Mass population screening for cervical cancer in Miyagi Prefecture

Abstract Mass population screening for cervical cancer was performed on 647,999 individuals in Miyagi Prefecture from January 1962 until March 1977. Invasive carcinoma of the uterine cervix was discovered in 1076 women, carcinoma in situ in 1066, and a high degree of dysplasia in 2637. The Cytology Activity Index of the examined women 30 years or older in 1965, 1970, and 1975 was 60.0, 116.7, and 193.2, respectively. The prevalence rate (per 10 5 ) of invasive carcinoma in the individuals of the population screening was 136.7 in 1971 and 71.5 in 1975, whereas the prevalence rate of carcinoma in situ was 136.0 and 99.9. The yearly incidence rate (per 10 5 ) of invasive cancer of the uterine cervix in Miyagi Prefecture for the 4 years from 1969 until 1972 was 32.9. The standardized death rate (per 10 5 ) from malignant neoplasms of the uterus has been roughly at the 5.0 level since 1972.

Nitroblue tetrazolium reduction by leukocytes in the cervix of pregnant women

We examined nitroblue tetrazolium reduction activity in leukocytes from cervical mucus as well as peripheral blood from pregnant and nonpregnant women by histochemical methods. More than 90% of the cells examined were polymorphonuclear cells. Nitroblue tetrazolium reduction in polymorphonuclear cells from peripheral venous blood was significantly enhanced in pregnant women compared with that in nonpregnant women. However, there was no difference in activity between pregnancy periods. On the other hand, nitroblue tetrazolium reduction in cervical polymorphonuclear cells excluding women with cervicitis was also greater in pregnant women than in nonpregnant women. Moreover, activity in cervical polymorphonuclear cells gradually increased as pregnancy progressed. This enhanced activity in polymorphonuclear cells may be involved not only in the defense mechanism but also in cervical ripening in pregnancy.

Analysis of interobserver reproducibility and its improvement in endometrial cytology.

子宮内膜細胞診における「観察者間の判定再現性」を解析するため, 内膜腺癌・増殖症および非腫瘍性変化を含む70個の上皮性細胞クラスターについて, 19人の経験豊かな観察者 (11人の細胞検査士と8人の指導医) に異型度判定を依頼した. 判定は, 各観察者が70個のクラスターのそれぞれに対し3種のカテゴリー (「陰性」,「疑陽性」および「陽性」) の中の一つを与えるものとした. 判定一致の程度はKappa (κ) 統計値を用いて表した。全体の一致はκ=0.36, 判定カテゴリー別の一致は「陰性」に対しκ=0.46,「陰性」に対しκ=0.47,「疑陽性」に対しκ=0.14であった.観察者が一堂に集まりこれらの結果を検討学習する機会をもち, その後2回目の異型度判定を依頼した. その結果, 全体でκ=0.48,「陰性」でκ=0.55,「陽性」でκ=0.61と著しい再現性の改善が認められた. しかし「疑陽性」についてはκ=0.22, 観察者間で依然として混乱のあることが示された. これは, 内膜増殖性病変における「疑陽性」の取り扱い基準が確立していない現状を反映しており, 早急なコンセンサス作りが必要である.

Cytological findings of endometrial adenocarcinoma.

分化型体部腺癌の細胞所見を検索するために, 分化型腺癌10例, 低分化型腺癌6例, 非癌 (内膜増殖性病変を認めずかつ最近子宮内操作をうけていないもの) 20例の子宮内膜吸引細胞像を比較検討し, 以下の結果を得た.(1) 分化型例で低分化型例と比べ出現頻度の低かった所見は, 細胞の乳頭状配列・顆粒状クロマチン・赤色肥大核小体・核小体周囲の透明化像などであった.(2) 分化型例では, 低分化型例と比べ, 核の増大と大小不同が軽度のものが多く, なかには核の増大がほとんどないものもあった.(3) 分化型例でも, 低分化型例と同様に,(4) で述べるクロマチン所見とともに核重積性と核縁不整が高頻度に認められた.(4) 分化型例と低分化型例で例外なく認められ, 非癌例でほとんど認められない所見は,「クロマチンが核内に密に充満すること」および「クロマチンの不規則凝集像がみられること」であった.以上の所見の中で,(1),(2) は分化型腺癌を悪性と判定するために有用とはいいがたいが,(3),(4) は分化型腺癌を悪性ないしは悪性疑いとしてチェックアップするために有用な所見と考えられた.子宮内膜吸引細胞診では, これらのことに留意し分化型体部腺癌の検出に努める必要がある.

異型度の低い分化型子宮頸部腺癌の細胞像を検索するために, 細胞診上疑陽性であった分化型子宮頸部腺癌13例について, おもに核計測を中心として細胞診標本を検討し, 以下の結果を得た.1) 異型細胞にはside by side配列をなすところが認められ, クロマチンが増量していた.2) 核の大きさが増大していない異型細胞も認められた.3) 核の大きさが増大しているものも核の大きさが増大していないものも, 核の大小不同および多形性は軽度であった.4) 核の大きさが増大していないものでは, 核形がより細長いだ円の傾向を示した.以上のような異型を示す細胞の出現をみるときは, 分化型子宮頸部腺癌の存在を念頭におくことが必要と考えられる.
