A sucção não nutritiva do recém-nascido prematuro como uma tecnologia de enfermagem

Estudio experimental con un enfoque cuantitativo, cuyo objetivo fue demostrar que la succion no nutritiva es eficaz en el tratamiento del dolor durante la instalacion, por personal de enfermeria, de la CPAP nasal en bebes prematuros; y demostrar que el uso de la succion no nutritiva, concomitantemente a la instalacion de la CPAP nasal, puede ser considerada una tecnologia de enfermeria. La poblacion-objetivo consistio en 20 recien nacidos prematuros sometidos a la instalacion o reinstalacion de este artefacto, en un total de 30 procedimientos. Los recien nacidos fueron divididos aleatoriamente en dos grupos, control y experimental, en el que se ofrece la succion no nutritiva, lo mismo no ocurriendo con el grupo control. Las reacciones de dolor se midieron por la escala de NIPS. En 100 % de los procedimientos concomitantes con succion no nutritiva, los recien nacidos no sentiran dolor; y 100 % de los recien nacidos mostro dolor cuando no se le ofrecio la succion. Llegamos a la conclusion de que este procedimiento puede ser clasificado como una tecnologia de cuidado de enfermeria.This is an experimental study with a quantitative approach, whose goal was to demonstrate that non-nutritive sucking is effective in pain management during installation, by the nursing staff, of nasal CPAP in preterm infants; and to demonstrate that the use of non-nutritive sucking, concomitantly with the installation of nasal CPAP can be considered a nursing technology. The target population consisted of 20 preterm infants undergoing installation or reinstallation of this artifact, totaling 30 procedures. The newborns were divided randomly into two groups, control and experimental, in which non-nutritive sucking was offered, the same do not happening with the control group. The reactions of pain were measured by the scale of NIPS. In 100% of the procedures that occurred concomitant with non-nutritive sucking, newborns did not feel pain; and 100% of the newborns showed pain when such suction was not offered. We conclude that the procedure can be classified as a technology of nursing care.

Central venous catheterization in Pediatric and Neonatal Intensive Care Units

This descriptive, prospective cohort study aimed to analyze the process of central venous catheterization in neonatal intensive care and pediatric units; describe the variables related to study characterization, including admission unit, age, and sex; and to investigate related variables, such as catheter type, reason for insertion, number of lumens, insertion site, type of professional who performed the procedure, medication therapy infused, reason for withdrawal, length of time catheter was in situ, and mechanical and infectious complications. Data collection was performed with 82 charts in the intensive care units (ICUs) of the Instituto Fernandes Figueira. In the majority of cases, the indications for catheter insertion were prolonged drug infusion and total parenteral nutrition. Removal was predominantly required due to mechanical and infectious complications. THE CHILD VICTIM OF DOMESTIC VIOLENCE: LIMITS AND CHALLENGES FOR NURSING PRACTICE

Objective: to identify the main difficulties of the child submitted to the chemotherapy and discuss the nursing care to the child submitted to the chemotherapy. Methodos: a exploratory, qualitative, bibliographical study, accomplished at the Virtual Health Library (LILACS and BDENF), where was accomplished a pre-reading and selective reading, selecting 10 scientific productions, which was accomplished the interpretative reading and thematic analysis. Results: like this, was resulted in the following categories: the main difficulties of the child submitted to the chemotherapy, and the importance of the nursing care to the child submitted to the chemotherapy. Conclusion: the child underwent chemotherapy and his family need a continuous support, it is not easy to live or even monitor the entire process. It is important that the family and children are always well informed to minimize your anxieties and doubts, therefore, it’s ended that the nurse is an important mediator for the promotion of the health of the child submitted to the chemotherapy.Objetive: identifying what are the needs of these children when they are hospitalized as human growth and development, aiming to identify the nurse’s performing on the children’s hospitalization process, taking into account the emotional difficulties experienced by them. Methods: this study is exploratory, descriptive, bibliographic research was conducted with a qualitative approach, held in the Library of Health (LILACS and BDENF). After the data collection, an exploratory, selective, critical reading and a thematic analysis were performed. Results: emerging the following categories: Children’s emotional difficulties on the hospitalization process and the importance of nurse’s performing in order to decrease the effects caused by hospitalization. Conclusion: we conclude that the hospitalization and the consequents procedures constitute unpleasant and painful experiences to the children, causing fear, anguish, loneliness, sadness, etc. By having scarce and immature emotional resources, children’s response to these situations may be negative and traumatic. Therefore, the nurse with the team must value the humanized care, according to children’s emotional needs and not only on the technical procedure, creating mechanisms that may ease that suffering, this way, playful insertion, the play and family presence join as effective instruments in order to decrease the stress and make the hospitalization less traumatic.Objetivo: identificar o processo educativo dos enfermeiros no PSF, descrever o conhecimento e utilizacao dos enfermeiros no PSF dos jogos educativos, e analisar as possibilidades e dificuldades dos jogos educativos como uma nova estrategia de ensino. Metodos: pesquisa descritiva exploratoria, abordagem quanti-qualitativa, de campo, respaldada pela Resolucao no196/96, com enfermeiros atuantes em PSFs coordenados pelo Polo Sanitario Washington Luiz Lopes, no municipio de Sao Goncalo (RJ). Resultados: a pesquisa aponta para um grande interesse dos enfermeiros entrevistados em conhecer os jogos educativos e sua dinâmica, bem como de coloca-los em pratica. Conclusao: o presente trabalho descreve e discute uma nova estrategia educativa, pouco conhecida como instrumento facilitador na educacao.Objetivo: O presente estudo analisa historicamente formacao e exercicio profissional de enfermeiras diplomadas pela Escola de Enfermagem Lauriston Job Lane, do Hospital Samaritano, Sao Paulo. Metodo: Por intermedio da historia oral tematica, foram realizadas entrevistas com sete colaboradoras, que vivenciaram o ensino nesta Escola, das quais seis foram ex-alunas e uma ex-diretora. As entrevistas foram interpretadas a partir das categorias, formacao e exercicio profissional, enfocando as anotacoes de enfermagem. Resultados: Os relatos revelaram experiencias de mulheres inseridas em uma categoria profissional identificada como feminina, bem como o cotidiano da formacao e orientacao profissional nos anos 1960. A analise dos registros, na perspectiva historica, destacou a importância da anotacao na formacao profissional, reflexo de uma assistencia de enfermagem cientifica e qualificada desde suas origens preconizada pelo Modelo Nightingale. Conclusao: O estudo permitiu identificar a importância do ensino dos registros/anotacoes como instrumento de comunicacao em enfermagem e assim dimensionar suas transformacoes, conforme sugere a pesquisa em historia do exercicio profissional.

Impacto de uma portaria ministerial: aspectos da concretude social e política da enfermagem obstétrica

This qualitative study presents the impact of the Ministerial Edict (number 2.815/98) relative to the inclusion of a set of procedures created by the obstetrician nurses in Public Health System (Sistema Unico de Saúde--SUS). The objectives of this study are: present two excerpts which were published in a very popular newspaper and in a bulletin of the medical professional segment in 1998; analyze TV news which constitute different and tense discourses. The study identified two ideas, which were based on three pieces of news collected from the press. The first idea corresponds to the lack of visibility regarding the technical competence of obstetrician nurses in the social context. The second is related to the political acknowledgement of this service, which can be observed through the payment of obstetric nursing procedures according to a price list established by the Public Health System. Our final considerations are related to the social and political situation of obstetric nursing, in spite of the interventionist manipulation of society carried out by the media, which tries to create social-ideological discourses in order to obtain consensus and (re) produce truths.This qualitative study presents the impact of the Ministerial Edict (number 2,015/98) relative to the inclusion of a set of procedures created by the obstetrician nurses in Public Health System (Sistema Unico de Saude - SUS). O cuidado de enfermagem com o corpo sem vida

El presente articulo es un estudio descriptivo que tiene como objetivo proveer subsidios para la reflexion de un grupo de profesionales sobre el cuidado de enfermeria dado al cuerpo sin vida, a partir de una dinamica realizada en un hospital general. La referida dinamica, primero fue presentada en sala de clase durante la realizacion de una disciplina que forma parte del curriculo de licenciatura de la Escuela de Enfermeria Alfredo Pinto, de la Universidad Federal de lo Estado de Rio de Janeiro, y despues fue llevada al mencionado hospital, teniendo en cuenta la creatividad de los alumnos con ella vinculados. Se concluye que, al relacionar el arte de la escenificacion a las diferentes ciencias que inciden en el acto de cuidar, es posible abordar con estetica y belleza, un procedimiento tan arduo, cuanto fino y sutil, el cual forma parte del cotidiano de la practica de cuidar del equipo de enfermeria.

O "pacote" e a enfermagem (análise crítica de uma cena de morte)

This is a descriptive study based upon an experience related by a group of nursing students in their senior year at a public university in the city of Rio de Janeiro. As part of a course content in Methodological Perspectives in Nursing II, based on problem solving, the authors analysed a death scene which occurred during practical training, in a large public general hospital, in order to describe how nursing staff deal with such an event. The main goal of the study was to provide thought provoking material from different points of view--cultural, religious and ethical--concerning the subject of death and the professional nursing ethics code. The authors concluded that the such a theme must be discussed in the academic environment, but not only from a technical point of view, for the process of human interaction which is established during health assistance demands the provision of nursing care to the patient during his entire lifespan, of which death is the last stage.

Noise levels in a pediatric intensive care unit: an observational and correlational study

Aim: To measure noise levels in a pediatric intensive care customer unit and discuss the consequences of such noise in relation to professional actions. Methods: This is an observational and correlational study, performed in the pediatric intensive care unit of a federal hospital in Rio de Janeiro. We measured the noise level by decibel DEC-460 for five non-consecutive days and nights at five different hours. After a descriptive analysis of data, we performed a study of linear regression. Results: We identified an average of 62.64 dBA, with a standard deviation of 6.893 dBA and peak of 82.5 dBA during the daytime. The linear regression found that 44% of the variability of the noise is explained by the covariables. Discussion: The noise levels identified exceed recommendations of national and international organizations. We identified the professional actions and interactions as the main sources of noise. Conclusion: There is an urgent need to rethink this environment and establish educational strategies for noise reduction, making it safer and healthier.

Mobilização terapêutica como cuidado de enfermagem: evidência surgida da prática

The objective of this observational and descriptive study was to identify the behavior of relative parameters related to pulmonary mechanics of critical patients undergoing mechanic ventilation, after the nursing staff performed the technical procedure of decubitus position change. The accessible population, by non-probabilistic sampling, consisted of nine critical patients undergoing mechanical ventilation and their respective parameters related to pulmonary mechanics. Positive and negative alterations in pulmonary mechanics in all patients were identified, correlating with different positions, possibly associated with the change in decubitus position. Prevalence of lower back pain and physical inactivity: the impact of psychosocial factors in pregnant women served by the Family Health Strategy.

OBJECTIVE This study analyzed the impact of psychosocial factors on pregnant women with lower back pain and an associated lack of physical activity prior to pregnancy. METHODS The sample included 66 pregnant women who were randomly selected from a total of 84 patients in the waiting rooms of the Family Health Units in Cuitegí, Paraíba, from September to November 2009. An epidemiological questionnaire adapted from the Quebec Back Pain Disability Scale was used for data collection. The questions about back pain, physical activity, and psychosocial factors were emphasized. SPSS 16.0 was used for the data analysis. The prevalence of lower back pain and its relationship to gestational age, habitual physical activity, and psychosocial factors were studied using the descriptive statistics and relative percentages in the SPSS Crosstabs procedure. The odds ratio and 95% confidence interval for lower back pain were calculated. RESULTS The prevalence of lower back pain was 75%, which suggests that psychosocial factors were related to the presence of pain. Anxiety was reported in 42.8% of the women with lower back pain, and 38.7% of the women with lower back pain experienced physical fatigue at the end of the day. A higher percentage of pain (53%) was noted in the women who did not exercise prior to pregnancy. CONCLUSION Lower back pain prior to pregnancy is associated with lack of physical activity and with psychosocial factors in the Family Health Strategy patients of Cuitegí county.
