Masticatory afficiency in denture wearers with bilateral balanced occlusion and canine guidance

The maintenance of masticatory function is especially important for patients wearing complete dentures due to their limitations. Thus, the bilateral balanced occlusal concept is used to achieve greater masticatory efficiency. However, a critical review of the literature reveals that there is not sufficient scientific evidence to support bilateral balanced occlusion as the most appropriate occlusal concept in complete dentures. Therefore, the aim of this study was to evaluate the masticatory efficiency in complete dentures wearers with bilateral balanced occlusion and canine guidance. A double-blinded controlled crossover clinical trial was conducted. The sample was composed by 24 edentulous patients who wore sets of complete dentures with both occlusal concepts during equal periods of 3 months. Objective data were collected through the masticatory efficiency test performed by the colorimetric method with the beads, in which capsules of a synthetic material enclosing fuchsine-containing granules were used. Subjective data were recorded by patients ratings of their chewing function. No significant statistical difference was found for masticatory efficiency (p=0.095) between the two occlusal concepts studied. The results suggest that bilateral balanced occlusion does not improve the masticatory efficiency in complete denture wearers.

Confiabilidade da eficiência mastigatória com beads e correlação com a atividade muscular

BACKGROUND the use of the colorimetric method with beads to evaluate mastigatory efficiency may be promising, however no report is found about its reliability. AIM to investigate the reliability of the beads to test masticatory efficiency and its correlation with the electromyographic activities of the anterior temporal and masseter muscles. METHODS participants of this study were nineteen young adults, nine males and ten females, aged eighteen to twenty-eight years, with full dentition, Angle class I, with no history of temporomandibular disorder, neurological or cognitive deficit, previous or current tumors or traumas in the head and neck region, and orthodontic treatment or orofacial myofunctional therapy. The masticatory efficiency test was performed using beads, fuchsine-containing granules, in the following conditions: habitual chewing, right and left unilateral chewing, for 20 seconds. Electromyographic recordings were obtained simultaneously. Also, the maximal clenching was registered. The amount of fuchsin released upon chewing was measured using a Beckman DU-7 UV-Visible Spectrophotometer (Beckman Inc., Palo Alto, CA, USA). RESULTS high reliability was observed for the masticatory efficiency test (r = 0.86, p < 0.01) and correlation with the electromyographic activities (r = 0.76, p < 0.01). Also, positive and significant correlations were observed when the conditions were separately analyzed. CONCLUSION the masticatory efficiency test performed with beads proved to be a reliable method and positively correlated to the electromyographic activities of the anterior temporal and masseter muscles.

A importância da saúde bucal para adolescentes de diferentes estratos sociais do município de Ribeirão Preto

The objective of the present study is to know the importance of buccal health for adolescents of different social strata, identifying the importance of dental aesthetics, and the care that these students take in relation to their buccal health. The methodological process adopted in this investigation is of qualitative nature, using as technique of collection of data the semi-structured interview. We know the importance of buccal health in several contexts of their lives, as in their personal appearance, sexuality, employment, and general health. This investigation allows us to know the motivations of adolescents to preserve their buccal health, and we believe to be so valid to develop health promotionh through health education.The objective of the present study is to know the importance of buccal health for adolescents of different social strata, identifying the importance of dental aesthetics, and the care that these students take in relation to their buccal health. The influence of mandibular implant-retained overdentures in masticatory efficiency

OBJECTIVE To evaluate the masticatory efficiency of patients rehabilitated with conventional dentures (CDs) or implant-retained mandibular overdentures. BACKGROUND Despite the evident benefits of implants on mastication as assessed by subjective patient-based outcomes, the extent of implant overdenture treatment effect on food comminution is not well established. MATERIALS AND METHODS A randomised clinical trial was carried out with 29 completely edentulous patients divided into two groups. The first group was rehabilitated with a mandibular overdenture retained by two splinted implants with bar-clip system, while the second group was rehabilitated with a mandibular CD. Both groups also were rehabilitated with maxillary CDs. Masticatory efficiency and patient satisfaction were assessed 3 months after denture insertion. Masticatory efficiency was evaluated through the colorimetric method with the beads as the artificial test-food. Comparisons for masticatory efficiency and patient satisfaction were performed using Students t-test (α = 0.05). RESULTS No significant statistical difference was found for masticatory efficiency (p = 0.198). Patient overall satisfaction was significantly higher for the mandibular overdenture (p < 0.001). In addition, mandibular overdenture patients were significantly more satisfied with chewing experience (p < 0.05) and retention of the lower denture (p < 0.005). CONCLUSION The results of this study suggest that mandibular overdenture significantly improves chewing experience, although limited effect on masticatory efficiency has been observed.

Comparison of different methods for the occlusal dentine caries diagnosis

The aim of this study was to compare the effectiveness of in-vitro methods for the occlusal dentine caries diagnosis. Thirty-eight sites were evaluated on third molars without macroscopic carious cavitation in adult individuals from the city of Barretos (SP), Brazil. Visual inspection (VI), endoscopic evaluation (AcuCam), visual inspection and blunt-tipped exploratory probes (Tactile), conventional bite-wing radiographs (CR), direct bite-wing digital radiograph (DR), and direct digital radiograph with contrast and brightness controled (DRbc) were used by five observers. In order to validate the data, the teeth were sectioned and histologically evaluated. The average sensitivity and specificity values of the methods were respectively:0,25 , 0,96 (VI); 0,15 , 0,92 (AcuCam); 0,17 , 0,95 (Tactile); 0,45 , 0,73 (CR); 0,33 , 0,80 (DR) and 0,35 , 0,84 (DRbc) , the effectiveness of clinical methods (VI, AcuCam and Tactile) as well as that of radiographic methods (CR, DR and DRbc) were comparatively similar. The clinical methods presented a smaller number of false-positive diagnosis than the radiographic methods. It was concluded that visual inspection is an important diagnostic method; conventional bite-wing and digital radiography aid the diagnosis and are equally efficient to diagnose carious lesions in the dentine of teeth without visible cavitation.

Impact of mandibular conventional denture and overdenture on quality of life and masticatory efficiency.

The aim of this non-randomized controlled clinical trial was to evaluate the oral health-related quality of life and masticatory efficiency of patients rehabilitated with mandibular two-implant overdentures with immediate loading or conventional dentures. Fifty completely edentulous patients wearing bimaxillary conventional dentures, for at least one year, were recruited. The patients were then assigned to either two treatment groups: mandibular overdentures supported by two implants with bar-clip system and a maxillary conventional denture (n = 25), and new maxillary and mandibular conventional complete dentures (n = 25). Masticatory efficiency and oral health-related quality of life were assessed before and 3 months after denture insertion. The Brazilian version of OHIP-Edent questionnaire was used to assess the oral health-related quality of life. Masticatory efficiency was evaluated with chewing capsules through a colorimetric method. The results revealed fewer oral health-related quality of life problems in patients wearing mandibular two-implant overdentures compared to the conventional dentures group. In addition, the implant overdenture group presented statistically significant improvement in masticatory efficiency (p = 0.001). There was no correlation between masticatory efficiency and OHIP in the implant group (p > 0.05), however a correlation was found in the conventional denture group (p < 0.05). Therefore, these short-term results suggest that mandibular overdenture retained by 2 implants with immediate loading combined with maxillary conventional dentures provide better masticatory efficiency and oral health-related quality of life than mandibular conventional dentures.

Método para avaliaçäo clínica da capacidade tamponante salivar

The present study aimed to develop and standardize a colorimetric method for assessing salivary buffer capacity adapted to the features of the Brazilian population. Samples of stimulated saliva were titrated without CO2 elimination. The assessment was carried out to study the influence of the loss of CO2 from the samples during the few minutes necessary for titration, and to standardize the instants for measuring pH. Saliva samples were titrated from 206 individuals from both genders, between 5 and 50 years of age, and in a proportion of 0.77 : 1 : 0.44, respectively of children, youngsters, and adults. Case frequency distribution; determination of average values of initial pH, pKa, µ equivalents related to titration end point and final pH; and estimate of the percentage of saliva titrated at a pH of 5.0 were calculated based on the data collected. The results were used to classify individuals according to their salivary buffer capacity by colorimetric pH determinations using 11,12, and 13 µ equivalents of H+/ml saliva, and were compared to case frequency distribution obtained from saliva titration data. The results indicate that 11 µ equivalents H+/ml of saliva is the most appropriate for clinically evaluating the salivary buffer capacity of our population.

Comparison of dynamic electromyographic analysis of masticatory capsules with materials of different textures

Objectives: The aim of this study was to compare the mastication electromyographic indexes of capsules used for the masticatory efficiency evaluation “ME-mastig” and of materials of different textures (gum, raisins, and peanuts). Methods: Thirty young, healthy, adult subjects, with an average age of 23.46 years old, with no symptoms of temporomandibular dysfunction (TMD), confirmed by the clinical exam, RDC/TMD, and the static test of electromyography (EMG) were chosen. Afterward, dynamic tests were performed (bilateral mastication of the compounds) to determine number of chewing cycles (CYCLES); masticatory frequency (FREQ); SMI (symmetrical masticatory index – corresponding to the value of neuromuscular coordination during mastication); and IMPACT (corresponding to impact – total electromyographic activity performed by the system during chewing). Results: Cycle and frequency were similar. Regarding SMI and IMPACT, the raisin, the capsule, and the peanut were similar between each other, and different from the gum. Conclusions: Masticatory capsules and the natural foods with different textures had similar results regarding EMG indexes.

Oral health conditions and quality of life of elderly users of the Unified National Health System

OBJECTIVE: This study aimed to analyze the oral health status and socio-economic conditions of the elderly, which are users of the Brazilian Unified Public Health System (SUS) and its relationship to a subjective indicator of impacts of oral conditions on quality of life (Oral Health Impact Profile OHIP-14).METHODS: It is a descriptive cross-sectional exploratory study developed in the second semester of 2011. The sample consisted of 76 elderly (65-74 years old) users of the public dental service at the Dental Specialties Center at the Faculty of Dentistry of Ribeirao Preto, Brazil. The methodological strategy adopted for data collection was the structured interview and secondary data were obtained from medical records and reports. To perform the statistical analysis it was used an exploratory analysis of multivariate data by means of STATA Software.RESULTS: The frequencies of the dimensions of the OHIP-14 with the greatest impact were pain (13.16%) and psychological distress (14.47%). The low impact association was represented by social disability (71%) and social disadvantage (63%). The gender, type of housing, necessity and prosthesis wearing were not associated with measures of quality of life related to oral health. It was observed a good correlation between the perception (r = 0.542, p <0.001), a moderate association between higher income (r = 0.28, p <0.01) and the scores of the OHIP.CONCLUSION: It was concluded that the elderly which reported good self-perception of oral health and good income have expressed the perception that oral health reflects a good impact on quality of life.
