Effect of Magnesium Deficiency on Lipid Metabolism in Uremic Rats

The effects of acute magnesium deficiency on lipid metabolism were examined in five-sixths nephrectomized uremic rats and sham-operated rats. Three weeks after the surgery, both groups were divided into two subgroups. Half of the uremic and sham-operated rats received a magnesium-deficient diet. The rest of the experimental animals received a control diet. After 2 weeks on this regimen, all animals were sacrificed. In uremic rats, magnesium deficiency increased serum triglyceride levels and decreased high-density lipoprotein cholesterol levels as in sham-operated rats. Total serum cholesterol levels were higher in uremic rats than in sham-operated rats with or without magnesium deficiency. Serum free fatty acid levels were increased only in uremic rats with magnesium deficiency. These results suggest that magnesium deficiency worsens several parameters of lipid in uremic rats.

Effect of 1,25-dihydroxyvitamin D3 on intestinal disaccharidases in uremic rats.

The effect of 1,25-dihydroxyvitamin D3 (1,25-D) on the activity of intestinal disaccharidases, maltase, sucrase, trehalase and lactase, was studied in five-sixths nephrectomized uremic rats. In uremic rats, maltase, sucrase and trehalase activities were lower than in sham-operated rats. Administration of 1,25-D produced significant improvement of maltase, sucrase, trehalase and lactase activities in uremic rats. These results suggest that activities of intestinal disaccharidases are reduced in uremic rats and these activities are normalized after 1,25-D administration.

Studies on amyloid deposition in the parathyroid glands of long-term dialysis patients.

長期透析患者で副甲状腺摘除を受けた50例の副甲状腺について検索し, 50例中47例において副甲状腺組織にアミロイド沈着を認めた. この47例中9例は手根管症候群の手術を受けており手根部横靱帯にアミロイド沈着が認められた. 副甲状腺の組織学的検索では, 被膜および結節間の隔壁, 小血管壁にアミロイド沈着が認められた.最近このアミロイドーシスの主要構成蛋白がβ2-microglobulin (β2-MG) であることが確認されている. 本研究でもこのアミロイドは過マンガン酸カリウム処置に抵抗性であり (非AA蛋白), 免疫組織化学的検査 (peroxidase-antiperoxidase法, immunofluorescence法) で抗ヒトβ2-MG抗体と反応した. これらの成績は透析アミロイドーシスは全身的に生じ得るし, 種々の臓器にアミロイド沈着が生じている可能性を示唆している.

Evaluation of overhydration in diabetic patients at the beginning of regular dialysis treatment.

糖尿病性腎症由来の腎不全患者においては, 慢性腎炎由来の腎不全患者よりも, 慢性透析導入時に溢水状態にある患者の多いことが知られている. これらの溢水状態にある糖尿病性腎症患者の, 慢性透析導入時の病態と治療成績を検討した. 本院において慢性透析に導入した糖尿病性腎症患者69例 (男40例, 女29例, 平均年齢59歳, 平均糖尿病罹病期間16年) を, 導入時点において溢水を伴う症例と溢水を伴わない症例に分け検討した. 溢水症例としては肺水腫23例, 心胸比の拡大24例, 胸水貯留13例, 腹水貯留14例, 心嚢液貯留7例を認めた. これらのいずれかを有する症例33例を溢水症例とした.溢水症例は非溢水症例よりも導入時のクレアチニン, 血中総蛋白, 酸素分圧が低値であり, 1日尿蛋白の排泄量が高値であった. 一方, 性, 年齢, 糖尿病罹病期間, インスリン使用に関しては両群間に差はみられなかった. 導入3か月目までの死亡は6例にみられ, 全例溢水症例であった. 死因は, 心不全2例, 感染症2例, 脳出血, 消化管出血各1例であった. 溢水症例においては, 直接穿刺による緊急透析例が多く, 導入後10日間における透析回数, 透析時間, 総除水量が多かった. 溢水状態にある症例に対してはより早期の導入を心がけた, 1984年以後の症例の導入期死亡例は23例中3例と, それ以前の10例中3例より若干減少した.以上, 溢水状態にある糖尿病性腎不全患者は, 温水のない症例よりも導入時死亡の頻度が高く, より早期に透析療法に導入し, 溢水の是正をはかる必要があると考えられた.

糖尿病性腎不全で4回の血液透析後に消化管出血で死亡, 剖検により膵ラ氏島にβ2-MG由来のアミロイド沈着を認めた症例を報告する. 68歳, 女性, 約10年前初めて糖尿病と診断され食事療法を行っていた. 約2年前よりインスリン療法開始, 蛋白尿出現. 4か月前より高窒素血症, 浮腫のため入院, 保存的治療を行っていたが高窒素血症増悪のため血液透析を開始, 計4回の透析を行うも消化管出血で死亡. 剖検膵のラ氏島にアミロイド沈着がみられ, PAP法による免疫組織化学検査でアミロイド蛋白はβ2-MGを含むことが証明出来た. 他の全身臓器にはアミロイド沈着はみられなかった. 長期透析症例の全身臓器のアミロイド沈着の報告はあるが, 短期の糖尿病性腎不全の膵ラ氏島にβ2-MG由来のアミロイド沈着がみられたのは稀で珍しく貴重な症例と考えられた.

Clinical study of gastrointestinal bleeding in patients with chronic hemodialysis.

1974年から1989年までに本院において経験した, 消化管出血を合併した慢性透析患者32例, 33回, 平均年齢53歳, 男性21例, 女性11例, 平均透析歴26か月を対象として, 慢性透析患者の消化管出血に関する病態および治療成績の検討を行った. 消化管出血の原因としては, 胃潰瘍14例, 胃癌2例, Mallory-Weiss症候群2例, 腸結核2例, 食道静脈瘤, 出血性胃炎, 十二指腸潰瘍, 下行結腸ポリープ, 下行結腸潰瘍, S状結腸癌各1例, 不明7例であった. 死亡例は11例 (死亡率34%) で, 消化管出血の持続による者が8例, 感染症2例, 房室ブロック1例であった. 6例に開腹手術による止血が試みられたが, 4例は術後合併症のために死亡した. 1984年以降の症例に対しては, メシル酸ガベキサートやメシル酸ナファモスタットを使用する非ヘパリン透析により十分な血液浄化を行い, 上部消化管病変に対してはH2受容体拮抗剤の使用, さらにより早期から高カロリー輸液による十分な熱量の補給に心掛けた. その結果, 1984年以降の症例の死亡率は11%と, それ以前の死亡率64%より減少した.以上, 非ヘパリン透析による十分な透析, 高カロリー輸液による栄養管理, 上部消化管病変に対するH2受容体拮抗剤の使用等を総合的に行うことにより, 慢性透析患者における消化管出血に対する治療成績の改善が得られた.

A case of polycythemia vera complicated by chronic renal failure under hemodialysis requiring parathyroidectomy for secondary hyperparathyroidisim

A case of polycythemia vera complicated by chronic renal failure under maintenance hemodialysis requiring parathyroidectory (PTH) for secondary hyperparathyroidism (2° HPT) is reported.A 62 year old female presented with 75000 white blood cells (WBC)/µl, 703×104 red blood cells (RBC)/µl, 23×104 platelets (PLT)/µl, hyperuricemia and hypertension in 1970 and the diagnosis of polycythemia vera was made. Hemodialysis was started in October 1974 for chronic renal failure. Blood cells in peripheral blood rapidly decreased in number after the beginning of dialysis, reaching the level of 10000∼20000 WBC/µl, and 150∼250×104RBC/µl. In August 1988, marked bone resorption in X-ray picture and high serum alkaline phosphatase and parathyroid hormone (PTH) noted along with 17400 WBC/µl, 370×104RBC/µl and 35.9×104PLT/µl.After subtotal PTX removing 3.21g parathyroid gland, serum PTH rapidly fell. At 3 months after PTX, WBC rose to 23600/µl, RBC 372×104/µl and PLT 94.0×104/µl. At 6 months, WBC was to 31000/µl, RBC 429×104/µl and PLT 78.0×104/µl, suggesting an inhibitory action of PTH on not only RBC, but also WBC and PLT.

Effect of 1,25-Dihydroxyvitamin D3 on Intestinal Glucose Absorption in Uremic Rats

The effect of 1,25-dihydroxyvitamin D3 (1,25-D) on the absorption of glucose in the small intestine was studied in five-sixths nephrectomized uremic rats and sham-operated rats. Four weeks after the nephrectomy, the uremic animals were divided into two groups: One group was given 1,25-D (300 pmol/kg body weight/day, three times per week) intraperitoneally, and the other was left untreated. One week after the 1,25-D treatment, an in vivo glucose absorption test was performed at 00.00 h in consideration of the circadian rhythm of glucose absorption. In untreated uremic rats, the glucose absorption rate was lower than in sham-operated rats. In 1,25-D treated uremic rats, the glucose absorption rate was higher than in untreated uremic rats and not lower than in sham-operated rats. These results suggest that the absorption rate of glucose of the small intestine is reduced in uremic rats and that it is recovered on treatment with 1,25-D.
