Dental caries related to quality of life in two Brazilian adolescent groups: a cross‐sectional randomised study

OBJECTIVES To analyse dental caries-related quality of life (QoL) in adolescent (15-19-year-old) subjects in a suburban area (SA) and a downtown area (DA) of Bauru, São Paulo, Brazil, in 2009. METHODS This was a cross-sectional, randomised study. The sample consisted of 185 and 147 adolescents from the SA and DA, respectively. The caries index used was that for decayed, missing and filled teeth (DMFT) (World Health Organization criteria). The 14-item Oral Health Impact Profile (OHIP-14) was used to assess QoL. The non-parametric Mann-Whitney test, Spearmans correlation coefficient and chi-squared test were used in the statistical analysis. RESULTS The DMFT index, Significant Caries (SiC) Index and percentage of caries-free students were similar (P > .05) between these populations, but findings on the Care Index differed (P < 0.05). There was a correlation between the DMFT index and OHIP-14 score in SA subjects (mean DMFT index = 3.01) in the dimensions of physical pain (r = 0.25; P < 0.01) and psychological disability (r = 0.17; P = 0.02). The DMFT index in DA subjects (mean DMFT index = 2.95) showed a correlation with functional limitation (r = 0.19; P = 0.02). The correlation in SA subjects between the caries component (mean = 1.22) of the DMFT index and OHIP-14 was significant only for the physical pain dimension (r = 0.16; P = 0.03). In DA subjects, correlations between the caries component (mean = 0.37) and all dimensions of OHIP-14 were statistically significant (P < 0.05). CONCLUSIONS  Despite the similarities in means in the DMFT index, the SiC Index and the caries-free percentage of subjects in both localities, access to dental treatment proved to be less effective for SA adolescents. Physical pain and psychological disability were the most frequent negative manifestations of impact on QoL. The SA adolescents were more negatively affected by dental caries in terms of QoL.

Decline in dental caries among 12-year-old children in Brazil, 1980-2005

OBJECTIVES This study was conducted to determine changes in values on the decayed, missing and filled teeth (DMFT) index in 12-year-old children in Brazil between 1980 and 2005, and to correlate DMFT values with human development index (HDI) values, time, population size of municipality and fluoridation of the water supply. METHODS The present study represents a retrospective ecological study using secondary data from epidemiological surveys published in indexed journals, as well as data obtained from epidemiological official surveys carried out in Brazil in 1986, 1996 and 2003, and in the State of São Paulo in 1998 and 2002. Units of study were represented by Brazilian municipalities holding average DMFT index values for 12-year-old children. Multiple regression analysis was used to examine the correlations among DMFT and HDI values, and time, population size and fluoridation of the water supply. RESULTS The final database included 550 records of DMFT values in 428 different towns. Regression analysis showed statistically significant correlations between DMFT index values and time (P < 0.001), fluoridation of the water supply (P < 0.001) and size of municipality (P < 0.001). Estimated mean DMFT index values were 8.36 in 1980, 6.08 in 1985, 4.45 in 1990, 3.29 in 1995, 2.46 in 2000 and 1.86 in 2005. CONCLUSIONS Data showed a significant decrease in dental caries across the entire country, with an average reduction of 25% occurring every 5 years. General trends indicated that a reduction in DMFT index values occurred over time, that a further reduction in DMFT index values occurred when a municipality fluoridated its water supply, and mean DMFT index values were lower in larger than in smaller municipalities.

Dental caries profile in Monte Negro, Amazonian state of Rondônia, Brazil, in 2008

Objective This epidemiological survey assessed the dental caries profile in Monte Negro, a small town in the Amazonian state of Rondônia, Brazil, and its relationship with the northern region, national and global goals for oral health in the years 2000 and 2020. Material and Methods The groups randomly examined were composed of individuals aged 5, 12, 15 to 19, 35 to 44, 65 to 74 years, living in both rural and urban areas. Results The means dft (standard deviation) and DMFT (standard deviation) for the groups were, respectively, 3.15 (3.12), 3.41 (2.69), 5.96 (4.19), 16.00 (7.30) and 25.96 (9.82). Caries-free individuals were 34.42%, 14.81% and 8.16% in the preschoolchildren, schoolchildren and adolescent groups, respectively. The Significant Caries Index percentages applied to the two younger groups were 6.65 and 6.70, and they increased to 32.00 in the individuals aged 65 to 74 years. Care Index percentages for adolescents, adults and elderly groups were, respectively, 29.40, 25.00 and 1.41. The dental caries profile in Monte Negro in 2008 shows that, 8 years after the year 2000, no FDI/WHO goal for any age settled in 1982 has been achieved. Dental caries increased with age and the main dental problem of adult and elderly groups was tooth loss. Conclusion Oral health promotion and prevention of oral disease policies are urgent needs. Setting of oral health goals and targets to people living in Monte Negro or Amazonia to be pursuit and achieved in a near future is an important action to do because of the culture, sanitary conditions and socioeconomic aspects of this particular population.

Dental caries-related quality of life and socioeconomic status of preschool children, Bauru, SP

Aim: To evaluate oral health-related quality of life of preschool children of Bauru, State of Sao Paulo, Brazil, and associate it with socioeconomic profile of households. Methods: The sample consisted of 229 preschool children between 3 and 5 years and the dmft (decayed, missing due to caries, filled teeth) index was adopted for assessment children’s dental caries in accordance with the standards recommended by the World Health Organization. Questionnaires were used for evaluation oral health-related quality of life (Early Childhood Oral Health Impact Scale) and socioeconomic profile of parents or guardians of the preschool children. Statistical analysis was performed descriptively by relative and absolute frequencies and by Spearman’s correlation and Kruskal-Wallis test (p <0.05). Results: A dmft of 1.65 (± 2.87) and a Sic Index 4.88 (± 3.20) were found, indicating the polarization of dental caries in the studied group. It was verified low influence of oral health on quality of life of the children examined. With respect to socioeconomic classification, 66.38% of families were in the lower middle class. Linear and statistically significant correlation was found between dmft and oral health-related quality of life for the overall score and domains of the questionnaire (p<0.001). Conclusions: It was found low influence of oral health on quality of life of the preschool children and the assessment of socioeconomic conditions of the children’s families may guide practices aiming to reducing inequalities in the distribution of dental caries in the population.

Oral health-related quality of life before hematopoietic stem cell transplantation

ObjectivesThis study was planned to evaluate the impact of oral health on the quality of life (QoL) of patients undergoing hematopoietic stem cell transplantation (HSCT).Material and methodsWe assessed 200 patients divided into two paired groups: 100 patients prior to HSCT (SG) and 100 healthy volunteers (CG). We applied the Oral Health Impact Profile instrument, which is based on the biopsychosocial problem gradation of World Health Organization (WHO) and relates oral health problems with QoL according to seven dimensions.ResultsFourteen patients in SG were referred for extraction of one to eight teeth, mostly due to deep caries with risk of pulpal infection and possible spread of infection via blood (r = 0.59, p = 0.000). The presence of severely compromised teeth by extensive decay correlated with Oral Health Impact Profile (OHIP-14). The Mann-Whitney test showed a significant difference between SG and CG in the following dimensions: functional limitation (p < 0.001), physical pain (p = 0.025), physical disability (p = 0.016), and social disability (p = 0.01).ConclusionsThe impact of oral health on QoL of onco-hematologic patients is weak but is greater as compared to healthy ones. Nevertheless, a significant impact is seen in patients with severely compromised teeth.Clinical relevanceThe negligence of oral care, proper hygiene, and the search for dental care may increase the risk for local and systemic infections.

Correlation between dental caries and nutritional status: preschool children in a Brazilian municipality

INTRODUCTION: Dental caries and nutritional status in children in preschool age are serious public health problems, with multifactorial etiology, with diet as a common risk factor. OBJECTIVE: This cross-sectional study assessed the relationship between dental caries and nutritional status of preschool children attending public schools in a city in the State of Sao Paulo. MATERIAL AND METHOD: The study population was comprised of 3-5 year-old preschool children (n = 229) attending public schools, in which dmft (decay, missing, filled, teeth) was used for dental caries assessment and Body Mass Index (BMI) was used for nutritional status in accordance with the recommendations of the World Health Organization. Statistical analysis was performed in a descriptive way through absolute and relative frequencies and Spearman Correlation test and Kruskal Wallis (P<0.05). RESULT: A dmft of 1.65 (2.87) and a SiC index (Significant Caries Index) of 4.88 (3.20) have been found, indicating polarization of dental caries. It was observed that 66.81% of children presented with nutritional status within the normal range and children with malnutrition had a mean dmft of 4.0 (3.66), which is two times higher than the other categories of nutritional status. No statistically significant correlation has been found by correlating dmft and components with Body Mass Index. CONCLUSION: This research did not identify a significant correlation between the occurrence of dental caries and nutritional status of preschool children, researches should be conducted to elucidate this relationship.

Vigilância do desenvolvimento da linguagem da criança: conhecimentos e práticas de profissionais da atenção básica à saúde

Objetivo: analisar os conhecimentos e as praticas dos enfermeiros, medicos e cirurgioes-dentistas que atuam na Atencao Basica em relacao ao desenvolvimento da linguagem da crianca nos primeiros anos de vida. Metodos: tratou-se de uma pesquisa qualitativa com 30 profissionais de uma rede de Atencao Basica dentre eles, medicos, enfermeiros e cirurgioes-dentistas. Uma consulta individual foi realizada mediante um questionario semiestruturado. Empregou-se a tecnica de analise tematica de discurso, utilizando-se tres figuras metodologicas: a Ideia Central, as Expressoes-Chaves e o Discurso do Sujeito Coletivo. Resultados: os conhecimentos que os profissionais tem sobre o desenvolvimento da linguagem da crianca estavam ancorados as ideias centrais de conhecer alguns marcos do desenvolvimento, a linguagem da crianca depende do estimulo do meio, da familia e da audicao normal, saber muito pouco ou nao saber nada sobre o assunto. Os profissionais relataram que gostariam de obter mais informacoes sobre o assunto em relacao aos marcos do desenvolvimento da linguagem da crianca, desvios da normalidade e orientacoes para pais de forma a aprimorar o atendimento da saude infantil. Conclusao: o conhecimento dos profissionais sobre o assunto apresentou-se limitado, havendo necessidade de ampliar praticas de educacao em saude pela Fonoaudiologia, em parceria com as Instituicoes de Ensino e os profissionais dos Nucleos de Apoio Saude da Familia, junto aos profissionais da Equipe de Estrategia Saude da Familia, destacando-se o trabalho sobre os marcos do desenvolvimento da linguagem da crianca.

Odontologia desportiva – proposta de um protocolo de atenção à saúde bucal do atleta

A consulta ao cirurgiao-dentista deve fazer parte do protocolo inicial e periodico de atencao a saude para pratica esportiva. Inicialmente, a odontologia privilegiava o atleta atraves do uso do protetor bucal, principalmente em esportes de contato fisico com o adversario para prevenir traumatismos. Atualmente, a atencao deve ser mais ampla para promover a saude, aumentar o rendimento fisico do atleta e nao somente prevenir traumatismos. Propoe-se aqui, baseado em uma revisao de literatura, um protocolo de atencao a saude bucal do atleta em um ciclo de quatro fases. Na primeira, a eliminacao de focos infecciosos, na segunda, o tratamento ortodontico conjugado ao da respiracao bucal. A terceira fase contempla a reabilitacao bucal e na ultima indica-se a manutencao e o controle do padrao de saude bucal alcancado por meio de educacao em saude bucal e atencao preventiva. Nao se deve olvidar que a atencao a saude do esportista deve ser observada de forma multidisciplinar e a odontologia deve fazer parte desta atuacao em saude. ABSTRACT The dentistry practice must be part of a protocol in the beginning and periodically of the health attention before and during sports practice. First of all, the dentist used to privilege the athlete only with custom-made mouthguard, mainly in contact sports with the aim of prevention of trauma. Now a day, this attention must be higher to promote health, get the athletic rates to the limit and not only prevent trauma. A proposal has been given here, based in a literature review, as a protocol of oral health attention into a cycle of four moments. Firstly, the elimination of infectious sites must be done. Secondly, oral breathing and malocclusion treatment is indicated. In the third moment, oral rehabilitation treatment and in the last, maintenance and control of oral health reached will be done through oral education and preventive dentistry. Equidade na Estratégia Saúde da Família (ESF)

Objective: To associate the implementation of the Family Health Strategy (FHS) between 2001 and 2010 with the variables of the Municipal Human Development Index (MHDI) and socioeconomic indicators. Methods: Ecological study collected information on the MHDI, socioeconomic indicators, and the FHS, regarding the number of Community Health Workers (CHW), Family Health Teams (FHT), Oral Health Teams (OHT), and population and relative (%) coverage in 645 municipalities in the state of Sao Paulo, Brazil, in the period from 2001 to 2010. Descriptive analysis was conducted by means of absolute and relative frequencies for evaluation of each indicator. Non-parametric tests were used: Spearman’s correlation coefficient for the FHS variables and the socioeconomic indicators, and Wilcoxon test (p<0.05) for comparison between the two dates. Results: There was positive correlation between the increase in FHS and OHT teams, and the MHDI (p<0.01). Social data (access to water, waste collection percentage, and average income presented positive correlation with FHS and population coverage. A negative correlation was found between population coverage (%) and all the social variables except illiteracy. Conclusion: The evolution of the FHS in the state of Sao Paulo during the studied period was positive in fighting the health inequalities.
