions related to the impact on the quality of life of adolescents. Material and Methods: Cross-sectional study with 1,417 students of both sex aged 15-19 years in Sao Lourenco da Mata - PE, Brazil. The collected information comprised sociodemographic data (gender, age, race / color), oral health status (dental caries, periodontal disease and pain) and impact on oral health-related quality of life of adolescents through the OIDP test. Statistical analysis used the Pearson’s chi-square test and multiple logistic regression, with 5% significance level. Results: High impact on oral health-related quality of life was evidenced in 66.1% of adolescents. The prevalence of dental caries, gingival bleeding and pain was respectively 51.29%, 49.60% and 73.6%. The multiple regression analysis found that the high impact on the quality of life was related to the oral health condition and sex of teenagers, showing that girls, who had higher prevalence of dental caries and pain, had greater impact related to oral health. Conclusion: Dental caries and pain of dental origin cause a high impact on the quality of life of adolescents, being higher among girls.

Use of dental services in areas covered by the Family Health Strategy in Olinda, Brazil

OBJECTIVE:The aim of the present study was to analyze factors associated with the use of dental services in areas covered by the Family Health Strategy in the city of Olinda, northeastern Brazil.METHODS:A quantitative, analytical, cross-sectional study was conducted involving individuals over 18 years of age (n=492) registered at Family Health Units. Data analysis was performed with SPSS program (version 11.0) and involved 2 test and logistic regression analysis.RESULTS:The number of individuals who sought dentists at public services (58.6%) was greater than that who sought dentists in the private sector (37.4%). Among those who used public services in the previous 12 months, only 18.9% had access to a dentist from a Family Health Unit. Multiple logistic regression analysis showed that the use of services was associated with toothaches in the previous six months and living in a rented home (p<0.05).CONCLUSION:The results show that access to dental services was highly limited, even in areas covered by an Oral Health Team. Moreover, access was potentiated by indicators of oral health needs and socioeconomic status.
