Summary Background A city-wide sanitation intervention was started in Salvador, Brazil, in 1997 to improve sewerage coverage from 26% of households to 80%. Our aim was to investigate the epidemiological effect of this city-wide sanitation programme on diarrhoea morbidity in children less than 3 years of age. Methods The investigation was composed of two longitudinal studies done in 1997–98 before the intervention (the sanitation programme) and in 2003–04 after the intervention had been completed. Each study consisted of a cohort of children (841 in the preintervention study and 1007 in the postintervention study; age 0–36 months at baseline) who were followed up for a maximum of 8 months. Children were sampled from 24 sentinel areas that were randomly chosen to represent the range of environmental conditions in the study site. At the start of each study an individual or household questionnaire was applied by trained fieldworkers; an environmental survey was done in each area before and after introduction of the sanitation programme to assess basic neighbourhood and household sanitation conditions. Daily diarrhoea data were obtained during home visits twice per week. The effect of the intervention was estimated by a hierarchical modelling approach fitting a sequence of multivariate regression models. Findings Diarrhoea prevalence fell by 21% (95% CI 18–25%)—from 9·2 (9·0–9·5) days per child-year before the intervention to 7·3 (7·0–7·5) days per child-year afterwards. After adjustment for baseline sewerage coverage and potential confounding variables, we estimated an overall prevalence reduction of 22% (19–26%). Interpretation Our results show that urban sanitation is a highly effective health measure that can no longer be ignored, and they provide a timely support for the launch of 2008 as the International Year of Sanitation.

Background Sanitation affects health, especially that of young children. Residents of Salvador, in Northeast Brazil, have had a high prevalence of intestinal parasites. A citywide sanitation intervention started in 1996 aimed to raise the level of sewer coverage from 26% to 80% of households. Objectives We evaluated the impact of this intervention on the prevalence of Ascaris lumbricoides, Trichuris trichuria, and Giardia duodenalis infections in preschool children. Methods The evaluation was composed of two cross-sectional studies (1998 and 2003–2004), each of a sample of 681 and 976 children 1–4 years of age, respectively. Children were sampled from 24 sentinel areas chosen to represent the range of environmental conditions in the study site. Data were collected using an individual/household questionnaire, and an environmental survey was conducted in each area before and after the intervention to assess basic household and neighborhood sanitation conditions. Stool samples were examined for the presence of intestinal parasites. The effect of the intervention was estimated by hierarchical modeling, fitting a sequence of multivariate regression models. Findings The prevalence of A. lumbricoides infection was reduced from 24.4% to 12.0%, T. trichuria from 18.0% to 5.0%, and G. duodenalis from 14.1% to 5.3%. Most of this reduction appeared to be explained by the increased coverage in each neighborhood by the sewage system constructed during the intervention. The key explanatory variable was thus an ecological measure of exposure and not household-based, suggesting that the parasite transmission prevented by the program was mainly in the public (vs. the domestic) domain. Conclusion This study, using advanced statistical modeling to control for individual and ecological potential confounders, demonstrates the impact on intestinal parasites of sanitation improvements implemented at the scale of a large population.

This study was conducted in an artisanal fishing community. The main health complaints included musculoskeletal disorders (MSD) attributable to working conditions. The present work found a prevalence of neck and distal upper limb MSD among the artisan fisherwomen/shellfish gatherers in Saubara, Bahia, Brazil. This was a cross-sectional cohort epidemiological study involving 209 artisanal fisherwomen/shellfish gatherers. The Brazilian version of the Job Content Questionnaire (JCQ), the Nordic Musculoskeletal Questionnaire (NMQ) and a survey listing physical demands adapted to shellfish gathering were used for the study. The MSD values obtained in some part of the body, neck or shoulder, and distal upper limb were 94.7%, 71.3% and 70.3%, respectively. The shellfish gatherers were found to work long shifts despite the high prevalence of MSD. The factors that cause these women to keep performing such activities include the need to make a living and provide food for their families through the sale and consumption of seafood.This study was conducted in an artisanal fishing community. The Brazilian public health system (SUS) has not been able to assure self-employed workers their universal right for health. These workers have neither the employment protection, nor the institutional support that is typical in the formal labor market. The present study aimed at analyzing the risks and health problems related to the artisanal fishermen’s and shellfish pickers’ work in order to contribute for the development of a workers’ health policy toward the self-employed. It is a case study which adopts a qualitative methodology with ethnographic, ergonomic and health surveillance approaches. The study was conducted in artisanal fishing communities between 2005 and 2010. The results showed ergonomic overload caused by exposure to different risks; precarious living conditions; invisibility of accidents and occupational diseases, such as repetitive strain injuries; lack of actions concerning prevention and health care, and the relevance of traditional knowledge within the artisanal fisheries’ labor organization. The Brazilian public health system and the National Occupational Health Network should provide both occupational health surveillance actions and occupational medical exams. These organizations should also contribute to increase awareness of occupational diseases, ensuring artisanal fishermen’s and self-employed workers’ right for health.

Quality of life is an indicator of how well one perceives that he/she is functioning physically and mentally. The aim of this paper is to determine the health-related quality of life (HRQOL) of artisanal fisherwomen/shellfish gatherers from the Saubara municipality in Bahia, Brazil in comparison to the general population. A structured questionnaire was administered to a sample of 209 artisanal fisherwomen selected at random. The HRQOL questionnaire, known as the 36-Item Short-Form Health Survey version 1 (SF-36v01), was also used, having been translated and verified cross-culturally for the Brazilian population. Sociodemographic, lifestyle and comorbidity information was also collected. Chronic diseases and indicators of musculoskeletal disorders (MSDs) were self-reported. The study population consisted primarily of individuals between 30 and 45 years of age (78%), of self-classified races black or brown (96.2%), with no more than an elementary school education (77%) and married (64.6%). In all the SF-36v01 dimensions, the values in the sample were lower than in the general population of Brazil, which was used as the reference population. In the “Physical Health” domain (Physical Functioning; Physical Role Limitations; Bodily Pain; General Health Perception) a tendency toward a lower health-related quality of life was observed among those who were older, had a lower education level, and had a prevalence of MSDs, hypertension or arthritis. The interference of health conditions linked to the fisherwomen’s work activities may contribute to lower HRQOL in all analyzed aspects, in comparison to the general population. In light of these findings, public health policies must consider these informal workers who contribute greatly to Brazil’s economy and food system.

The dialogue of knowledge between subsistence fishermen and shellfish gatherers on the right to a healthy working environment is established as a new process for claims for an improvement in working conditions by populations affected by environmental problems, and especially in Todos os Santos Bay (BTS). The communities surrounding the BTS have complained to the State Public Prosecutor about the harmful effects to health and the environment caused by the Aratu Industrial Complex and the Port of Aratu. Researchers in the fields of, chemistry, toxicology, oceanography, biology and medicine from the Federal University of Bahia (UFBA) have demonstrated the effects of contamination on the BTS in sundry scientific publications. The scope of this article is to reflect on the contribution of that dialogue on environmental labor law (DAT) in Brazil. The methodology of this study involved semi-structured interviews, participant observation and document analysis. The conclusion reached is that environmental labor law in Brazil must include the dialogue of knowledge to ensure access to a healthy working environment for subsistence fishermen and shellfish gatherers.

This paper discusses the role of qualitative approaches in epidemiological studies, beginning with a general discussion of epidemiological and anthropological methods. It focuses on a case study of the health impact of an environmental intervention carried out in Salvador, Bahia State, Brazil. Most of the precedent studies fields, based on primary date, use to devote little attention to the methodological and theorethical questions attached to long-term studies. Four specific aspects of this experience are highlighted: (a) drawing up epidemiological study; (b) construction of an observational instrument to measure hygiene habits; (c) an ethnographic study that was carried out before the epidemiological study; (d) observation of the effects of health intervention on health inequalities. Finally, the report details how the findings of qualitative and quantitative studies might be synthesized. It provides a critical overview of follow-up strategies, illustrated with proper examples whenever possible.

The aim is to analyze upper limb functioning and disability, and its association with health-related quality of life among artisanal fisherwomen from Bahia, Brazil. Cross-sectional epidemiological study was conducted with a sample of 209 fisherwomen. Structured questionnaires were used for socio-demographic and comorbidity information, as well as the instruments Disabilities of the Arm, Shoulder and Hand (DASH) and Short-Form Healthy Survey (SF-36) respectively, to evaluate the upper limbs and health-related quality of life. The results demonstrated that the presence of musculoskeletal disorders in the upper limbs directly affects the values of the DASH instrument and the SF-36v01 questionnaire scores, while also generating a negative correlation between the DASH and SF-36v01. The varying functioning abilities, pain and social aspects negatively affect upper limb function, and the daily activities and work of fisherwomen. The presence of chronic disease and the absence of intervention and rehabilitation for these professionals, that could produces, in a long-term, cases of disability.

INTRODUCTION The Occupational Accident (OA) is considered to be an important public health problem in Brazil. Traumatic Brain Injury (TBI) is the most common among them. The TBI is associated with high morbidity and mortality rates among workers. OBJECTIVE To identify factors associated with providing social security benefits for TBI due to occupational accidents according to the specific type of economic activity in Brazil, in 2009. METHODS This is a cross-sectional study that was conducted with all workers who were part of the General Regime of Social Security (RGPS) of Brazil. Secondary data were obtained from the National Information System Benefit, from the Synchronized National Register of the Ministry of Finance and from the General Register of Employed and Unemployed Persons. Data were analyzed using the multiple logistic regression method. RESULTS We analyzed 2,006 cases of social security benefits for traumatic brain injury due to Occupational Accident. Factors associated with the concession of the benefit according to the economic activity of the Company of the beneficiary were identified. Associations were found with sex, income and the region of the Company. CONCLUSION Factors associated with the concession of social security benefits by TBI resulting from OA differ depending on the type of economic activity in the study. Understanding these factors may contribute to the planning of preventive policies.

Estudo observacional transversal realizado em Goiânia, Goias, Brasil, em uma amostra de 56% de 111 pessoas, objetivando avaliar a qualidade de vida dos individuos expostos ao cesio-137 e sua associacao com fatores sociodemograficos. Utilizou-se o WHOQOL-BREF e o Sistema de Monitoramento dos Radioacidentados. Os participantes foram divididos segundo criterios internacionais em: Grupo I - radiodermites e/ou dosimetria citogenetica acima de 20 rads (n = 33); e Grupo II - dosimetria citogenetica ≤ 20 rads (n = 29), totalizando 62 sujeitos. Dentre os dominios do WHOQOL-BREF, o meio ambiente apresentou a media de escores mais alta (59,88; DP = 20,39) e o psicologico a media mais baixa (53,02; DP = 17,98). As associacoes entre os dominios fisico, psicologico e relacoes sociais foram significativas para a variavel idade. Nao houve diferenca entre grupos. A associacao entre os fatores sociodemograficos e qualidade de vida nao foi significativa. Os radioacidentados sofrem consideravel impacto na qualidade de vida, com persistencia de problemas psicossociais, especialmente para aqueles com mais de 41 anos.This cross-sectional observational study in Goiânia, Goiás State, Brazil in a sample of 56% of 111 individuals exposed to cesium-137 aimed to evaluate their quality of life and associations with socio-demographic factors. The study used the WHOQOL-BREF and the Monitoring System for Radiation Victims. Participants were divided according to international criteria: Group I - radiation dermatitis and / or cytogenetic dosimetry above 20 rads (n = 33) and Group II - cytogenetic dosimetry ≤ 20 rads (n = 29), totaling 62 subjects. Among the WHOQOL-BREF domains, environment showed the highest mean scores (59.88, SD = 20.39) and psychological the lowest (53.02, SD = 17.98). Associations between the physical, psychological, and social domains were significant for the age variable. There was no difference between groups. The association between socio-demographic factors and quality of life was not significant. Radiation victims suffer considerable impact on quality of life, with persistent psychosocial problems, especially among those older than 41 years.
