Comparison between panoramic radiography and cone-beam computed tomography findings for assessment of the relationship between impacted mandibular third molars and the mandibular canal

ObjectivesThe aim of this study was to assess the relationship between the mandibular canal and impacted mandibular third molars using cone-beam computed tomography (CBCT) and to compare the CBCT findings with panoramic radiographic signs.MethodsThis study involved a retrospective radiographic review of 781 impacted third molars in 500 patients who showed a close relationship between the mandibular canal and the third molars on panoramic radiographs. Panoramic radiographic images were evaluated for interruption of the white line, darkening of the roots, diversion of the mandibular canal/roots, and narrowing of the mandibular canal/roots. The authors evaluated CBCT images to determine the course of each canal and its proximity to the roots. The statistical correlations between the panoramic radiography and CBCT findings were examined using the Chi-square test and Fisher’s exact test.ResultsCone-beam computed tomography examination showed that darkening of the roots and deviation of the canal associated with the absence of corticalization between the mandibular third molar and the mandibular canal on panoramic radiographs were statistically significant, both as isolated findings and in association. No significant associations were observed for the other panoramic radiographic findings, either individually or in association.ConclusionsThe results of this study suggest that darkening of the roots, deviation of the mandibular canal, and interruption of the white line observed on panoramic radiographs, both as isolated findings and in association, were effective for determining the risk relationship between the roots and the mandibular canal, requiring three-dimensional evaluation of such cases.

Evaluation of temporomandibular fossa and mandibular condyle in adolescent patients affected by bilateral cleft lip and palate using cone beam computed tomography

The purposes of this study were to investigate the position of the mandibular condyle and temporomandibular fossa between the adolescent patients affected by bilateral cleft lip and palate (BCLP) and well-matched controls without any cleft by using cone-beam computed tomography (CBCT). The study sample consisted of 17 patients (7 females and 10 males; mean age, 14.27 ± 2.83 years) affected by BCLP and 17 patients (6 females and 11 males; mean age, 14.27 ± 2.12 years) as age-and sex-matched control group without any cleft. Using cone-beam computed tomography segmented three-dimensional temporomandibular fossa and mandibular condyle images were reconstructed and angular, linear, and volumetric measurements of the patients in both sides of the groups were examined using Paired and Students t-tests. Comparison of the sides showed that both sides were found to be similar in BCLP and control groups, except the condylar angulation of the right side was found to be higher compared to that of the left side in both groups (p < 0.05). Comparison of the groups showed that the patients affected by BCLP and non-cleft patients had similar values for all parameters, except for the condylar angulation in the right side (BCLP group had less angulation compared to controls; p < 0.05). The condylar volume was found to be slightly less in the BCLP group in both sides compared to the controls (p > 0.05). The positions of the mandibular condyle and temporomandibular fossa were found to be similar in patients affected by BCLP and control group of without any cleft. SCANNING 38:720-726, 2016.

KONİK IŞINLI BİLGİSAYARLI TOMOGRAFİ KULLANILARAK ELDE EDİLEN İKİ VE ÜÇ BOYUTLU LATERAL SEFALOMETRİK ANALİZLERİN KARŞILAŞTIRILMASI

Amac: Bu calismanin amaci, konik isinli bilgisayarli tomografi (KIBT) kullanilarak elde edilen iki (2D) ve uc boyutlu (3D) lateral sefalometrik analizlerin karsilastirilmasidir. Gerec ve yontem: 25 hastanin (12 erkek ve 13 bayan; ortalama yas: 25,22 ± 4,92 yil; yas dagilimi: 18-35 yil) 2D ve 3D olarak cizilen lateral sefalometrik goruntuleri bu calismanin materyalini olusturmaktadir. Tum KIBT goruntuleri KIBT (NewTom 5G, QR Verona, Italy) kullanilarak supin pozisyonunda alinmistir. DICOM dosyalari SimPlant yazilimi (SimPlant Pro 2011, Materialise, Leuven, Belgium) kullanilarak elde edildi ve tum olcumler bu program kullanilarak yapilmistir. Sefalometrik analizlerin karsilastirilabilmesi icin 8 iskeletsel, 8 dissel ve 3 yumusak dokuyu iceren toplam 19 parametre (14 acisal ve 5 boyutsal) olculmustur. Her iki yontemle belirlenen olcumler eslestirilmis t-testi kullanilarak karsilastirilmistir. Ayrica, Pearson korelasyon katsayilari hesaplanmistir. Bulgular: 2D ve 3D olarak cizilen lateral sefalometrik filmlerin tekrarlanabilirligi kabul edilebilir sinirlar icinde bulunmustur. Eslestirilmis t testi sonucunda; SN-GoGn (°) ( p = 0,011), MP-PP (°) ( p = 0,006), Y (°) ( p = 0,009) ve IMPA (°) ( p = 0,002) acilarinda, N-Me (mm) ( p = 0,043) ve U1-NA (mm) ( p = 0,000) olcumleri arasinda istatistiksel olarak anlamli farklilik oldugu gorulmustur. Pearson korelasyon katsayisi U1-NA (°) (r = 0,575) ve nasolabial aci (°) (r = 0,641) ve L1-APog (mm) (r = 0,658) mesafesi haric tum olcumlerde yuksek olarak tespit edilmistir. Sonuc: KIBT goruntuleri kullanilarak elde edilen 2D ve 3D lateral sefalometrik analizler karsilastirildiginda, Pearson korelasyon hemen hemen tum olcumlerde yuksek olmasina ragmen, SN-GoGn (°), MP-PP (°), Y acisi (°) ve N-Me (mm) gibi dik yon ile ilgili degerlerde istatistiksel olarak anlamli farklilik gozlenmistir. Unilateral Mandibular İkinci Premolar Eksikliği Olan Hastalarda Kondiler Asimetri

OZET Amac: Unilateral mandibular ikinci premolar eksikligi olan hastalarda kondiler ve ramal vertikal asimetriyi arastirmaktir. Bireyler ve Yontem: Unilateral mandibular ikinci premolar eksikligi olan 30 kisiyi iceren calisma grubu (25 kadin, 35 erkek) ve 57 kisilik kontrol grubun (41 kadin, 16 erkek) hem iskeletsel hem de dental Sinif I iliskiye sahipti. Kondiler, ramal ve kondiler ramal yukseklikler toplami Paired t testi kullanilarak her iki cene segmenti icin hesaplandi. Ayrica; calisma ve kontrol gruplari icin asimetri indekslerinin hesaplanmasi icin Student t testi kullanildi. Bulgular: Asimetri indeks olcumleri cinsiyetler arasinda istatistiksel olarak farkli degildi ve kiz ve erkeklere ait veriler istatistiksel degerler birlestirilerek degerlendirildi. Calisma grubunda kondiler, ramal ve kondiler-ramal yukseklikleri toplami dis eksikligi olan tarafta normal taraftan daha dusuk bulundu. Ancak, bu farklilik istatistiksel olarak anlamli degildi.
