A integração ensino-serviço na formação de enfermeiros e médicos: a experiência da FAMEMA

The scope of this study is to identify contributions and limits of teaching-service integration between the School of Medicine of Marilia and the Municipal Health Department of Marilia for academia, for the health teams and for the community. The study involved cross-sectional research using a quantitative approach. A questionnaire with 20 affirmations structured around a Likert scale was applied to students, professors and collaborating professors with a total of 183 participants. A trend for a positive evaluation of teaching-service integration for professional education was revealed. As regards contributions of teaching-service integration to the community, the positive evaluation was close to 50%. The items referring to contributions to the health teams were the ones that had the highest number of negative responses. Data indicate that it is necessary to move forward such that teaching and service are considered interdependent processes with possibilities of joint reflections.

Nicotine dependence in the mental disorders, relationship with clinical indicators, and the meaning for the user

OBJECTIVE: to identify the degree of nicotine dependence among patients with schizophrenia and other mental disorders hospitalized in a general hospital, correlating these indices with clinical indicators and the meaning for the user. METHOD: the study was performed in the psychiatric unit of a general hospital, interviewing 270 patients with mental disorders using a questionnaire and the application of the Fagerstrom test. A descriptive statistical analysis of the data and thematic analysis of the content were performed. RESULTS: among the 270 patients with mental disorders, 35.6% were smokers; of whom, 53.2% presented high or very high nicotine dependence. Of the 96 smokers, 32 (33.3%) were schizophrenic, among whom, 59.4% presented high or very high dependence. Higher levels of dependence were also found among the 59 elderly people (61.5%) and 60 subjects with somatic comorbidities (62.5%). Meanings of smoking for the subjects: helps to forget problems and face daily conflicts; alleviates side effects of the medications; self-control; distraction; part of life. CONCLUSION: more intense tobacco dependence among schizophrenic patients is justified due to it helping them to cope with the difficulties of the disease. The Meaning of Smoking for Patients with Mental Disorder

When patients smoke cigarettes in psychiatric services, it brings to the forefront current ethical and political dilemmas. This study aims to explore the meaning attributed to smoking by mental health patients who smoke and who are hospitalized in a psychiatric ward of a general hospital. This qualitative descriptive study was conducted with 96 smokers who were hospitalized in a psychiatric ward in Brazil. Semi-structured interviews, test of nicotine dependence, and content thematic analysis were carried out. The results show that tobacco has an important role in the lives of psychiatric patients. The meanings they attribute to tobacco use are related to overcoming difficulties that are consequential of mental disorders and of side effects caused by their treatments.

CUIDADOS DE ENFERMAGEM MAIS VALORIZADOS E FREQUENTES DURANTE INTERNAÇÃO PSIQUIÁTRICA

Objetivo: identificar os cuidados de enfermagem mais valorizados e frequentes na assistencia aos pacientes psiquiatricos, internados em hospital geral, segundo percepcao dos portadores de transtornos mentais e enfermagem. Metodos: estudo descritivo de abordagem quantitativa. Participaram 27/84 pacientes, 6/6 enfermeiros e 10/14 tecnicos enfermagem de unidade de internacao psiquiatrica. Realizadas entrevistas a partir de 13 fotos de cuidados de enfermagem. Elas eram embaralhadas e entregues aos participantes para que as ordenassem segundo importância e ocorrencia. Analise descritiva. Resultados: os participantes discordaram sobre os cuidados mais importantes: enfermeiros (50%: “passagem plantao”), tecnicos (40%: “ouvir atentamente”), (37%: “apoio espiritual”). Embora o apoio espiritual tenha sido a intervencao mais valorizada pelos pacientes, foi a menos frequente na opiniao dos tres grupos(76,7%). Conclusao: o cuidado de enfermagem ocorria de modo generalista com enfase em cuidados tecnicos basicos. E preciso considerar a opiniao dos usuarios, pois o cuidado que eles mais valorizavam era o menos implementado.

Aspectos da integração ensino-serviço na formação de enfermeiros e médicos

This qualitative research aims to identify advances and challenges of teaching-service integration in nursing and medical education between the Marilia Medical School and Marilia Municipal Health Secretary. Data were collected from accounts given by teachers, students and health professionals. The analysis was based on the hermeneutic-dialectic method. The results showed that teaching-service integration allows early integration of the student in the workplace, guides the construction of knowledge in professional practice, supports the exchange of information and increases the quantity and quality of health actions aimed at the community. The identified challenges were a lack of recognition of the students role by health team members, a lack of management support for the development of Family Health Strategy principles and a lack of definition of academia and service roles in partnership contracts. Suggestions for the improvement of the integration included expanding the insertion of the collaborating teacher in academic activities, improving the management of Primary Health Care and reviewing the partnership agreement. These advances are needed to enhance co-responsibility in the process of in-service learning.
