Diagnósticos de enfermagem em terapia intensiva: mapeamento cruzado e Taxonomia da NANDA-I

Objective: to identify nursing diagnoses in intensive care unit (ICU) patients by means of a cross-mapping of terms contained in nursing records with the NANDA-I taxonomy. Method: an exploratory, descriptive study with a retrospective analysis of nursing records in 256 medical records of patients that were hospitalized in the general ICU of a hospital in the western border of the state of Rio Grande do Sul. Terms indicating conditions demanding nursing interventions were collected from the records; cross-mapping of these terms with the NANDA-I taxonomy diagnoses was performed and confi rmed in a nursing focus group. Data were analyzed through descriptive statistics. Results: a total of 832 terms and expressions referring to 52 different diagnoses in 9 of the 13 domains of the NANDA-I taxonomy were identifi ed. Conclusion: the present study enabled the identifi cation of nursing diagnoses in patients hospitalized in ICUs, affecting care management, the training process of experts in the area, and information systems.

Bicarbonate buffered dialysate and replacement solutions for CRRT: effect of crystallization on the measured levels of electrolytes and buffer

The aim of this study was to investigate the impact of the addition of calcium to bicarbonate solutions for CRRT. We tested single bag (SB; bicarbonate and calcium mixed 24 h before testing) and double bag (DB) solutions (mixed immediately before), with and without the addition of 4 mEq/l acetate. Prescribed calcium varied from 0-5 mEq/l. In all test solutions prepared with calcium 5 mEq/l there was a decrease in the measured calcium concentration. SB solutions presented lower concentrations of calcium, compared with DB solutions. When the prescribed calcium concentration was increased, there was a parallel increase in calcium deficit (prescribed-measured). The prescribed calcium showed a negative correlation with sodium and potassium and a positive correlation with pCO2. We also found a positive correlation between calcium deficit and pCO2 (r = +0.59; P < 0.001). The crystallization, as measured by the weight of the crystals, was greater in the SB solutions when compared to the DB solutions (17.7 ± 7.0 mg versus 9.1 ± 1.8 mg, n = 14; P = 0.01). The crystallization correlated with the measured concentration of calcium (r = -0.62; P = 0.02), and pCO2 (r = +0.75; P = 0.002). We also observed a negative correlation between the pH, and the pCO2 (r = -0.82; P < 0.001). Our results suggest that the use of bicarbonate solutions containing calcium as replacement fluids for CRRT is a potentially unsafe procedure.

Projeto Terapêutico Singular e Processo de Enfermagem em uma perspectiva de cuidado interdisciplinar

OBJECTIVE To analyze the single therapeutic project and the nursing process regarding its specificities and intersection points from the interdisciplinary care perspective. METHODIntegrative review of the literature from articles available in the Lilacs, SciELO, MEDLINE and PubMed databases, published in Portuguese, English and Spanish from 2005 to 2015. RESULTS It was identified 23 articles. From these, 17 on the nursing process, six on the single therapeutic project and one about multiprofessional residency. From the analysis, their specificities and intersection points that describe the alignment and similarities between them were identified in the primary and mental health cares. CONCLUSIONS The single therapeutic project and the nursing process are aligned in health practices in primary and mental health cares. The multiprofessional residency allows this alignment among them, and the nurse contributes to the interdisciplinary care with the nursing process.

PROCESSO DE ENFERMAGEM: DESAFIOS E ESTRATÉGIAS PARA SUA IMPLEMENTAÇÃO SOB A ÓTICA DE ENFERMEIROS

A utilizacao do Processo de Enfermagem (PE), na pratica assistencial, constitui-se em importante metodo de organizacao do trabalho de enfermagem, pois permite organizar e planejar as acoes de enfermagem a partir da tomada de decisao do enfermeiro. Objetivou-se conhecer os desafios enfrentados por enfermeiros para implementacao do PE e possiveis estrategias para sua implementacao. Trata-se de um estudo exploratorio descritivo, com abordagem qualitativa, realizado em um hospital geral da fronteira oeste do RS, Brasil, no segundo semestre de 2013. A populacao foi composta por enfermeiros assistenciais atuantes na instituicao em estudo. Os resultados deste estudo apontaram duas categorias denominadas Desafios enfrentados para a utilizacao do PE: no ensino e na pratica profissional; e Estrategias para implementacao do PE. Conclui-se que os enfermeiros compreendem o PE como um metodo cientifico e estrategico para qualificacao da pratica assistencial. Reconhecem que sua utilizacao na pratica contribui para a autonomia e valorizacao da profissao. Descritores: Processos de Enfermagem; Cuidados de Enfermagem; Enfermagem.
