Is early diagnosis of hypertension a function of cuff width

This study followed blood pressure in 11 subjects 5 years after slight or established hypertension had been revealed using a cuff of the correct width (appropriate to arm circumference); at that time, the use of a standard-width cuff (12 cm) had shown a blood pressure within the normal range. For the present study, blood pressure was determined indirectly under very strict conditions so that a faithful comparison between the readings with both cuffs could be achieved. An important underestimate of blood pressure was detected in thin arms when the standard width cuff was used. The present measurements showed a consistent increase in blood pressure with standard-width cuff readings, confirming the hypertension that had been identified using the correct cuff width 5 years previously. These results support our previous hypothesis that early diagnosis of hypertension can be masked by the standard-width cuff, particularly in lean persons.

According to the International Council of Nurses the measurement of blood pressure is the procedure most performed by nurses in all the world. The aim of this study is to analyse the polemical aspects of instruments used in blood pressure measurement. FATORES QUE INFLUENCIARAM ALUNOS INGRESSANTES NA ESCOLA DE ENFERMAGEM DA USP, EM 1981, NA ESCOLHA DA ENFERMAGEM COMO OPÇÃO PROFISSIONAL

O estudo objetivou caracterizar alguns aspectos socio-economicos da populacao ingressante na Escola de Enfermagem da Universidade de Sao Paulo e verificar quais as opcoes destes alunos nas diversas vezes em que fizeram exame vestibular. A populacao estudada foi de 51 alunos que cursavam a disciplina Introducao a Enfermagem. O instrumento foi um questionario, onde foram abordados itens relacionados aos motivos que influenciaram a escolha da enfermagem como opcao profissional. Concluiu-se que a maioria dos alunos e do sexo feminino, de 18 a 20 anos, procedente da Capital e Grande Sao Paulo, proveniente de familias de variadas faixas economicas da populacao; 47% dos alunos apontaram enfermagem como a profissao que gostariam de exercer, embora tivessem prestado vestibulares para outras carreiras, preferencialmente medicina. Mais de 60% dos alunos pretendem realizar nova tentativa em exame vestibular. Os motivos mais citados que influenciaram a escolha profissional destes alunos foram: interesse pelas ciencias biologicas, desejo de ajudar pessoas e preocupacao com o nivel de saude da populacao.

REFLEXÕES SOBRE A RESPONSABILIDADE DO ENFERMEIRO NA ADMINISTRAÇÃO DE MEDICAMENTOS

This paper is intended to stress the nurses responsability in therapeutic drugs administration, mainly as related to her (or his) Knowledge of the pharmacological principles involved in drugs prescriptions. The Nursing Process regarding drug and client/patient assessment is herein presented as well as some examplie of nursing diagnosis. Planning and evaluation of nursing actions are also discussed and other issues are stressed that require a high degree of reasoning and research in the field.This paper is intended to stress the nurses responsibility in therapeutic drugs administration, mainly as related to her (or his) Knowledge of the pharmacological principles involved in drugs prescriptions. Blood pressure was measured in 103 individuals using a 0.40 cuff width and 0.80 cuff length on both segments and followed by a measure with a usual standard cuff after one minute. Arm and forearm circumferences were measured at the middle point. Results: Significant differences indicated overestimation of diastolic blood pressure in the forearm. These differences were lower than differences reported in other studies. Korotkoff sounds were improved after Forsberg‘s maneuver. Conclusion: Although, we found an overestimation of diastolic blood pressure in forearm, other studies suggest that the use of appropriate cuff sizes in the forearm derive more accurate measures of diastolic blood pressure. Adolescent hypertension identified with correct cuff and its cardiovascular and gestational problems after 29 years

Objective to identify, in 2011, rates of hypertension, cardiovascular and gestational problems in subjects presenting high blood pressure in 1982, when correct cuff size was used, according to the American Heart Association Arm Circumference/Cuff Width ratio of 0.40. Methods high blood pressure was defined in 2011 as systolic = 115 mmHg and diastolic = 80mmHg, resulting in 20 subjects between 39 and 43 years old. (Risk Group). They were compared to 20 subjects from the original sample with lower blood pressure values (Control group). Results the rates of hypertension, cardiovascular and gestational problems were significantly higher (Fisher: p=0.02) in the Risk Group, with one case of cardiovascular death. Our findings arouse speculations about whether, if a proper cuff had been used in clinical practice, the complications and death could have been avoided. Conclusions data suggest compliance with the use of cuff width corresponding to 40% of arm circumference, despite polemics concerning cuff availability and difficulties of using many sizes.OBJETIVO: identificar, no ano 2011, indices para hipertensao, problemas cardiovasculares e gestacionais em sujeitos com pressao elevada, detectada com manguito correto em 1982, quando a razao entre Circunferencia Braquial e Largura do Manguito de 0,40, proposta pela American Heart Association, foi aplicada. METODO: definiu-se como pressao elevada em 2011 sistolica =115mmHg e diastolica =80mmHg, resultando em 20 sujeitos com 39-43 anos (grupo de risco), que foram comparados a 20 com niveis menores que esses em 1982 (grupo controle). RESULTADOS: indices de hipertensao, problemas cardiovasculares e gestacionais foram significantemente mais elevados (Fisher: p=0,02) no grupo de risco, com uma morte cardiovascular. Os achados, aqui, levam a indagar se, caso um manguito apropriado tivesse sido usado na clinica, complicacoes e morte teriam sido evitadas. CONCLUSOES: os dados indicam atendimento a largura do manguito correspondendo a 40% da medida da circunferencia braquial, apesar das polemicas discussoes sobre dificuldades na disponibilidade e uso de diversos manguitos.

Communication Strategies Of The Nursing Team In The Aphasia After Cerebrovascular Accident

This is an exploratory, cross-sectional study of quantitative design that aimed to identify the communication strategies used and reported by the nursing staff in the care of aphasic patients after a stroke. The techniques used were the participant observation and interviews with 27 subjects of the nursing staff of neurological units in a general hospital. The most frequently mentioned strategies were gestures (100%), verbal communication (33.3%), written communication (29.6%) and the touch (18.5 %). Among the observed strategies, the gestures reached 40.7% and the touch was present in all situations, given its instrumental character essential to care. The findings show lack of knowledge of nonverbal, proxemics , kinesics and tacesics communication. No significant differences were observed among the professional categories depending on the length of experience with respect to the strategies reported by members of the nursing staff in the care for aphasic patients.Estudo exploratorio, transversal, de delineamento quantitativo, cujo objetivo foi identificar estrategias de comunicacao referidas e usadas pela equipe de enfermagem durante o cuidado de pacientes afasicos apos acidente vascular encefalico. Foram utilizadas a tecnica de entrevista e a observacao participante com 27 sujeitos da equipe de enfermagem de unidades neurologicas de um hospital geral. As estrategias mais referidas foram os gestos (100%), a comunicacao verbal (33,3%), a comunicacao escrita (29,6 %) e os toques (18,5%). Entre as estrategias observadas, os gestos atingiram 40,7% e o toque esteve presente em todas as situacoes, dado seu carater instrumental imprescindivel aos cuidados. Os achados indicam desconhecimento da comunicacao nao verbal, proxemica, cinesica ou tacesica. Nao foram observadas diferencas significativas entre as categorias profissionais em funcao do tempo de experiencia com respeito as estrategias referidas pelos membros da equipe de enfermagem para cuidar do paciente afasico.Estudio exploratorio de corte transversal, cuantitativo, cuyo objetivo fue identificar estrategias de comunicacion utilizadas por el equipo de enfermeria en el cuidado de pacientes afasicos despues de un accidente cerebrovascular. Se utilizaron las tecnicas de entrevistas y de observacion participante en 27 sujetos del equipo de enfermeria de unidades de neurologia de un hospital general. Las estrategias mas referidas fueron gestos (100%), comunicacion verbal (33,3%), comunicacion escrita (29,6%) y los toques (18,5%). Entre las estrategias observadas, los gestos alcanzaron el 40,7 % y el toque estaba presente en todas las situaciones dado su caracter instrumental esencial para cuidar. Los resultados indican un desconocimiento de la comunicacion no verbal, proxemica, kinesica o haptica. No se observaron diferencias significativas entre las categorias profesionales en funcion del tiempo de experiencia con respecto a las estrategias mencionadas por los miembros del personal de enfermeria para el cuidado de los pacientes con afasia.
