Prevalence and Association of Developmental Defects of Enamel with, Dental- Caries and Nutritional Status in Pre-School Children, Lucknow

BACKGROUND Developmental Defects of Enamel in the primary dentition may be associated and predictors of dental caries and nutritional status. The aim of the present study was to assess the Prevalence of Developmental Defects of Enamel and its Association with, Dental-Caries and Nutritional Status in Pre-School Children of Lucknow, India. MATERIALS AND METHODS Multistage Sampling was done. A total of 302 pre-school (Rural and Urban) children were examined. Type III examination was conducted with WHO Probe. Developmental Enamel Defects (DED) and Dental Caries were assessed using WHO (1997) Proforma. RESULTS The prevalence of DED of any type was 39.9% with that of demarcated opacities being the highest, followed by hypoplasia. The most frequently affected teeth were maxillary anterior teeth, while the least affected teeth were mandibular incisors. The mean dmft was 3.5. A positive association between DED and caries was observed. Association between Dental Caries & BMI was non-significant whereas Pearson correlation showed a negative correlation between the two. CONCLUSION The prevalence of enamel defects and caries was high, as the enamel defects were strongly associated with caries.

Diagnostic Methods for Detection of Cotinine Level in Tobacco Users: A Review.

The greatest disease-producing product known to man is tobacco. It is a cause of many oral diseases and adverse oral conditions. In India, tobacco is available in smokeless and smoking form. Tobacco contains nicotine which metabolises to form a toxic alkaloid i.e. cotinine. It stimulates autonomic ganglia and central nervous system. Cotinine is the best indicator of tobacco smoke exposure. Various methods are used to measure cotinine level in blood, saliva and urine such as high performance liquid chromatography, colorimetric assay, gas chromatography, NicAlert saliva test, etc. Thus such wide range of methods for cotinine detection in tobacco users requires a detailed discussion regarding their utility. This review will help readers to compare various methods for cotinine detection and enable them to make scientifically informative decision.

Evaluation of knowledge and attitude of school teachers about emergency management of traumatic dental injury

Introduction: Traumatic dental injuries (TDIs) are widespread in the population and are a serious dental public health problem among children. Dental trauma may cause both functional and esthetic problems, with possible impacts on the patient′s quality of life. Aim: To investigate teacher′s knowledge and attitudes of Mathura city about emergency management of TDIs in children. Materials and Methods: A total of 352 teachers from total 23 schools of Mathura city were included in the study. Data were collected through a survey, which included a self-administered questionnaire. The questionnaire consisted of three major parts containing multiple-choice questions. Results: Among the teachers 51.1% were males and 48.9% were females. Majority of the respondents, that is, 33.5% were between 31 and 40 years of age. Most respondents (34%) had more than 10 years of teaching experience. Majority of the teachers (39.2%) had educational qualification other than B.Ed. and M.Ed. degrees. Physical education teachers comprised the largest group of school teachers. Regarding knowledge and attitude, the teachers with 10-20 years of teaching experience, physical education teachers, and the teachers other than B.Ed. and M.Ed. qualifications had given more correct answers to the questions when compared with other groups. Conclusion: For the teachers having a low level of knowledge, there is a need for greater awareness to improve teachers′ knowledge and attitudes related to the emergency management of TDIs in children by organizing educative and motivational programs.

Mutans streptococci estimation in saliva before and after consumption of probiotic curd among school children.

Background: According to the World Health Organization, probiotics are live microorganisms which when administered in adequate amounts and confer a health benefit to the host. Use of probiotics in children to improve oral health may lead to non-pathogenic bacteria replacing cariogenic bacteria. Dairy foods like cheese, curd, and milk are considered useful vehicles for probiotic bacteria. Aim: To compare the levels of salivary mutans streptococci before and after consumption of probiotic curd. Materials and Methods: Thirty school children of Mathura city who were caries free, in the age group of 12-14 years, were selected and divided equally into group I and II which were given 200 ml probiotic curd and normal curd for 7 days, respectively. Assessment of saliva samples was done at baseline, 1 h after consumption, and on the 7 th day. Mitis Salivarius Bacitracin Agar was used for analysis. The number of colonies was counted and subjected to statistical analysis using unpaired Student′s t-test. Results: The study showed mark reduction in salivary mutans streptococci counts in saliva after 1 h and on the 7 th day in the probiotic group. Results were found to be statistically significant (P ≤ 0.05) when the differences in the reduction of mutans streptococci counts with probiotic curd and normal curd at 1 h and on the 7 th day were compared. Conclusion: Pathogenic microorganisms could be displaced by probiotic bacteria. Thus, use of probiotic products could be exploited for the prevention of enamel demineralization.

Assessment of the efficacy of licorice versus 0.2% chlorhexidine oral rinse on plaque-induced gingivitis: A randomized clinical trial

Background: Supragingival plaque control is elementary to the prevention and management of periodontal diseases. Conversely, significant proportions of all individuals fail to practice a high standard of plaque removal. The adjunctive use of chemicals would therefore appear a way of overcoming deficiencies in mechanical tooth cleaning habits. This prospective, randomized positively controlled clinical trial was aimed to evaluate the short-term clinical effects of a licorice oral rinse in the reduction of plaque and gingival inflammation in individuals with gingivitis. Materials and Methods: A total of 104 individuals, 12–15 years of age diagnosed with chronic generalized gingivitis, were selected and randomly divided into two groups: Group 1: chlorhexidine mouthwash and Group 2: licorice mouthwash. Clinical evaluation was undertaken using the gingival index, the plaque index, and bleeding on probing at baseline, 1st, 2nd, and 4th week. Results: Both chlorhexidine and licorice mouthwash showed a significant reduction in plaque and gingival index scores from baseline to 1st, 2nd, and at 4th week. However, the improvement in plaque and gingival index scores were better in chlorhexidine group than herbal mouthwash. Both mouthwashes were found to be equally effective in reducing bleeding on probing. Conclusion: Unlike chlorhexidine mouthwash, licorice mouthwash was not associated with any discoloration of teeth or unpleasant taste and was effective in reducing plaque accumulation and gingival inflammation. However, chlorhexidine still remains a gold standard in reducing plaque, gingivitis, and bleeding on probing.

Oral health status among health personnel of primary health centers in Mathura district

Introduction: Health is dynamic and multifactorial in nature. Oral health is an integral part of general health. Health personnel, especially in primary health centers (PHCs), can play an important role in grooming health in their patients. Aim: The aim of this study was to assess the oral health status of health personnel of PHCs. Materials and Methods: A cross-sectional study was carried out among 520 health personnel working in PHCs of Mathura district in the month of September-October 2014. The WHO Oral Health Assessment Form (2013) was used to collect data from each subject. Comparison of oral health status of various health personnel was also done. Results: In the present study, 45 (8.7%) belonged to the upper socioeconomic class, 295 (56.7%) were from upper-middle socioeconomic class, and 180 (34.6%) were from lower-middle socioeconomic class. The mean decayed missing filled teeth was 1.11 ± 2.63 for doctors, 1.24 ± 3.10 for pharmacists, 1.10 ± 3.55 for lab technicians, 1.78 ± 3.80 for ward boys/ward nurses, 0.25 ± 0.50 for lady health visitors, and 1.53 ± 3.16 for auxiliary nurse midwives. The difference among study subjects according to occupation was statistically significant (P = 0.787). Conclusion: The oral health status of health personnel of Mathura district was moderate. These health workers can serve as a valuable resource for population-based health promotion approaches in achieving health for all.

Sugar Substitutes and Health: A Review

Artificial sweeteners also referred to as sugar substitutes or low-calorie sweeteners are non-nutritive, high-intensity sweeteners. Artificial sweeteners may be in so many foods, drinks, drugs, and hygiene products that some argue that every citizen of Western countries probably uses them. Because they are so sweet, less is needed to sweeten foods and fewer calories are added. The availability of a variety of safe sugar substitutes is of benefit to consumers because it enables food manufacturers to formulate a variety of goodtasting sweet foods and beverages that are safe for the teeth and general health.

Orofacial Pain: A Review

Orofacial pain is the field of dentistry devoted to the diagnosis and management of chronic, complex, facial pain and oromotor disorders. This specialty in dentistry has developed over a number of years out of the need for better understanding of a group of patients who somehow were not clearly suffering from dental pain disorders. Orofacial pain represents a significant burden in terms of morbidity and health service utilization. It includes very common disorders such as toothache and temporomandibular disorders, as well as rare orofacial pain syndromes. Many orofacial pain conditions have overlapping presentations, and diagnostic uncertainty is frequently encountered in clinical practice. Our profession has realized the importance of basic knowledge and differential diagnosis in order to manage these patients properly. This modification in the approach has caused several changes in education as well as in clinical activities. This paper aims to discuss the importance of basic knowledge to perform successful management.

Expressed needs associated with orthodontic treatment in a private dental college, Mathura

Introduction: The primary goal for most of the patients who seek orthodontic treatment is a discernible improvement in some aspect of their dento-facial appearance. Orthodontic therapy makes people look better and feel better about themselves and perhaps influences their ability for social interaction. Hence, this study was conducted to assess expressed needs and barriers associated with orthodontic treatment. Objectives: To know main motivational factor behind seeking orthodontic treatment and to correlate these factors with other variables such as age, gender, and socioeconomic status. Materials and Methods: The study included 200 orthodontic patients between 12 and 25 years visiting a private dental college, Mathura during July 2014 to September 2014. Structured proforma consisting of closed ended questions was used for assessing social demographic characteristics, factors and the barriers, which were associated with orthodontic treatment. Statistical analysis was performed using SPSS version 22. Results: This study comprised of 120 females. Self-confidence (48%) was the main motivating for undergoing treatment among the subjects. Social interaction (44%) was the most common area of improvement expected by the patients after orthodontic treatment. Financial constraints were the main barrier for seeking treatment found among 30% patients. Conclusion: Patients seek orthodontic treatment mainly to enhance facial esthetics, self-confidence and social acceptability. The mismatch of need and desire for treatment is a problem for orthodontists.

Effect of long-term smoking on salivary flow rate and salivary pH

Introduction: Saliva is a complex and important body fluid which is very essential for oral health and it is the first biological fluid that is exposed to cigarette smoke, which contains numerous toxic compositions responsible for structural and functional changes in saliva. Aim: To evaluate the long-term effect of smoking on salivary flow rate (SFR) and salivary pH. Materials and Methods: The subjects of the study were divided into smokers and nonsmokers. Each group comprised of 35 male adults. The saliva of each subject was collected under resting conditions. They were asked to spit in a graduated container at an interval of 60 s for 5 min. Salivary pH was measured immediately after measuring SFR using the (Indikrom Paper) pH indicator and calibrated cylinder. Based on the color change of the indicator paper strip, the pH was assessed in comparison with a color chart. Data were analyzed by Student′s t-test using SPSS 15. Results: The mean (±standard deviation) SFR and pH were 0.20 (±0.05) ml/min and 6.30 (±0.36) respectively in smokers while the mean SFR and pH were 0.36 (±0.06) ml/min and 7.10 (±0.24) in nonsmokers. The difference was statistically significant (P = 0.00). Conclusion: Long-term smoking significantly reduces the SFR and salivary pH.
