Adaptation of dementia screening for vision-impaired older persons: Administration of the Mini-Mental State Examination (MMSE)

In epidemiologic field studies on the prevalence and incidence of dementia the problems associated with the cognitive testing of visually impaired individuals are rarely discussed. In the Leipzig Longitudinal Study of the Aged (LEILA 75+) a version of the Mini-Mental State Examination for the visually impaired (MMSE-blind) was employed from which all items requiring image processing had been omitted. To be able to interpret the test results and include vision-impaired individuals in the field study, the scores for the full MMSE were estimated by conducting linear transformation of the scores obtained on the MMSE-blind. The method of linear transformation is based on certain theoretical assumptions that are examined in this article. Linear transformation of scores has proved to be a valid procedure only for individuals with very high or very low cognitive performance. Thus, evaluation of the estimated full MMSE scores based on the norms for the original MMSE is not recommended. A blind version of the MMSE with age- and education-specific norms that has been validated as a screening tool for dementia is therefore presented.

Determinanten des Psychopharmakagebrauchs in Alten- und Altenpflegeheimen

Zusammenfassung Neben der hohen Prävalenz psychischer Krankheiten bei Bewohnern von Alten- und Altenpflegeheimen wurde in der Literatur auch auf andere Ursachen für den vermehrten Psychopharmakagebrauch in stationären Pflegeeinrichtungen hingewiesen. Im Rahmen der vorliegenden Arbeit wird an einer Stichprobe von 560 Alten- und Altenpflegeheimbewohnern der Einfluss individueller Merkmale der Bewohner und institutioneller Bedingungen auf den Psychopharmakagebrauch untersucht. Es ergaben sich signifikante Zusammenhänge hin-sichtlich Alter, Geschlecht und bestimmter Verhaltensauffälligkeiten (Agitiertheit, Schlafprobleme mit nächtlichem Stören) der Bewohner. Darüber hinaus unterschied sich der Gebrauch psychotroper Substanzen zwischen den einzelnen Heimen signifikant, was auf einen Einfluss institutioneller Merkmale auf den Psychopharmakagebrauch hinweist.¶   Ziel weiterer Forschungsbemühungen sollte ein umfassender Einblick in das Bedingungsgefüge der individuellen und institutionellen Einflussfaktoren auf den Psychopharmakagebrauch sein, um daraus gezielte Interventionsvorschläge zur Optimierung der Behandlungspraxis abzuleiten.Summary A large proportion of the residential and nursing home population is mentally ill. The causes for increased prescription of psychotropic drugs other than mental disorders have been discussed. This study presents data on the role of the individual‘s characteristics and institutional conditions on psychotropic drug use in residents of residential and nursing homes. Therefore the psychotropic drug use of 4 old-people’s homes in Leipzig (Germany) was reviewed. Sociodemographic characteristics and a number of behavioral patterns of the residents were examined. Regression analysis revealed associations of age, gender and certain behavioral patterns (e.g., agitation, insomnia with nightly disturbances) and psychotropic drug use. Beyond that, psychotropic drug use varied among the 4 institutions‘ practices. Further research should be aimed at gaining insight into the interplay of these individual and institutional influences on psychotropic drug use in order to develop specific interventions to optimize treatment.

[Future psychotherapists? Vocational plans and motivation for choosing psychotherapy as a career in german psychology students].

BACKGROUND in Germany psychology students can be seen as the major personal resource in psychotherapy. Nevertheless there are few studies on their vocational plans and their interest in psychotherapeutic training. METHOD 480 psychology students completed a self-developed questionnaire on their career expectations. RESULTS 90% of respondents report interest in clinical work and psychotherapeutic training. Most frequently they mention improving therapeutic competencies and career options as pros and current training requirements as cons. Their theoretical orientation (38% behaviour therapy, 19% psychodynamic therapy) is associated with their psychotherapeutic knowledge, study conditions and respondents characteristics. CONCLUSIONS psychology students consider working and training conditions for psychologists and psychotherapists, when thinking about their future career.

[Wishes of nursing home residents concerning their life situation--results of a qualitative study].

Zusammenfassung. Im Hinblick auf eine zufriedenstellende Gestaltung der Lebenssituation alter Menschen in Altenpflegeheimen wurden Bewohner danach befragt, wie sie ihre aktuelle Lebenssituation im Heim einschätzen und welche Wünsche sie bezüglich dieser Lebenssituation haben. Der Untersuchung liegt eine repräsentative Stichprobe von n = 1656 Bewohnern von 24 Altenpflegeheimen einer ostdeutschen Großstadt zugrunde. Mit Hilfe einer inhaltsanalytischen Vorgehensweise wurden verschiedene Bedürfniskategorien der Bewohner herausgearbeitet und in ihrer Bedeutung für die Gestaltung und Organisation von Pflege im Heim analysiert. Im Ergebnis wurde deutlich, dass aus der Sicht der Heimbewohner vor allem Verbesserungen in der psychosozialen Betreuung in den Altenpflegeheimen erforderlich sind.Summary.Objektive:This study examined the wishes of nursing home residents concerning their life situation in the nursing home.Methods: Using a qualitative study design, a representative sample of nursing home residents (n = 1656) of 24 nursing homes in a city situated in the eastern part of Germany were interviewed.Results:The analyses of residents’ wishes lead to major domains such as the quality of care, interpersonal contact, architecture and organization of the house, diversification, financial support, as well as themes like health and death and the wish to leave the nursing home. Residents focus on an individualized approach to care.Conclusions:Nursing home residents’ views support the need to improve the psychological and social aspects of the quality of care. Implications of and consequences for the organization of care and concepts of nursing are discussed.

Demenzkranke in der Pflegeversicherung

Zusammenfassung: Im Rahmen einer bevolkerungsreprasentativen Untersuchung zur Pravalenz der Demenz («Leipziger Langzeitstudie in der Altenbevolkerung») wurden bei 1265 uber 75-jahrigen Personen neben der Untersuchung hinsichtlich einer Demenz auch Informationen zu den Pflegestufen erhoben. In der hier vorgestellten Analyse steht die Berucksichtigung der Demenzkranken im Rahmen der Pflegeversicherung im Mittelpunkt. Mittels multivariater Datenanalysemethoden wurde der Einflus verschiedener Variablen (Mobilitat, Demenzgrad, Alter, Geschlecht und Wohnort) auf die Beantragung einer Pflegestufe fur Demenzkranke untersucht. Die Ergebnisse zeigen, das uber ein Drittel aller Demenzkranken im Rahmen der Leistungen der Pflegeversicherung unterversorgt sind, d. h. entweder keinen Antrag stellen oder bei Antragstellung unangemessen eingestuft werden. Gering gehbeeintrachtigte Demente stellen signifikant seltener einen Antrag auf Pflegestufe als gehbeeintrachtigte Demente und werden signifikant haufiger unangemessen h...
