Mucoepidermoid carcinoma arising in Warthin's tumor of the parotid gland

Malignant transformation of Warthins tumor is extremely rare, although it is the second most common benign tumor of the parotid gland. We describe our experience of mucoepidermoid carcinoma arising in Warthins tumor of the parotid gland in a 64‐year‐old man. He had a swelling in the left parotid‐masseteric region. The removed tumor was well encapsulated and histopathologically comprised Warthins tumor and low‐grade mucoepidermoid carcinoma. The mucoepidermoid carcinoma infiltrated lymphoid stroma of the Warthins tumor, but capsular invasion is not found. Considering the clinical course and physical examination, the lesion was diagnosed as mucoepidermoid carcinoma arising in Warthins tumor because its features filled the criteria of malignant transformation of Warthins tumor. There has been no recurrence or distant metastasis for 19 months. Histological change from Warthins tumor to mucoepidermoid carcinoma may be implicated in squamous or goblet cell metaplasia of epithelial cells. It is considered that the neoplastic cells of Warthins tumor acquire malignant genotypes simultaneously with this dual differentiation.

Prevention of death of axotomized hypoglossal neurones and promotion of regeneration by chitin grafting.

Abstract1. Chitin is known to promote skin wound healing. In this study, chitin, prepared from Zuwai crab shell, was used as a bridge between the proximal and distal stumps of cut hypoglossal nerves in shrews. We compared the effects of chitin on the regeneration of transected right hypoglossal nerve axons, with those of porcine dermis, bovine dermal aterocollagen, and autologous nerve bundles.2. To assess the survival of neurones, the size of neuronal cell body, and number of motoneurones were determined in the absence of any bridged material and in the presence of porcine dermis, bovine dermal aterocollagen, chitin, or autologous nerve bundles as a bridge.3. Our results revealed a significantly better outcome in chitin and autologous nerve bridged groups; the size of neuronal cell body and number of hypoglossal neurones were higher than in the other groups. Chitin also enhanced the regeneration of neurones; the number of horseradish peroxide positive neurones indicative of repaired axonal processes was significantly higher in chitin and autologous nerve-bridged groups than in other groups.4. Our results demonstrated that the use of chitin sheet or autograft successfully prevented the death of severed neurones and promoted the regeneration of the lesioned nerve. Although the mechanisms underlying the effects of chitin are still unknown, chitin seems to be a potentially useful biocompatible material for nerve repair and regeneration.

A case of recurrent adult T-cell leukemia/lymphoma in the mandible.

ヒトT細胞白血病ウイルスI型 (HTLV-I) に関与するT細胞リンパ腫は成人性T細胞白血病/リンパ腫 (ATL/L) と呼ばれている。このたび成人性T細胞白血病 (ATL) 治療後に下顎骨に腫瘤体病変を形成したATL/Lの一例を経験したので, その概要を報告した。患者は69歳男性で, 左側下顎部の腫脹を主訴として当科を受診した。当科初診の22か月前, 成人性T細胞白血病にて化学療法を受けた既往があった。初診時口腔内所見として, 左側下顎小臼歯部に大きさ35×30×15mmの弾性硬の腫瘤を認めた。血液血清検査において, 抗HTLV-I抗体陽性であった。胸腹部CT検査の結果, 他の部位に病変はみられなかった。生検術の結果, 病理組織学的にびまん性多形細胞型T細胞リンパ腫と診断した。放射線療法および化学療法を行ったが, 治療開始から約5か月後に死亡した。

Advanced osteosarcoma of maxilla for which palliative treatment was usefull. Report of a case.

高度に進展した上顎骨肉腫の1例を経験した。患者は32歳女性で, 7年前に左上顎良性骨芽細胞腫の摘出術を受けていた。初診時, 巨大な腫瘤は左顔面全体に及んでいた。生検の結果は骨肉腫であり, MTX大量療法ならびに放射線療法を行ったところ, 腫瘍は縮小し, 姑息療法として減量手術 (腫瘍切除術) を施行した。術後, 腫瘍は静止的であったが, その後右上肺野, 右脳に転移が認められるようになった。右脳転移巣に対して定位的脳放射線療法を行ったが, 呼吸不全により死の転帰をとった。剖検の結果, 右上肺野の他に, 右脳, 心臓, 舌, その他に転移巣が確認された。

Clinicopathological analysis of the mucoepidermoid carcinoma of minor salivary gland.

小唾液腺に発生した粘表皮癌6症例について病理組織学的に検討した。症例はすべて1次症例で, 発症部位は硬口蓋が3例, 頬粘膜が2例, 下歯肉が1例であった。1991年W.H.O.分類にもとつく組織学的分類では, well differentiated typeが3例, poorly differentiated typeが3例で, poorly differentiated typeの1例に頸部リンパ節転移がみられた。治療は外科療法単独症例が5例, 外科・放射線・化学療法を併用した症例が1例であった。現在のところ全例において再発や転移は認められず, 治療後の経過は良好である。

Carcinoma of the lower alveolus and gingiva of a extreme elderly woman which dramatically responded to chemotherapy with UFT alone: Report of a case.

症例は84歳女性で, 左側下顎臼歯部の腫瘤および接触痛を主訴に受診した。病理組織学的診断は中等度分化型扁平上皮癌であった。高齢者であることおよび家族の在宅治療希望のため, UFT単独投与を行ったところ, 腫瘤は徐々に消失し, 投与開始4カ月後には臨床的にも組織学的にも腫瘍の消失がみられたUFT投与開始から6カ月後, 造血機能障害のため投与を中止したが, さらに1年1カ月後まで再発がなく, 経過良好である。本報告例は, 癌の化学療法の効果判定基準でcomplete response (CR) に相当し, 高齢者歯肉癌におけるUFTの有用性を示す症例と考えられた。
